Employee passed out from a personal medical condition.

Employee became ill while weighing customer trucks. 911 was called immediately, MSHA number was also called. ee was transported to Iredell Hospital, was diagnosed with 2 tumors on his brain. The tumors triggered a seizure which is what ee suffered in the quarry office. *This is not a reportable injury however we are reporting because we called the 800 number as a precaution.

Ee was installing a saddle onto the mainframe arm of the omnicone 1560 crusher and caught the end of his middle finger on his left hand between the saddle and the frame arm removing the skin degloving from the tip of the finger. Dr treatment included skin graft making this a reportable medical injury with no lost workdays.

Fall from ladders

EE HAD CLIMBED AN EXTENSION LADDER TO OBSERVE WORK BEING DONE IN THE BED OF A KOMATSU HAUL TRUCK. THE LADDER WAS LEANING AGAINST THE TRUCK BED & SLIPPED RESULTING IN THE EE FALLING WITH THE LA DDER TO THE FLOOR.

EE WAS INSTALLING A RIDE CYLINDER INTO A HAUL TRUCK USING A CHAIN HOIST WHEN CYLINDER SLIPPED STRIKING EE ON LEFT THIGH.

EE INSTALLING NEW TAIL PULLEY, THE PULLEY BECAME WEDGED IN CONVEYOR FRAM. EE WAS TRYING TO THE PULLEY WHEN PULLEY CAME LOOSE AND TRAPPED ON HIS LEFT HAND.
