Employee was lowering fork units to the ground and when it fell to the ground the other employee's finger that was in the expanded metal got bent and dragged causing a minor fracture to the tip of the finger.

Employee was washing material out of the bed of a truck and was struck by a 5"" piece of stone that was stuck in the corner of the bed with fine material. The stone rolled down the bed and bounced off a piece of rubber toward the employee striking him in the shin.

EE WAS REMOVING PUMP AT WASH POND. A 4"" PIPE WAS ATTACHED TO A 6"" PIPE. THE FLANGE BOLTS HAD TO BE CUT OFF TO REMOVE PUMP. THERE WERE 3 2"" PIPES THAT BRANCHED OFF THE 4"" PIPE. A 410 JOHN DEERE BACKHOE WAS PUT IN LINE WITH PIPE. A CHAIN WAS ATTACHED TO BUCKET AND LOOPED AROUND PIPE. THE PIPE WAS MOVED AND THE CHAIN SLIPPED CAUSING THE 2"" PIPE TO HIT EMPLOYEE.
