Driller slipped off drill rig falling approx. 2'. Landed on feet and jammed right knee.

Fall to the walkway or working surface

EE stepped through a rotted piece of flooring in front of the furnace in the Ogden scale house. It was found that the furnace condensation line had been leaking directly under the furnace and rotted the plywood flooring adjacent to the furnace.

Struck against stationary object

Worker installing rubber skirt on main crusher area scatched arm on bolt. There was no concern for severity at that point, and the worker returned to work. Worker reported the next day that he had gone for stitches during the night because bleeding continued, making injury recordable.
