EE injury his left knee while lifting a bucket of rock drill oil out of the drill truck. EE recently went to the doctor and filed a claim.

Flash burns (welding)

EE and a coworker were air arching on the main frame liner inside of the cone crusher. They had their backs to each other when the UV rays we think reflected off the inside of EE's welding hood and burned his eyes.

Right heel fractured after jumping off the second step of the WA600 loader that was on fire.

Employee was moving boxes w/paper & supplies several weeks ago. Injury was to lower back and was not known to be a work place injury until 6/4/04.First incident was in March, no medical treatment required. Recurrance on 5/24/04 medical attention was sought. Supervision was not notified of the recurrence and determination made that it was work related until 6/4/04.

Struck by falling object

EE WAS GOING TO UNLOAD A 1 1/2"" AIR IMPACT WRENCH FROM THE BED OF THE TRUCK. HE JERKED THE LATCH ON THE TAILGATE TO OPEN IT. THE TAILGATE DISLODGED FROM ITS MOUNTING POINTS, FALLING AND STRI KING THE EE'S LEG.
