Employee reported pain in LT pinky after using a hammer to move a grizzly bar. Employee was evaluated and received 3 stiches, rigid splint, and an antibiotic prescription. X ray confirmed fracture in tuft. Employee released to full duty.

Fall from machine

Employee was tightening bolts on crane wench. Socket came off bolt and employee contacted ground. CT scan positive for LT pelvis fracture.

Employee was weed-eating the bank behind the office building when they slipped on the bank. The employee fell backwards landing on left shoulder and stove it up.

Fall from scaffolds, walkways, platforms

The employee was helping unclog a shoot non belt 8. After this was completed the employee stepped onto the walkway. The panel of the walkway gave way and the employee fell to the ground.

Employee was helping replace belt when the rope that was being used to pull new belt threw the conveyor structure broke striking the employees right hand. The employee was 35 feet away from the tail pulley at the time the rope broke.

This started out as a non reportable but due to complications it had turned into a reportable. The employee was helping unload some cross ties off a flatbed truck. When he cut the band around the cross ties they sprang forward pushed the employee off the truck to the ground. (3feet). Again this was a non reportable but due to complications it has turned into a reportable.

The metal sides of the rock box on the M 6ft x 20ft screen deck did not go up high enough to stop small rocks from bouncing out of rock box when the conveyor belt first starts up.

EE was removing a bolt from the right rear final drive when he pinned his left hand between the air wrench and the frame of the truck.

Ee was operating skid loader. he was starting to exit the skid loader and placed his right foot on the bucket. his foot slipped off the bucket and became pinned between the bucket and lock. he lost his balance and fell forward causing disolcation to three toes on his right foot.

EE ON HAUL ROAD; RAN OVER A DIP ON ROAD CAUSING SCRAPER TO BOUNCE & BOWL TO BOTTOM OUT STOPPING SCRAPER. THE FORCE OF THE SCRAPER STOPPING CAUSED EE TO GO FORWARD & HIT AGAINST STEERING WHEEL OF SCRAPER. WHEN HE HIT STEERING WHEEL IT THEN KNOCKED HIM OUT OF CAB OF SCRAPER. EE STATED HE BUCKLED HIS SEATBELT BUT EVIDENTLY IT WAS NOT SNAPPED IN THE LOCKED POSITION.
