Rubbed or abraded

While changing cutting edges on a 992 loader bucket, employee reached under to check if bolt was spinning or not. While grabbing the bolt the employee accidentally pulled the trigger on the impact gun and turned the bolt causing it to slice their finger.

While removing a guard on a motor to measure a sheave, employee smashed left finger.

Struck by falling object

Supervisor was spraying out the throughs bin underneath the screen on top of the tower. Employee walked down to the ground level to see if material was flowing out of the chute. As EE was walking past the torch box, all the material sloughed out and knocked the torch box over and onto the employee.

While walking the area to inspect the screen a mechanical failure caused a guard to contact the employee after the flywheel contacted it knocking the employee backward into the adjacent screen.

Employee washed out bed of articulating truck in plant and did not lower bed as EE drove off the raised bed contacted the conveyor protection bridge causing the cab to lay over on its side.

While ascending the ladder on the motor grader the employees right foot slipped causing him to lose his balance and slip back to the ground resulting on a groin area strain

EE received seven staples at the top of his head after an air wrench from above struck him while standing underneath the grizzly bar at the primary feeder. EE was not wearing his hard hat.

EE possibly strained stomach after trying to sit up from a flat position on his back while in the pit underneath a PC 400 Rockbreaker to repair a loose wire. EE did not go to the doctor until November 21, 2007.

Employee was walking on top of rebar mat when his foot slipped into a hole in the rebar mat, causing pain to his foot & back. Did not seek medical attention until 5-1-07. Called him 5-9-07. His care is now beyond first aid.

Struck against stationary object

Employee's left middle finger was contused while pushing forward to break mounting bolts loose on the turbo base. The bolt broke loose jamming his finger into the turbo.

Customer truck driver pulled up to automated ticket chute and reached into box to get ticket. The truck rolled back catching his finger between the chute and the truck mirror causing a laceration to his finger, requiring 5 stitches.

Over-exertion in lifting objects

EE strained his lower back after he and a co-worker lifted a freon recovery unit onto a truck. This task should have been completed with a forklift. Employee initially went to the doctor on 11/14/06; however, restricted duty was prescribed on 12/15/06.

EE received burns to his upper extremities due to mechanical failure that occurred on processing equipment.

We think the EE was cutting sleeves off coveralls when he received a self-inflicted wound to his left chest; subsquently a knick to his heart.

Contact with heat

EE was placing concrete around new office. He was showing the warning signs of heat stroke. EMS was called to give care in case EE's condition got worse. The EMS carried him to a local hospital. The hospital administered fluids and kept him overnight for observation. He was released the next day to go back to work.

Employee was visually inspecting pugmill as part of daily routine and reached to lift a piece of wood with one hand and lift an inspection plate up with the other when he felt a sensation in his back between his shoulder blades.

EMPLOYEE WAS SHOVELING OFF THE ROCK THAT WAS ON THE CONVEYOR WHEN IT STALLED OUT. METAL SHIRT BOARDS ARE ON BOTH SIDES OF THE BELT. AS HE PUSHED THE SHOVEL DOWN, THROUGH THE ROCK, INSIDE THE S KIRT BOARDS, HIS LEFT INDEX FINGER WAS PINCHED BETWEEN THE SHOVEL HANDLE AND THE METAL SHIRT BOARD. CAUSED LACERATION ON LEFT INDEX FINGER REQUIRING 10 STITCHES.

EE WAS REPLACING THE TRACK ROLLERS ON THE D8 DOZER. WHILE HE WAS REMOVING THE TRACK ROLLER BOLTS HE STARTING TO FEEL A TIGHTENING IN HIS BACK. THE PAIN WORSENED OVER THE WEEKEND SO ON MONDAY, HE WENT TO THE DOCTOR TO BE CHECKED OUT.

EE WAS MOVING A PIECE OF SCRAP METAL BY HANDS TO GET TO THE BOOM TRUCK THAT WAS NEEDED IN THE PIT. WHILE MOVING THE METAL, IT CUT HIS THIRD FINGER ON HIS LEFT HAND CAUSING HIM TO GO TO THE HOS PITAL FOR 2 STITCHES.

INJURED AND ANOTHER EMPLOYEE WAS PICKING UP HOSES FROM SAME PILE. OTHER EE RAISED UP WITH A STEEL TUBE, STRIKING INJURED IN THE MOUTH. HIS FRONT TWO TEETH WERE DAMAGED. ONE TOOTH WAS BROKE, OT HER TOOTH WAS CHIPPED.

EMPLOYEE WAS CLEARING ROCK BY HAND THAT HAD BRIDGED UP IN THE HP700 CRUSHER, WHEN HE REMOVED A PIECE OF ROCK IT CAUSED ANOTHER PIECE TO FALL FROM ABOVE, WHICH LACERATED HIS RIGHT INDEX FINGER.

EMPLOYEE WENT TO TUNNEL TO REMOVE ROCKS THAT WERE HUNG IN THE CHUTE BELOW THE FEEDER. AS HE WAS REMOVING THE ROCKS, ONE ROCK FELL OUT OF THEFEEDER MASHING HIS RIGHT INDEX FINGER AGAINST AN OTHER ROCK.

EMPLOYEE WAS REMOVING A KEYWAY FROM A PULLEY SHAFT USING A PUNCH AND HAMMER WHEN THE PUNCH FRACTURED PERJECTING A PIECE OF METAL IN HIS LEFT FOREARM.
