Employee was removing metal from conveyor and struck finger against the magnet.

Employee was assisting in crusher bowl removal and was moving pusher arm into position. Hand slipped on the cylinder allowing finger to be pinched in the retracting pusher arm.

Employees were beginning to pour a crusher. One employee was removing the bolts from the box located over the crusher. One of the bolts was bent and would not pull out of the box. The employee reached to try to pull the bolt out, the box shifted pinching the employee's tip of the finger between the box and the head of the bolt.

An employee was rolling a piece of scrap metal into a machine bucket when a burr caught his glove. This resulted in the piece of metal smashing his pinky finger. The finger required stitches.

EE was working on welding shop roof performing repairs to roof. He was wearing safety harness & rope lanyard. When he relocated towards edge of the roof to be lowered down by the man-basket, he stepped on a skylight causing him to fall through. Slack created in the lanyard caused him to strike the floor. Results, cut on rt arm 3 stitches, fracture to lt elbow. RTW next day, lt duty

EE was assisting in changing out a motor for the Primary Crusher. Upon removing the old v-belt sheave, the injured EE pulled on the sheave to check if it was tight, the sheave then slid down the shaft and fell on the EE's finger, resulting in a fracture.

EMPLOYEE WAS TAKING JAM NUT LOOSE ON BOLT THAT HOLDS JAM BREAKER TO STRUCTURE WITH A 1"" DRIVE AIR WRENCH. THE JAM NUT CAME LOOSE CAUSING THE NUT TO TRAVEL UP THE BOLT. HIS RIGHT THUMB WAS PINNED BETWEEN PART OF THE TELEDYNE AND THE AIR WRENCH. HE HAD TO REVERSE WRENCH TO GET HIS FINGER BACK OUT
