Employee alleges that the brakes locked up in the Komatsu WA 500 loader he was operating injuring his lower back. He was seen at a clinic on 5/8/2015 and released to duty. Employee went to a specialist on 5/19/2015 and was taken out of work.

EE was removing a 3.5"" piece of shaft used as a safety block in the crusher when the shaft slipped from the far side of the crusher pinching the EE's right thumb against the frame of the crusher resulted in lacerations and a chipped bone. Medical only no lost time.

SLIP AND FELL WHILE MOUNTING LOADER. EE WAS NEAR THE TOP OF THE LADDER WHEN HIS LEFT HAND SLIPPED CAUSING HIM TO ROTATE TO THE REAR OF THE GRADER. HE FELL OFF THE LADDER AND HIT THE LEFT REAR TIRE ON HIS WAY DOWN RESULTING IN BRUISES AND A MILD FRACTURE TO THE COCCYX FROM THE LOWER SPINE.
