Employee was assisting in removing a rock jam in the primary crusher. A rock slipped and struck his finger on his right hand. This injury resulted in three stitches.

EE was operating Haul Truck, he reached down to pick up a sweater that was next to the buddy seat. The truck has an air conditioning unit under the seat, his hand went into the hole where the fan is located cutting the 3rd finger (ring finger) on his right hand. He was taken to the hospital, this cut required 5 stitches.
EMPLOYEE WAS OPERATING ROCK BREAKER. THE ROCK BREAKER BROKE THROUGH A ROCK, THE MACHINE SHOOK HIM. EE FELT BACK PAIN AND REPORTED IT TO THE SUPERVISOR SHORTLY AFTER. HE CONTINUED HAVING BACK PAIN AND WENT TO THE DR 2/2/06.

EE was getting off a 980 f loader and when he got to the last step, he put his foot on the ground. he didn't see a pothole and his right ankle twisted and his weight went down on it. He went to the hospital and they said it was a bad sprain. Became a lost time on 6/17/05.

EMPLOYEE WAS STANDING ON TILT CYLINDER BASE, RATHER THAN ON THE CATWALK, CLEANING HIS WINDSHIELD, THERE WAS ICE ON THE PLACE HE WAS S TANDING. HE SLIPPED ON THE ICE, FALLING AND HITTING HIS LE FT ARM ON THE HEADLIGHT FRAME CUTTING HIS LEFT FOREARM BELOW THE ELBOW, REQUIRING STITCHES TO CLOSE.
EE WAS REMOVING A WIRE SCREEN FROM THE OIL DRAIN TUB IN THE SHOP WHEN THE ROUGH EDGE SLIPPED AND CUT HIS HAND.
