Employee right thumb was smashed between hydraulic cylinder and pushing wedge. Resulting in a crush thumb.

Employee was using a wrench attempting to break a bolt loose with body in full tension and body in prone position approx. 6-8 inches off the ground. When the bolt broke loose, he struck the ground, buttocks first, but sustained no apparent injury. He awoke the next morning with severe muscle spasm in lower back.
Employee was using a chainsaw to clear/fell trees. Tree kicked chainsaw back between legs resulting in a laceration of leg requiring sutures. Employee returned to duty without restrictions.

Employee was swinging an 18 lb sledge hammer to tap a wrench to loosen a nut on the tension rod of a Hewitt Robbins jaw crusher. The tip of his left index finger was smashed between the hammer handle and a piece of steel. He was wearing work gloves & safety glasses. He was taken to the emergency room where he received 8 stitches.
EE WAS WIDENING A HOLE IN A PLASTIC COLLAR WHENT THE POCKET KNIFE SLIPPED AND CUT LEFT INDEX FINGER AT THE MIDDLE KNUCKLE, RECIEVED 6 STITCHES
Employee was descending the ladder steps of the R25. His right hand was holding the pole to the rearview mirror/handrail as he stepped down with his left foot. His left foot slipped off the top rung and he fell to the ground. On the ground, he looked at his left shin and saw a puncture wound about 1"" long by 1"" deep. He received 5 stitches.

EE PLACED HAND AT THE BASE OF THE STEPS ON A WEIGHTED TAKEUP. THE FORKLIFT OPERATOR LOWERED THE WEIGHT CAUSING THE SLIDE TUBES TO COME DOWN AND SEVER THE END OF HIS FINGER.
