While unlocking a camlock fitting on a pit pump the employee was pulling on the 4 inch female fitting trying to get it lose. When the fitting came loose it struck the employee hitting between the nose and upper lip. *** Resulting in four sutures to his upper lip - justifying a reportable medical injury with no restrictive duty or lost days***

WHILE DOING ROUTINE PLANT MAINTENANCE HE BECAME DIZZY, VERY HOT, AND SWEATY. DID NOT FEEL WELL.

EMPLOYEE WAS EXITING SKIDSTEER LOADER. SHE STEPPED DOWN THE WRONG WAY AND TWISTED HER LEFT ANKLE CAUSING INSTANT SWELLING/BRUISING.

EMPLOYEE STATES WHILE REMOVING 4 BOLTS THAT HOLD THE CABLE TENSIONER UNIT IN PLACE ON A LINK BELT CRANE THE UNIT SUDDENLY MOVED STRIKING EMPLOYEE ON HIS LEFT HAND.
