Over-exertion in wielding or throwing objects

EE was throwing a piece of metal in the bed of truck and felt a small strain by EE's groin on right side then a bump came and it is hot to touch now. EE will be out of work for 3 days.

EE was trying to climb into the truck and had EE's hands on the handrail. EE had to pull self up to the next step and started to feel sharp pain in EE's right hand going all the way to EE's arm. After miners latest doctor visit, Miner has been put on ""Do not Work"" status per miners Medical provider until next evaluation.

Employee experiencing shoulder pain on 12/3/25 from a preexisting injury that occurred on 4/18/25. Employee slipped on ladder descending from 70-ton truck. Right hand slipped, quickly re-grabbed rail, felt pain in right shoulder.

Employee was walking in tunnel when EE slipped on EE's left foot and EE's right foot stayed put. EE felt a pop in EE's right knee and felt pressure.

Associate was removing hydraulic pipe from excavator boom. When fitting was loosened, pressurized hydraulic oil (cold) sprayed EE in the face causing EE's hard hat to be pushed back and EE's safety glasses to come off. Associate flushed eyes with eye wash but was taken to Emergency Room for discomfort as a precautionary measure.

Miner was working on removing stones from a chute when EE started feeling discomfort in back, backend, arms and neck area. Next day miner was in pain.

While stepping down from a platform, the employee rolled their ankle and injured left ankle/leg. Employee was unable to walk so 911 was called and the employee was taken to the hospital for evaluation.

Employee was closing the hood of the service truck. When they closed the hood of the truck, they noticed a pain in their right upper pectoral area.

Miner was tested for annual hearing loss. Retesting has stated that the miner is above the threshold of 25% deeming a hearing loss on miners part 62.

A few hours after the employee's shift ended EE called saying EE felt a tweak in left shoulder at work while using the high torque wrench to tighten the primary crusher bolts. When EE got home EE noticed some swelling and the pain got worse so EE went to the ER to get shoulder checked. EE was given prescription pain medication.

Employee was cutting rotting pieces of tower floor with a torch. While removing a piece with a pry bar the slag from the previously cut welds hit employee below the safety glasses. Employee reported a burn and swelling under EE's eye.

Employee felt discomfort in EE's right eye while grinding. EE thought EE flushed everything out but woke up the next day with continued irritation so EE went to the doctor. Doctor confirmed there was no foreign bodies remaining in eye but prescribed eye drops out of precaution. **Employee wears prescription safety glasses

Struck against stationary object

While attempting to remove a wheel chock that was stuck underneath the operator's haul truck EE kicked the chock and fractured EE's right big toe.

Employee was moving a boulder in the pit when they experienced back discomfort. Was not reported until 3 days later.

Customer driver exited the truck, stood on the back bumper to clean back of truck, when frozen material came down pinning customers leg between body of truck and frozen material. Customer was able to release their leg and remained on the ground for EMS arrival. Customer was taken to hospital. Compound fractures.

Employee had out patient surgery on right knee to remove a small fragment of chipped bone. Employee on 1/16/19 reported knee discomfort when employee went to start up a power washer that had a hand pull power cord. On 3rd pull, felt knee discomfort.

While lancing lower hoist pin with lancing equipment, slag dripped onto steel toe shoe landing on cloth tongue burning through and top of foot. 1/25 original incident date-no recordable medical attention, 2/6 follow up request - prescribed ointment provided for 7-10 days. While removing the counterweight from a Vibrating Screen Shaft, the Counterweight shifted/motioned to the right due to the location of the chain used to hold it. Employee's right hand remained on counterweight with motion and became pinched with stationary Shaft next to it.
