Employee was using a hydraulic tool when it slipped and struck employee in the mouth breaking one tooth.
Employee was attempting to free a rock lodged between a belt conveyor and a tail pulley. Using a chain come-a-long to move the tail pulley, the hook slipped off the wing of the tail pulley and struck the employee in the chin. This resulted in 10 stitches.
EMPLOYEE WAS USING A SIDE GRINDER TO REMOVE OLD LAGGING FROM A HEAD PULLEY WHEN THE GRINDER CAUGHT ON A WELD & KICKED BACK STRIKING THE LEFT KNEE RESULTING IN A CUT WHICH REQUIRED 5 STITCHES.
