A/C vent cover fell off of the 988F wheel loader. Employee was attempting to put the A/C vent cover on. They stuck their fingers into the vent hole, the fan was still running and a finger came into contact with the steel fan blades which cut the fingers.

EE was working in secondary plant inspecting & greasing when he began to feel ill. He went to employee break room to sit for a few minutes but felt no better so he went to office. Plant manager took him to nearby med. clinic. The doctor suspected a heart problem & had him transported to Wake Med. hospital. He was kept, tested & released on 06/09 with no diagnoses & no restrictions.

EMPLOYEE WENT OVER A ROCK ON THE 988F PIT LOADER. WHEN HE CAME OFF THE ROCK IT CAUSED THE EMPLOYEE'S HEAD TO HIT THE WINDOW CUTTING HIS EAR REQUIRING 11 STITCHES TO CLOSE THE WOUND.

AN OUTSIDE CARRIER BROUGHT IN A STEEL STRUCTURE FOR THE NEW PLANT EXPANSION. IN AN ATTEMPT TO HELP THE DRIVER, EMPLOYEE RELEASED THE CHAIN BINDERS OFF OF THE STRUCTURE. WHILE RELEASING ONE OF THE CHAIN BINDERS WITH A 2-3' PIPE, THE CHAIN BINDER BROKE AND STRUCK HIM BETWEEN THE EYES.
A MECHANIC WAS WORKING ON A HAUL TRUCK WHEN A PRESSURIZED HYDRAULIC LINE CAME LOOSE,HITTING THE MECHANIC IN THE HAND, INJECTING HYDRAULIC FLUID INTO THE LEFT INDEX FINGER.
