Absorption of radiations, caustics, toxic and noxious substances

Employee was clearing brush around electrical installations around the plant. They reported to the supervisor itching/burning eyes on 7/10. Employee rinsed eyes out and applied cooling packs but declined additional treatment. The following day 7/11 the employee was accompanied by Safety to the doctor where they were prescribed steroids.

Absorption of radiations, caustics, toxic and noxious substances

The employee was exposed to shotcrete on both legs and caustic burns at 3pm on 4/21. The employee informed the supervisor at 9am on 4/22, at which time the supervisor provided 1st aid. The employee did not mention the incident again until 6/9/25 and was sent to the doctor that day. The employee is off work until released by the doctor.

Fall from machine

While descending from a roof bolter, EE planted EE's right foot at the edge of the cab when EE lost EE's footing and missed the top step, causing EE to fall approx. 5 ft. onto EE's right shoulder. On 6/3/25 EE was evaluated at Ohio Health Work Health where EE was diagnosed with a strain and given pushing/pulling/lifting restrictions of 10lbs with right arm.

Caught in, under or between a moving and a stationary object

Employee was operating a jack leg installing bolts into the face of the high wall. The employee was installing the bolt at a downward angle. The socket slipped off the head of the bolt causing the jack leg to rotate forward and mash the operators hand between the machines handle and the highwall face.

Truck driver was walking on a blasting bench. It had rained so the bench was muddy and had some standing water. The driver stepped in a hole that was obscured by water and twisted left foot/ankle.

Fall from machine

Employee was working on a crusher and climbed onto the crusher's drive belt guard without the required fall protection. Employee slipped and fell 6 feet onto the crusher platform. The fall resulted in a fracture to the pelvic bone, the employee was released with restrictions.

Struck against a moving object

Operator in trackhoe using hydraulic hammer, secondary rock breaking. Heard a loud crack from above, dust flew and rock hit the cab. Lacerations on head, hands and left knee each requiring stitches. Admitted overnight for observation only.

Drowning

Non Employee and his friends trespassed onto an inactive section of the Quarry. They started swimming in a lake on the Quarry property. This person went under water and did not resurface. 911 responders arrived on site and recovered him from the water. Person was pronounced dead at 10:22 p.m. Drowning was the cause of death.

EE was making a repair to the belting on C-1 conveyor. He was using a utility knife to cut a piece of belting to use as a patch. He was kneeling on the conveyor catwalk and cutting the belt. As he was cutting the belt, the knife slipped and he struck his left thigh with the knife causing a laceration approximately 1 inch long by 3/8 inch deep.

Struck by flying object

Employees were adjusting a blow bar in a H.S.I. crusher. A 3lb. sledge type hammer was being used to knock the bar retaining wedges loose so the bar could be slid into position on the rotor. When an employee struck a wedge, a piece of metal broke off the hammer face and struck the employee in the right forearem. This caused a deep cut to the employee's arm.

Fall onto or against objects

Chute from 8'x20' screen to surge rock transfer conveyor plugged up w/material & had to be cleared. Once plug up was cleared EE & co-worker were walking under screen tower to haul trucks. A large wood block was on the concrete. EE saw the block & tried to step over it. He tripped & began to fall. He reached out & grabbed the side of the conveyor to keep from falling.

Fall to the walkway or working surface

EMPLOYEE WAS GREASING A LOADER WITH AN AIR GREASER LOCATED AT THE EQUIPMENT SHOP. THERE WERE SOME SHEEL CHOCKS AND A FEW STONES ON THE GROUND BETWEEN THE LOADER AND THE SHOP WALL. THE EMPLOYEE WAS WALKING BETWEEN THE LOADER AND THE SHOP WHEN HE TRIPPED OVER ONE OF THE STONES OR WHEEL CHOCKS AND FELL TO THE GROUND DISLOCATING HIS LEFT SHOULDER. HE WAS TAKEN TO THE HOSPITAL.

Struck by falling object

Working on V.S.I. crusher. Inspection doors opened to access table. While ee was reaching into the crusher, one of the inspection doors fell closed striking the ee on the hard hat. The ee began having neck pain & went to get checked by a physician. The physician gave no treatment at that time. 8/9/04 ee returned to physician w/neck pain. Medication prescribed.

EE WAS STANDING ON BOULDER AND SLIPPED, FALLING AGAINST TRUCK AND INJURING HIS SHOULDER. (EE WAS LOADING BOULDER WITH EXPLOSIVES TO SHOOT IT. GRABBED DOOR HANDLE ON TRUCK AS HE FELL - HYPEREXTENDED HIS SHOULDER.)

ALL EMPLOYEE WERE CHECKED AND TWO WERE DIAGNOSED WITH LATENT TB. IT IS NON-COMMUNICABLE & BOTH EMPLOYEE WILL BEGIN ANITBIOTIC TREATMENT. WHEN FINISHED BOTH SHOULD FACE ONLY AN EXTREMELY LOW CHANCE OF INFECTION RETURNING.

EE WAS CLEANING SPILLAGE FROM A WALKWAY ALONG THE CRUSHER. SOME OF THE MATERIAL WAS FROZEN, SO A SPUD BAR WAS BEING USED TO LOOSEN IT. ONE EE WAS USING SPUD BAR & 2ND EE WAS SHOVELING IT OFF T HE WALKWAY. THE SPUD BAR BOUNCED OFF A STONE AND THE OTHER END OF THE BAR STRUCK THE EE IN THE FOREHEAD CAUSING LACERATION TO HIS FOREHEAD ABOVE THE RIGHT EYE. HE RECEIVED 3 SUTURES TO CLOSE

EMPLOYEE WAS HAULING PIT ROCK UP TO CRUSHER, HE WAS LEAVING PIT WITH A LOADED TRUCK AND DID NOT FOLLOW THE TRAVEL ROUTE WE ESTABLISHED. HE RAN THE TRUCK THROUGH A LOW SPOT IN FLOOR OF PIT. THE FRONT SHOCK ON TRUCK CAME APART. EE CLAIMS HE HIT HIS HEAD ON ROOF OF CAB.

EE WAS ATTEMPTING TO OPEN THE HOOD OF THE VOLVO HAUL TRUCK. HE SPRAINED HIS LEFT SHOULDER WHILE TRYING TO RELEASE THE HOOD.

AT 1030 A BEARING LOCKED UP ON USI SHUTTLE CONVEYOR. TO REMOVE BEARING NEEDED TO GET STONE OFF BELT, STARTED TO HOSE THE BELT HE WAS STANDING ON THE TAIL PULLEY GUARD. WHEN HE TURNED THE ON TH E HOSE IT KNOCKED HIM OFF BALANCE AND HE STEPPED OFF THE GUARD BACKWARDS AND TWISTED HIS LEFT KNEE.

WAS GRINDING ON A PIECE OF METAL THAT HAD BEEN CUT WITH A TORCH AND A PIECE OF SLAG OR GRINDINGS FLEW UP AND HIT HIS CHEEK AND WENT UNDER GLASSES AND INTO HIS RIGHT EYE.

ACCIDENT HAPPEN ON 8-2-01 TIME UNKNOWN. EE CAME TO ME ON 8-3-01 AT 6:00 P.M. STATED HE NEED TO GO AND GET CHECK OUT CAUSE HIS BACK WAS HURTING. I ASKED WHAT HAPPEN. HE STATED HE WAS DOWN INT T HE VSI GETTING A BLOCK OUT OF THE CRUSHER AND HIS BACK STARTED TO HURT. HE FIGURE IT WOULD BE BETTER THE NEXT DAY.

EE WAS FUSING PLASTIC POLY-PIP TOGETHER AND WAS TRYING TO ALIGN THE 8"" PIPE WITH THE FUSING MACHINE. HE BENT OVER AND TRIED LIFTING THE FUSING MACHINE AND PULLED HIS BACK.

EMPLOYEE WAS CLIMBING UP THE LOADER AND SLEPT DUE TO FROST BUILD UP ON THE LADDER HANDRAIL. HE WAS REACHING FOR THE DOOR LATCH WHEN HIS OTHER HAND SLIPPED OFF THE HANDRAIL.
