Miner was operating the CAT 349F excavator approx 25-30' from an approx 42' highwall when it sloughed causing material to cave-in the bottom and top windshield allowing material to enter the cab trapping miner for <30mins. Miner made distress radio call and miners immediately began removing miner from cab. MSHA was notified <15mins. Miner received medical care on 10/4.

An EE along with IE and another labor where trying to move an aluminum piece of 12 inch pipe to get it to set correctly in coupling. As IE picked up pipe to assist EE, IE felt a sharp pain in IE's lower left side of IE's back.

Employee was repairing tracks and was driving a pin with a hammer when he was struck in the chest by a piece of metal. Reported this to management on 5/29 when he was sore in that area and was sent to medical facility. A small piece of metal was removed from his chest by the physician.

First knowledge of incident at 8:05; reported to MSHA at 8:12. EE suffered a massive heart attack while driving from the pit to the shop. He pulled over to the side of the roadway at the top of the pit and was found by other EE's following him. CPR was started and EMS was called. He was transported to Trauma Center and was pronounced dead @ appox 9:15 AM.

EE was putting adapter on the bucket cutting edge of the Cat 990 loader when he was struck by a metal chip. He was driving on the adapter with a 12 lb. hammer when a chip flew off and struck him in the upper right thigh. EE was taken to Pitt Memorial Hospital ER where the chip was removed and the wound closed with 7 stitches. Medical only. No lost time.

CHRONIC SILICOSIS. OUT OF WORK UNTIL FURTHER NOTICE. EMPLOYEE'S CONDITION WAS DISCOVERED DURING THE COMPANY'S ANNUAL ROUTINE MEDICAL SURVEILANCE. HE WAS REFERRED TO A SPECIALIST FOR FURTHER STUDY AND REMAINS OUT OF WORK PENDING DOCTOR'S RECOMMENDATIONS.
Cleaning gyratory bin, he stumbled, grabbed a chain that was hanging on a hoist and a second chanin that was on hoist struck him in the mouth, chipping front tooth.
EE IS A MECHANIC. HE WAS REPLACING A TRANSMISSION OIL COOLER ON A CAT 990 MACHINE. DURING THE PROCESS HE WAS USING A PRY BAR TO MOVE THE TRANSMISSION FOR POSITIONING AND THE BAR SLIPPED AND SMASHED HIS FINGER AGAINST THE TIRE.
