A train car wheel sparked on the tracks while leaving the site, starting a brush fire on the mine property.

A tree fell on mine property due to high wind, causing it to fall on a powerline, causing it to arc. This resulted in two brush fires to start burning on the mine property.

Employee reported chest pain while operating the Primary Controls in the plant. Employee was taken to the emergency room by ambulance. Employee was found to have had a collapsed stint and heart incident.

Employee was standing next to a conveyor when a rock fell from the conveyor striking his left hand causing a laceration.

Employee was changing a hydraulic hose on a front-end loader and injured his left arm.

Employee pinched left pinky finger while working on a clutch assembly requiring 8 stitches.

Trespasser entered wooded area near entrance road and died of a self-inflicted gunshot wound.

Miner was exiting pickup truck and apparently placed right foot out of truck to exit. When weight was placed on foot that was on a rock his ankle twisted and he fell to the ground. Fractured small bone in ankle.

Suffered a hairline fracture of his pelvis & numerous bruises when the loader overturned. He worked his way around the feeder until he was in a position sideways on the ramp. He dumped a bucket of wet material into the feeder and some of it stuck in the bucket, he backed up & exceeded machine's center of gravity &overturned. He unbuckeled his seatbelt and climbed out.

LACERATION TO HIS HAND REQUIRING 8 STITCHES. CHANGING A BEARING, RAISED TAIL PULLEY WITH A PRY BAR. PRY BAR SLIPPED CATCHING HIS LEFT HAND.

HIT FINGER WITH HAMMER WHILE PUTTING A BUCKET TOOTH ON RESULTING IN LACERATION REQUIRING STITCHES.

BACK INJURY OCCURED WHILE EE WAS SHOVELING ROCK SAND OUT OF THE STOPPED UP SCREEN ON THE SAND CLASSIFIER. STRETCHED TO AN AWKWARD POSITION TO REACH THE MATERIAL AND FELT A PAIN IN HIS BACK.

CUTTING OPEN A SILICON TUBE WITH A BELT KNIFE, PULLING TOWARD HIS BODY. IT SLIP CUTTING WRIST, 6 STITCHES.

FALL FROM DRAGLINE BOOM RESULTED IN BROKEN RIBS AND DISLOCATED SHOULDER. EE WAS PERFORMING ROUTINE MAINTENANCE GREASING THE SHEAVES ON THE GANTRY. THE BOOM WAS LOWERED TILL THE BOOM POINT WAS AT GROUND LEVEL AND THE GANTRY WAS LOWERED TO THE BOOM. HE WALKED DOWN THE BOOM AND GREASED THE LEFT SIDE, HE SLIPPED AND FELL 9' TO GROUND STRIKING THE BOOM STRUCTURE ON THE WAY DOWN.

ROLLER FELL HITTING EMPLOYEE ON THE LEG. CUT LEGREQUIRING 4 STITCHES.

EMPLOYEE RECEIVED CONTUSIONS AND LACERATIONS TO THE HEAD WHEN THE GRADER DOOR SLAMMED SHUT ON HIS HEAD. THE GRADER WAS BEING USED TO LEVEL THE GROUND AT THE STOCKPILE AREA. THE GROUND WAS VERY HARD THE BLADE WAS CATCHING AND BINDING. AT ONE TIME IT CAUGHT HARD AND EMPLOYEE STUCK HIS HEAD OUT THE RIGHT SIDE OF THE CAB DOOR TO SEE WHAT IT WAS CATCHING. BLADE BROKE FREE THEN CAUGHT.

EE WAS PUTTING BELT CLAMPS IN D CONVEYOR BELT. HE HAD TO CUT A PIECE OF BELT. KNIFE SLIPPED AND CUT LEFT LEG. REQUIRED STITCHES.
