Struck by falling object

TWO EMPLOYEES WERE IN THE PROCESS OF ASSEMBLING THE FRAMEWORK OF A SCREEN TOWER. WHEN ONE SIDE FRAME WAS SET WITH A 15 TON BRIDGE CRANE IT WAS DETERMINED THAT SOME ALIGNMENT WAS NEEDED BEFORE THE SIDE FRAME COULD BE BOLTED INTO PLACE. HE ATTEMPTED TO RECONNECT THE SPREADER CHAINS TO ENBLE THE SIDE FRAME TO BE LIFTED. WHILE RAISING CRANE IT FELL ON THE INJURED IN THE BACK.

Employee was finishing concrete for a flag pole. The employee was removing excess concrete from the trowels, as he slid the trowels together he received a small cut on the knuckle of his hand. The cut required three stitches.

Employee was operating an over-the-road truck on a public road, when he became nauseated due to a migraine headache. The employee turned the truck around and returned to the shop. We are submitting the 7000-1 at the request of the Field Office Supervisor. We respectfully protest the submission of the 7000-1, as we believe it is not a reportable event.

The injury occurred while an employee was using a hand held grinder, the grinder kicked back striking the employee in the thigh, which resulted in a cut that required medical attention.

EE was performing his regular job on 6/01/11 when a piece of slag burned him, at the end of June ee was still experiencing pain and reported to the office that he needed to seek medical treatment. On June 27, employee was written out of work for medical treatment due to his diabetes not allowing the injury to heal making this a lost time incident

Two employees were working on unloading a 4.25-crusher head. Injury was assisting the forklift driver to get the forks lined up. The fork was hooked on a beam where the head was resting, He pulled on the beam and his left thumb was caught between the head and the beam. Resulting in 2 stitches.

EE was removing cylinders from crate and cut right hand on strapping material.

A machinist with 28 years exp was trying to remove a lifting strap from a 42x65 gyratory counter weight by prying it up with a pry bar. In the course of feeding the strap trough, the pry bar slipped & the 600# weight of the counter weight came down on the steel loop end of the lifting strap & pinched his left index finger cracking the bone requiring 6 stitches.

While sliding a heavy steel member on table, EE pulled something in lower back. Wrote it on 1st aid board & con't to work. On the 27th & 28th, he tweaked it again & told supervisor. Dr. told him light duty, which makes this a lost time because of shop policy. He is supposed to get an MRI.

Fall to the walkway or working surface

Truck driver was releasing mechanism with hammer. He missed, throwing him off balance. He put hand out to catch himself causing most of his weight to be put on that wrist. This resulted in a sprain of his wrist. Upon x-rays it was determined that no bones were broken. A follow-up MRI has determined that wrist did receive fracture making this a reportable injury with no lost work.

While climbing down from a lathe set-up process, an intermediate step was not used which caused an oversized step or fall that resulted in an abrupt and additional force to employee's knee causing hyperextension injury.

EMPLOYEE WAS WORKING OUTSIDE ON UNEVEN GROUND. HE TWISTED HIS RIGHT ANKLE. HE REPORTED THIS TO HIS SUPERVISOR BUT DIDN'T NEED MEDICAL ATTENTION. HE WENT TO THE DOCTOR ONE MONTH LATER AND FOUND THAT HIS RIGHT FOOT WAS BROKEN. THIS BECAME A LOST TIME MEDICAL ON 6/10/05.

Fall from scaffolds, walkways, platforms

THE EMPLOYEE WAS STEPPING OFF OF A WORK PLATFORM & MISSED THE STEP. HE STARTED TO FALL & REACHED OUT TO CATCH HIMSELF WITH HIS RT. HAND. HE BROKE HIS RT. WRIST WHICH HAD PREVIOUSLY BEEN BROKEN IN A WORK RELATED ACCIDENT & WAS STILL IN A CAST.

EE WAS TURNING A NUT ON A ROD. THE WELD BROKE, CAUSING THE ROD TO FLY UP AND HIT EE'S RIGHT WRIST. THIS RESULTED IN A BREAK TO HIS WRIST. MEDICAL ONLY, NO LOST TIME.

PER SHOP MANAGER - AFTER HEATING THE COUNTERWEIGHT/GEAR TO RELIEVE THE PRESS FIT OF THE ASSEMBLY, TWO MECHANICS WERE HITTING THE COUNTERWEIGHT/GEAR WITH SLEDGE HAMMERS IN AN ATTEMPT TO REMOVE THE COUNTERWEIGHT/GEAR FROM THE ECCENTRIC BARREL. EE WAS ON UPPER DECK OF HIS FLATBED TRAILER ATTEMPTING TO STRAP DOWN HIS LOAD. HE LOST HIS BALANCE & FELL TO THE GROUND ON THE LEFT SIDE OF HIS FACE &ELBOWS. (BONES IN LEFT & RIGHT ELBOWS SHAT TERED. 2 FRONT TEETH KNOCKED OUT SURGERY ON ELBOWS IS SCHEDULED FOR 10/18/00
