Employee slightly bent over to lower a trailer onto a tow hitch using the trailer jack and pulled a muscle in EE's lower back

Miner was using a hand controlled loader/skidsteer to clean surge tunnel. Miner's finger became caught between the controls and the frame of the conveyor causing a laceration and compound fracture.

A rock jam occurred in the box over the 7' crusher. While attempting to remove the jam, the bar that was used shifted, striking EE in the mouth. This resulted in 8 stitches.

EE was holding the hydraulic wrench used to tighten swing die wedge bolts. While tightening a bolt, the bolt started getting tight, the wrench started to shift up mashing the EE's middle finger on his left hand. The injury resulted in 5 stitches and a fracture to tip of finger.

EE was exiting the boom truck, his foot got caught between the truck and fuel tank. EE fell straining groin muscle. He went to the doctor 3/13/2006, received treatment and returned to work. After a period of time he complained that the injury still existed, returned to the doctor 4/3/2006 recommended three weeks recovery.

EE WAS USING A HOIST CHAIN TO MOVE A METAL DOOR TO REDIRECT SCREENINGS. HOIST CHAIN HOOK SLIPPED OFF OF THE METAL DOOR AND HOIST ARM STRUCK THE EE IN THE MOUTH, CUTTING HIS LOWER LIP. REQUIRED STITCHES.

EMPLOYEE STATED THAT HE HIT HIS HEAD ON A PIPE WHICH CAUSED HIS HARD HAT TO STRIKE THE BACK OF HIS NECK WHICH CAUSED A KNOT ON HIS NECK. HE SAID HE REPORTED IT TO THE PLANT LEADMAN BUT NOTHING WAS DONE BECAUSE THERE WAS NO UNUSUAL PAIN OR ANY COMPLICATIONS AT THE TIME. ON 3/11/05 EE REPORTED THE INCIDENT AGAIN AS HIS DR. SAID HE COULD NEED SURGERY, SURGERY REQUIRED 11/07/05.

EMPLOYEE CLEARING JAW JAM WAS KNOCKED DOWN ACCIDENTLY WHEN BELT WAS STARTED ACCIDENTLY. THIS INCIDENT HAPPENED ON 7/10. ON 7/22 HE WAS PUT OUT OF WORK BY THE DOCTOR FOR CONTINUING NECK PAIN.
