Injured employee placed a piece of large angle iron 2""x2""x10' in chop saw to make cut for repair job. EE removed gloves to use soap stone for layout. As EE turned around to measure cut, EE struck the edge of the angle iron with the top of left hand, resulting in laceration, which required stitches to close.
Injured employee 1479199 was hooking up portable air compressor, when he slipped on the stones hitting his left elbow on the pick-up hitch causing laceration to elbow.
Employee was helping the men change and remove screens for the Telesmith S-2 Screen, when he pulled something in his lower abdomen.
EE assisting foreman with changing screen cloth on Wash Screen S-2. While installing middle deck screen, the employee bent down through the first deck with arms to bolt down center rail hitting back of left arm on worn angle iron, causing cut to back of arm. Resulting in three stitches.
Fire discovered and fought until3:40 P.M. exited building contacting Seneca County Sheriff dept. Fire Dept arrived at 4:07 P.M. fire extinguished at 4:45 p.m.
EE WAS LOOSENING A NUT UP ON THE CRUSHER WITH A BREAKER BAR. THE BREAKER BAR SLIPPED MAKING HIM HIT HIS RIGHT ELBOW ON A PIECE OF STEEL PLATE ON THE CRUSHER LID. X-RAY REVIELED A BONE FRACTURE.
WHILE WORKING ON STONE BOX ON TOP OF S-3 SECO 8 X 20 DECK SCREEN, EE WAS ATTEMPTING TO PLACE A 18"" X 40"" X 1/2"" PLATE UNDER STONE BOX THAT WOULD CONTROL FLOW OF STONE TO SECO SCREEN. WHILE SLI DING THE PLATE OFF THE MANLIFT ONTO SCREEN, PLATE DROPPED APP 6"" CAUSING HIS FINGER TO BE CAUGHT BETWEEN PLATE AND 2"" SPRAY BAR. THERE WAS ONLY 2' CLEARANCE BETWEEN STONE BOX & SCREEN FRAME,
EMPLOYEE WAS RUNNING FROM THE OFFICE TO THE LOADER; HE WAS CLIMBING UP THE SIDE LADDER AND APPEARED TO FALL STRAIGHT BACKWARDS, LANDING ON THE SIDE OF HIS FACE AND BODY.
CLIMBING DOWN OFF OF MAST ONTO CAB AND CAUGHT LEFT FOREARM ON PIECE OF SHIM STOCK FROM MAST CUTTING ARM. 7 STITCHES TO CLOSE WOUND.
