While performing crusher maintenance, employee stood on mid-rail of handrail and fell to lower level.

Fall from machine

Employee was exiting the rock breaker when employee missed a step and fell, ultimately breaking two bones in employee's wrist.

Over-exertion (Not Elsewhere Classified)

The employee was using a wrench to loosen wedge bolts that hold the bowl liner in the crusher. The employee leveraged the wrench to release the bolt. Immediately, EE felt a sharp pain in EE's left foot as EE's weight transitioned. An x-ray was performed and revealed a fracture of the fifth metatarsal bone in EE's left foot.

Over-exertion in pulling or pushing objects

After speaking with a customer in the yard, the injured employee climbed back into the loader's operator chair to continue working. From the chair (in a sitting position) the employee reached for the door to pull it shut with right arm. The door was not locked or latched. While pulling the door shut, EE felt a sharp pain in lower back.

INJURED & 2 CO-WORKERS WERE PUTTING PIECES OF TIN ON THE ROOF OF THE BUILDING. ONE WAS HANDING THE TIN TO OTHER WHO WAS IN THE BED OF THE TRUCK & HE WAS HANDING THEM TO INJURED (WHO WAS ON TOP OF THE CAB OF THE TEREX TRUCK). MOST OF THE PIECES WERE 8' IN LENGTH. INJURED HAD TURNED TO PUT ONE PIECE OF THE ROOF. HE TURNED TO GRAB THE NEXT PIECE THAT WAS 12'LONG & RAN INTO IT CUT NOSE

EMPLOYEE HAD GOTTEN OFF DOZER TO DO DAILY MAINTENANCE, WHEN HE TURNED TO WALK AWAY, HIS RIGHT FOOT WAS ON THE EDGE OF A SMALL RUT WHICH WAS SLIGHTLY DOWNHILL. HE LOST HIS BALANCE, TWISTING HI S RIGHT ANKLE, RESULTING IN A FRACTURE OF THE RIGHT ANKLE.

Fall to the walkway or working surface

EMPLOYEE WAS DOING PRE-SHIFT INSPECTIION OF LOADER. WHEN HE WALKED BEHIND THE LOADER, HE SLIPPED IN A WET AREA AND FELL. EMPLOYEE TRIED TO CATCH HIMSELF WHILE FALLING, INJURING HIS LEFT SHO ULDER.
