Removing plow bolts form a loader bucket cutting edge for replacement. Other ee operated the impact wrench to remove nut and the bolt backed out and started to spin so victim place his hand on the bolt to hold it in place. Bolt spun and sharp edge of bolt cut through the glove and into left index finger causing laceration requiring 6 stitches to close. Medical only

Laceration to left cheek area when air impact socket struck employee working on dragline. Employees were working under the Lima dragline removing nut from the vertical travel shaft pinion. When the air impact wrench was removed from the nut, the trigger was accidentally hit. The socket spun off and struck employee in the left cheek.

Struck against stationary object

Employee was getting into truck T-648 and did not bend over far enough (was wearing hard hat) causing him to hit top of hard hat on the inside of door opening. This caused his head to bend backwards very hard hurting his neck. His fingers, hands, and feet are numb and tingling. Also, his arms and legs feel weak.

Employee was cleaning up in the surge tunnel when he heard a spring on the feeder rattling. He placed his hand on the springs to determine which one was loose and his finger slipped between the broken spring and the bracket where it was pinched through his glove. He sustained a laceration to his index finger which required 5 stitches.

EE was working in the engine room of the lima 2400 dragline when he slipped on the slick floor plates due to mud and fell striking his head ont he engine. He suffered a laceration to his forehead above the left eye which required 9 stitches. Medical only.

EE CUT HIS FINGERS ON THE AIR CONDITIONER FAN. WHILE TRAMMING THE CAT 988F LOADER, ONE OF THE AIR COND VENTES FELL OUT OF THE OVERHEAD UNIT. HE WAS ATTEMPTING TO REINSTALL THE VENT WHEN THE MACHINE HIT A BUMP CAUSING HIS HAND TO GO INTO THE OPEN VENT HOLE ANC CONTACT THE BLOWER FAN LACERATING TWO FINGERS ON THE RIGHT HAND AND CAUSING A HAIRLINE FRACTURE OF THE MIDDLE FINGER

STRUCK ON LEG BY A CHAIN HOIST. EE WAS PULLING A 5 TON COME-A-LONG FROM INSIDE OF ROCK BOX THATSETS OVER THE JAWCRUSHER. THE COME0-A-LONG WAS BEING USED TO LIFT CHECK PLATES OUT FOR EXCHANGE. WHEN LIFTING THE RACHET PART OF THE COME-A-LONG OUT & DRAGGING THE LIFT HOOK BLOCK UP WITH THE COMEALONG, CHAINS FOR REMOVAL FROM THE BOX, THE HOOK BLOCK REACHED THE TOP EDGE OF THE BOX & SWUN

WHILE CLIMBING PLANT STRUCTURE TO ACCESS CONVEYOR, SLIPPED AND FELL AND CUT LEG ON BOLT ON CONVEYOR. LACERATION REQUIRED STITCHES TO CALF OF LEFT LEG.
