Employee was pulling wire through conduit and injured right shoulder and bicep. The injury on May 6, and employee reported it but did not get it checked until May 15, and they sent them for Xrays and they had surgery on July 3, 2024.
Loading the presplit and accord, cut the accord to tie, hand slipped, cut left shin.
Employee became dizzy on the job, transported to the hospital.
Employee had just cleaned out from under tailgate of stockpile truck. Employee started to step up on a concrete platform and placed his right hand on tailgate gate latch. It closed pinching right hand.
Employee was cleaning up the tunnel area when a rock fell off the belt. He was using a garden hoe and the rock hit the hoe handle which struck him under the lip/mouth.
Employee was trying to give a report to the operator of a D8 dozer when he was struck by the dozer on the left lower side of the blade. Fracture to both legs.
EE was greasing bobcat bucket pins. He released the couplings holding the bucket & bucket fell on left ankle.
EE WAS USING A SLEDGEHAMMER ON THE CRUSHER RUN BIN AND HE TWISTED HIS KNEE.
EMPLOYEE WAS REMOVING ROCKS FROM CRUSHER; PICKING UP A ROCK AND PULLED MUSCLE IN LEFT ARM.
EE HAD BEEN SHOVELING USING A HAND TOOL.
