Contact with hot objects or substances

While cutting steel with a torch under the primary crusher, sparks caused employee's pants to catch fire near EE's ankles. The fire watch told EE EE was starting to burn EE's pants, so EE stopped cutting and put fire out. Employee received burns on both ankles and calf area. EE will be a lost time from 12/24/25 and will be reevaluated on 1/2/26.

Over-exertion (Not Elsewhere Classified)

While walking to the long reach excavator, employee twisted knee while stepping through ruts in the frozen mud. EE feels some discomfort but refused medical attention at this time. Miner went to EE's doctors on 1/8/2025 and was prescribed physical therapy. No restrictions/ no time lost.

Over-exertion in lifting objects

Employee was shoveling overhead onto a conveyor belt. This resulted in discomfort in EE's left shoulder. Employee claims they did not feel the discomfort towards the middle to the end of their shift. After getting an MRI, a tear was revealed in EE's labrum. Employee continued to work until the scheduled surgery on 12/20/2023.

Accident type, without injuries

Miners noticed smoke coming from Tower 6 in the secondary. 911 was called and was able to contain the fire from reaching other towers. There were no injuries but substantial loss to equipment.

Contact with electrical current

At about 11:25pm a 2nd shift employee was welding on an arm guard to the primary crusher when EE felt an electric shock in elbow. EE asked the fire watch to turn off the welding machine and found that the leads had worn through grounding to the primary head causing the arc. The employee was brought to the hospital as a precaution and was given prescription medication.

Accident type, without injuries

Around 2pm a CSX freight train caused fires in the surrounding areas along the tracks due to a locked up wheel. Around 6:10pm the fires made their way to mine property. No injuries or major damage occurred on mine property but the brush fire burned semi controlled for longer than 30 minutes. MSHA was called at 6:34pm.

Employee was installing cylinders on the primary grapple when they hit their thumb with a hammer. After a week of the small cut not healing they went to a doctor on 3/15/2023 where they were given ointment as a precaution for infection. No restrictions.

Struck against a moving object

While being loaded by an excavator with shot rock, the employee says a rock hit the inner side wall of the dump body causing EE to shift in the seat and hit the inside of the door. Employee was wearing the seatbelt. Employee has pain in their shoulder.

While working in the C39 tunnel the employee rolled left ankle slightly. When the employee's ankle rolled it caused a small fracture in the 5th metatarsal.

While working at Tilcon NY Haverstraw Quarry, employee was fusing pipe and pinched right hand middle & ring fingers resulting in injury to nail area of middle finger & tip of ring finger. Injured party taken to hospital by ambulance.

while using hydraulic torque wrench to tighten eccentric bolts on the primary crusher, the wrench came loose when pressurized. As employee attempted to hold/control the wrench, their right shoulder was extended causing a strained bicep.

Employee assisting with flipping a conveyor belt troughing roller and the roller struck his right hand middle finger tip causing a fracture and also required stitches. He returned back to work same day and able to resume work.

Employee was working with other employee holding a 1/2""x6""x42"" plate to be welded to a structure. Other employee placed tack weld and released hold. Weld did not hold and injured employee was left holding weight, felt pain in right arm. Initially no findings until 2/11/13 when it was discovered he had torn bicep. Employee has been working since day of incident.

Employee's leg became stuck in skid steer bucket causing him to fall. He received an injury to his shoulder and was released back to work with no lost days. The pain continued and the employee was scheduled for surgery on 5/6/2009.

Employee was in manlift working at sand plant cutting conveyor belt with knife. While employee was cutting, the belt shifted, causing employee to slip. This caused the knife to strike the employee's hand and cut through the glove.
