Fall to the walkway or working surface

EE pulled up to quarry office to sign in for the day. As EE stepped out of truck onto the shoulder of road EE slipped on ice exiting the truck. EE tried to grab door but fell to ground with shoulder/right arm pain and was taken to urgent care.

EE was prying on a mechanical lacing clip. EE lost grip of bar, the bar then bounced off of the belt coming towards EE and hit EE in the mouth resulting in a chipped left central incisor.

The employee was moving an 8'x19'x1/2"" sheet of steel assisted by a telehandler. The employee tried to slide the sheet of the forks in a pulling motion then felt a pop in the left bicep.

Miner was installing an impact roller on C-12, Miner was applying force to the roller with right hand and bracing self with left hand. When Miner released right hand the roller came in contact with the Miner's left hand causing a break to thumb and hand to become bruised. The Miner was wearing all required PPE including gloves per Company Policy.

Employee lifted box from desk top moved to short file cabinet surface of the same level, twisting in the process. Pulled muscle in back, limited lifting for 5 days then released to regular work.

Employee was changing an alternator belt on a surface haulage truck. He was releasing the tension a spring tensioner with a pry bar. In the process he lost control of the pry bar causing the pry bar to strike his left index finger between the pry bar and the frame causing a laceration and minor fracture

Employee was cutting old conveyor with torch, as he cut a piece of angle the angle had unseen stored energy causing it to strike the employee's right hand causing a cut and swelling in the knuckle.

He was removing access panels to remove a turbo charger from a wheel loader and heard and felt a pop in his arm. He reported the incident to his manager immediately. He and his manager continued to monitor his condition until 02/12/14 when he informed his manager that the pain had not improved and he wanted to seek medical treatment.

Employee exited equipment improperly, not maintaining three point contact. Upon exiting employee twisted ankle. Employee was diagnosed with a distal fibular fracture and a torn tendon.

An employee tripped over a piece of cut railroad track that is used as weight in a counterweight box. When the employee tripped, he fell forward and tried to catch himself on a 10"" pipe. When he reached for the pipe, he hyper-extended his right shoulder and felt a ""pop"".

Employee began exiting a chute after inspection when his hand slipped and he fell backwards against the side of chute, striking his back against one of the steps. The employee did not report the injury until 06/18/2011, the employee got out of bed, stretched, and felt an extreme amount of pain. Employee sought treatment and was diagnosed with a fractured rib.

Employee reported that while cutting rubber for a screen deck, the knife slipped and lacerated his right inner thigh. Employee went to the hospital where he received 7 sutures, was given a Tetanus shot, and was not given any written prescriptions or restrictions. Employee is to return in 10 days for suture removal.

Employee alleges that he struck his knee while moving around in the cab of a piece of equipment. The employee failed to report the injury until 11/14/2010 and then sought treatment with his personal doctor on 11/29/2010 where he was diagnosed with a torn meniscus, received a Cortisone shot, and was prescribed two (2) days off from work.

At the wet mill doing a belt splice on conveyor 33, repetitively climbing on and off the belt during the repair and twisted his right knee.

Replacing a return roller on C-45 conveyor. Attempting to put the return roller into its holder. Bending & stretching while lifting caused pain in lower back. **Medical with restrictions.**

Stepping down ladder of T-8 haul truck, bolt broke holding a rung on, EE held on with his left hand but fell down a couple of rungs to radiator shroud.

While pulling a conveyor belt (c-45) down with a come-along, the come-along released by itself, this allowed the belt & roller to push EE's hand upward, traping it between the roller & bracket.
