Struck by falling object

EE was greasing the take up on the number 3 conveyor on the pit side. EE had finished greasing the outboard bearing and as EE moved to grease the inboard bearing a piece of accumulated frozen material, approx. 12""x12""x6"" and weighing approx. 35 pounds, fell from a height of 17 feet and struck EE directly on the hardhat.

Struck by falling object

On 8/18/22 The employee was collecting the shank and tooth (approx. 60 lbs.) from the loader that had been collected at the belt magnet. As they pulled the shank off the belt, which was shoulder height, their fingers slipped out of the tooth and the shank fell causing the employee to ""catch"" the weight with their left arm. This caused muscle strain in their left shoulder.

Employee was troubleshooting a Furuwaka DCR 20 Drill and finger was caught between moving parts. EE received a finger laceration requiring sutures. EE was wearing cut protective gloves but the impact cut through the gloves.

Over-exertion in wielding or throwing objects

Employee was attempting to dislodge a small rock wedged between a chute wall and the head pulley of a belt conveyor. While swinging a 4 lb hammer employee felt their shoulder tighten up. Employee was diagnosed with a torn tendon in the shoulder requiring arthroscopic surgical repair on May 30, 2018.

Caught in, under or between a moving and a stationary object

A spacer plate that went behind a jaw die fell off the wall of the jaw, landed on the skirts under the jaw and fell against employee, squeezing him between the wall and plate. Employee suffered soreness and stiffness.

Employee said step broke on equipment when mounting machine. Employee complained of pain in the left ankle and was taken to local walk-in clinic. X-ray showed swelling but no broken bones or fractures. Doctor released patient to return to work and perform only sit down work. Employee iced ankle that day, and returned to site next day.

Struck against a moving object

EE was operating a John Deere 12M Grader, hit a bump, bounced up and came down on his scrotum.

Employee was carrying equipment to load onto back of pick-up truck. Employee lost balance and fell on left hip. Initial diagnosis that day was bruised hip / return to duty. On follow up doctor visit (10/29/2010) employee stated he still had discomfort in hip. Physical therapy was prescribed.

Employee was using shovel to take material sample and when he turned to empty the shovel's load into the bucket, he strained his back

Person was using a water hose to wash off an area in which he was getting ready to work. Person lost his footing and fell onto left side hurting his hip and shoulder.

Inhalation of radiations, caustics, toxic and noxious substances

EMPLOYEE CUT A POLYDEX SCREEN PANEL WITH A CHOPSAW. HE BECAME ILL LATER THAT EVENING AND DEVELOPED A MILD COUGH. 2 DAYS LATER HE WENT TO THE EMERGENCY ROOM BECAUSE OF SHORTNESS OF BREATH. HE WAS DIAGNOSED WITH CHEMICAL PNEUMONIA, CAUSED EVIDENTLY DUE TO FUMES FROM THE SAWING OPERATION.

WHILE REPAIRING A WATER LINE FOR THE 52FC CRUSHER THE WIND BLEW A PARTICLE OF GRIT INTO EE'S LEFT EYE. WASH EYE OUT WITH WATER,EXPERIENCED IRRITATION. EXAMINED BY DR & PRESCRIBED ANTIBIOTIC EYE OINTMENT. WAS WEARING SAFETY GLASSES AT THE TIME.

WHILE PICKING UP A WATER HOSE EE SLIPPED AND FELL. HE STATED HE PULLED A MUSCLE IN HIS LOWER BACK.

WHILE POSITIONING STEEL PLATE INSIDE BIN WITH A CRANE, EE INSIDE OF BIN WAS STRUCK BY PLATE WHEN CRANE MOVED THE PLATE. EE STRUCK SHOULDER ON EDGE OF BIN, CAUSING FRACTURE TO HIS RT. SHOULDER.

EE WAS OPERATING THE PIT LOADER IN THE QUARRY PIT WHEN HE COMPLAINED OF CHEST PAIN, SHORTNESS OF BREATH, ARM HURTING ETC. PIT FOREMEN RETRIEVED EE FROM PIT VIA COMPANY TRUCK CALLED AMBULANCE THEN EMS PERSONNEL RESPONDED AND TRANSPORTED THEEE TO THE HOSPITAL.

Fall from machine

EMPLOYEE WAS REMOVING FEEDER SIDE OF 52FC CRUSHER WHEN THE CHAIN SLIPPED AND THE FEEDER FELL. EMPLOYEE JUMPED BACK OUT OF THE WAY AND FELL, SCRAPING HIS RIGHT LOWER LEG AND HIP.
