An employee was removing wear plates from a loader bucket. The employee was using an impact wrench to loosen the bolts. EE put EE's hand on the head of the bolt to push it back up, when the head cut through EE's gloves as it was being turned by the wrench.

Employee alleges he was rolling a piece of screen cloth out on a storage rack and felt a pull in his back. He was diagnosed with a muscle strain, but after no improvement was sent for an MRI which revealed a herniation of a disc.

Employee performing maintenance on a 980C front-end loader. While removing the loader teeth pins he struck one of the teeth with a hammer, a piece of metal sheared off into the right side of his stomach. the clinic removed the metal, 8 stitches were required.
Employee was working on the 980C loader. While reaching for the open end of the wrench, the wrench stabbed his left hand between the thumb and index finger resulting in a deep cut. Three stitches were required.
Employee was working on the Kaw Loader - tapping ujoint into place when a piece of metal broke off the hammer and went into his pointer finger on the left hand. Metal was removed at the clinic and one stitch was required.
EE was working on the wash tower, holding a screen. While he was holding the screen cloth, it slipped off I beam and fell about a foot and 1/2. When the screen slipped, it struck the EE's right hand resulting in a cut to his ring finger on his right hand. This injury required 6 stitches.

EE STATES HE WAS UNLOADING TRUCK AT THE QUARRY. THE LOAD SHIFTED AND THE BRACING GAVE WAY, HITTING HIM IN THE FACE, SIDE AND ARM. SPRAINED WRIST, CUT TO THE FACE.
