Struck by falling object

Metal plate fell onto employees foot. Incident did not become reportable until 2/16/2026 when employee sought metal attention and was diagnosed with fractures.

Struck by flying object

While removing an expanded metal guard bent around a motor shaft, the guard flexed back to a straight position after the bolt was removed striking and cutting the employee's nose. employee received 5 stitches.

Accident type, without injuries

Customer truck driver entered quarry entrance with bed raised striking overhead communication lines. This resulted in three poles breaking and the overhead power lines falling to the ground behind the truck. Driver was required to remain in truck until power company deenergized lines

Caught in, under or between a moving and a stationary object

EE was pulling the PC-100 air compressor about 2.5' by the tongue so that the fuel hose from the lube truck would reach the fuel tank on the compressor. The wheel rolled over a small hump which caused the compressor to roll forward quickly and pinched the EE's finger between the tongue and the door of the truck. Resulted in stitches to his right index finger.

EE WAS LOWERING A PLATE INTO A CHUTE USING A CHAIN HOIST. WHEN EE UNDER SLIPPED, THE HOIST MOVED CAUSING CHAIN TO PINCH EE'S HAND.

Fall onto or against objects

EE WAS PREPARING TOINSTALL A 36"" X 47"" PLATE OVER A CHUTE, HE WAS STANDING ON A BOARD. AS COWORKERS WERE LOWERING THE PLATE INTO PLACE WITH A CHAIN HOIST, EE SLIPPED AND FELL DOWN INTO THE CHU TE STRIKING HEAD ON METAL IN CHUTE.
