Caught in, under or between a moving and a stationary object

Improper use of hand tool. While unplugging a DTH, EE used a 48"" pipe wrench as a hammer. While striking the DTH, the crook EE's right hand was pinched between the hook and handle causing a puncture injury requiring 3 stitches.

EMPLOYEE WAS ATTEMPTING TO DRIVE VEHICLE AWAY FROM EMPLOYEE ROOM & ANOTHER VEHICLE WAS PULLING AWAY AT THE SAME TIME. 2ND VEHILCE SPUN WHEELS ON ICE AND DIRT FLEW IN THRU EE'S OPEN WINDOW & STRUCK HIM IN THE RT. EYE, CAUSING AN ABRASION.
