Employee lacerated forearm just below the elbow while running the bagging machine in the Fine Grind Plant. The wound required sutures to close.

Absorption of radiations, caustics, toxic and noxious substances

Employee was entering the driver's side door of the plant maintenance truck. After sitting in down in the seat, EE was stung in the lower back by a yellow jacket. An allergic reaction ensued, and the employee was transported to the local hospital.

Struck by falling object

While cleaning the tube over Portsmouth Rd. the tube collapsed and fell to the road. One the employees standing on the catwalk received a laceration to their left shin that required 3 sutures.

Maintenance employees were loading a feeder onto a flatbed trailer. The flatbed trailer had raised wheel wells. As the feeder was being lowered, a Maintenance Helper used his right hand in an attempt to keep the equipment from swinging. As the equipment lowered, the Maintenance Helper's right fifth finger was pinched between the feeder leg and the trailer fender.

Assisting mine employees to remove a bushing from 4 1/2 Simons cone crusher. Injured was holding a steel bar for another miner who was striking it with a sledge hammer. The bar slipped off and his right thumb was pinched against the bushing. Received 6 stitches and then returned to work.

While using a utility knife to trim rubber from a conveyor belt, knife slipped and struck employee on left forearm. Treatment required 4 stitches to forearm.

Fall from machine

Employee removed lid from top of anfo truck and lowered hand rail on top of truck while on top. He then stepped bacwards and caught his foot in hand-rail. apparatus causing him to fall on belly auger of truck and truck rear bumper. approx. 8'

While setting an oxygen cylinder onto the back of a service truck, the employee pinched his middle finger between the cylinder and a spud bar which was laying on the floor of the truck, resulting in a fracture to the tip of the middle finger of his left hand.

OVERHEAD CAGE PINCHED MY LEFT RING & MIDDLE FINGER IN THE HINGE WHEN I WAS PULLING IT DOWN. THIS CAUSED A LACERATION TO MY MIDDLE FINGER AND PARTIAL AMPUTATION OF MY RING FINGER (HALF-WAY DOWN TO THE FIRST JOINT). THIS LEAD TO NO USE OF MY LEFT HAND & SEVERE PAIN. GLOVES WERE BEING WORN AT THE TIME OF THE ACCIDENT.

EE STARTED TO FELL PAIN IN HIS LOWER BACK WHILE STACKING BAGS OF LIME. THE EE WAS TWISTING HIS BACK WHEN MOVING THE BAGS.

EE WAS PREPARING FOR LUNCH AFTER WORKING ON A CRUSHER. WHILE WRAPING UP SOME CABLE A BLOWBAR/HAMMER LYING ON THE GROUND FELL OVER ONTO HIS FOOT. HE WAS WEARING STEEL TOE BOOTS BUT RECEIVED A F RACTURE TO ONE OF THE METATORSAL BONES IN HIS LEFT FOOT.

EMPLOYEE ORIGINALLY FELL ON ICE WALKING ACROSS A ROADWAY. HE WAS FIRST DIAGNOSED WITH A CONTUSION WITH A NEGATIVE X RAY. THE EMPLOYEE COMPLAINEDTHAT HIS SHOULDER WAS IN PAIN. AN MRI ON 3-13-03 REVEALED A MAJOR TEAR IN HIS ROTATOR CUFF.

EE WAS REMOVING A DRILL BIT ON #75 DRILL UNIT WHEN THE CHAIN WRENCH HE WAS USING BROKE. THE CHAIN WRENCH BROKE SWINGING OUT AND STRIKING THE EMPLOYEE'S LEFT KNEE AND LEFT HAND. EE RECEIVED LAC ERATION TO THE LEFT KNEE AND CONTUSION TO THE LEFT HAND.

THIS INJURY HAS BEEN IN DISPUTE AND WAS ALLOWED BY THE OHIO BWC ON 8/27/01. EMPLOYEE COMPLAINED OF PAIN IN HIS WRIST WHEN OPERATING A PIT LOADERHE WAS LATER DIAGNOSED WITH BILATERAL CARPAL TUN NEL. HE IS RECEIVING PHYSICAL THERAPY FOR HIS CONDITION.

EMPLOYEE WAS CUTTING A WELD WITH A GRINDER WHEN HE FELT SOMETHING ENTER HIS LEFT EYE. HE WAS WEARING EYE PROTECTION AT THE TIME BUT IT WAS THE WRONG TYPE FOR THE JOB.

EMPLOYEE WAS LOWERING A TRAILER INTO IT'S NORMALPOSITION AFTER BOBCAT WAS UNLOADED WHEN A BEE STUNG HIM ON HIS NECK. HE WAS TAKEN TO THE HOSPITAL FOR TREATMENT AND RELEASED ON THE SAME DAY.

EMPLOYEE WAS WEIGHING TRUCKS ON THE SCALE HOUSE WHEN A BEE SOMEHOW STUNG HER THROUGH HER SOCK ON HER LEFT FOOT (ANKLE) AREA. SHE DID NOT GO TO DOCTOR UNTIL 8/24/00 WHEN HER FOOT WAS STILL RED . DOCTOR GAVE PRESCRIPTION FOR HER TO TAKE.

EMPLOYEE WAS REMOVING A LARGE BEARING FROM ITS HOUSING TO REPLACE THE BEARING. HE WAS DRIVING A BEARING OUT OF A HOUSING, WHEN IT SLIPPED OFF AND SMASHED HIS LITTLE FINGER ON HIS RIGHT HAND.

EMPLOYEE WAS PICKING UP A BLOCK OF WOOD OUTSIDE THE SHOP AREA WHEN A GUST OF WIND BLEW A PIECE OF FOREIGN MATERIAL INTO HIS LEFT EYE. HE DID NOT GO TO THE EYE DOCTOR UNTIL MAY 22. THE DOC- TOR REMOVED THE FOREIGN OBJECT AND PRESCRIBED ANANTIBIOTIC.
