An employee was using a belt cutter to cut a conveyor belt. After the belt cutter was placed under the belt, the employee reached across the belt to slide the cutter even with the cutline drawn on the belt. As the employee reached across the belt felt something in right arm pull.

Technician was standing on front axle installing pin in lift arm boom. Slipped off axle and struck hip on machine. Contusion, low back pain.

FOREMAN WAS HELPING TWO WORKERS CHANGE CONVEYOR ROLLER WHILE ON CONVEYOR PLATFORM. HE LOST FOOTING/BALANCE AND FELL HITTING MOUTH ON CONVEYOR ROLLER CAUSING LACERATION TO UPPER LIP AND BREAKING DENTURE PLATE. REQUIRED FOUR STITCHES, NO LOST TIME.

THE BLASTER WAS TRYING TO STABILIZE REEL HOSE BY PULLING ON IT AND LOST HIS BALANCE TWISTING HIS WRIST.

Fall from machine

EE FELL OFF WATER TRUCK WHILE GETTING INTO CAB. HE HAD FILLED UP THE WATER TRUCK AND CLIMBED UP THE LADDER TO GET INTO THE CAB AND LOST HIS FOOTING WHILE GETTING INTO THE DOOR. HE FELL ~8' TO THE GROUND & LANDED ON HIS RIGHT SIDE RESULTING IN CONTUSIONS AND ABRASIONS TO HIS RIBS, LEGS & BACK WITH SOME POSSIBLE MILD BACK STRAIN.
