EE injured right shoulder while setting up brick rig in kiln with other EE's. Possible dislocation with spontaneous relocation. EE has light duty and follow-up with Orthopidics

EE attempted to pick up a 2-1/2 gal. plastic jug of oil and pulled muscle in back. EE was given light duty (10lb restriction) for one week.

EE exiting personal vehicle and slipped in parking lot. EE thought he slipped on round object but could not find anything.

EE stated that he stepped of haul truck ladder at end of shift and aggravated a pre-existing back injury. EE stated he twisted back, possibly from a loose stone or small hole. EE did not feel any pain at that time, but woke the next day with back pain. EE's attending Dr. gave him 2 days off work, and returned to work with no restrictions after his scheduled week of vacation.

EE tripped while walking on drag chain in an attempt to mark it with spray paint for maintenance repairs. EE hit back on drag chain deck plate and suffered fracture to right rib. Prescribed pain medication. No restrictions.

EE was hauling dust from waste dust silo to the dump pile. After break at 4:00 AM, the EE was heading back to the job site from the break room and ran into Jersey Block type berm, hitting head on the right side window of the truck. EE was found at approximately 6:00 AM where he was semi-responsive.

EE was shoveling hard/frozen slurry and hit shovel hard and heard snap in hand. EE having trouble closing his hand.

EE fell while supervising coal spill clean-up at tripper belt in the material storage shed.

EE was walking from Clinker Cooler when EE stepped off 3"" platform which he did not see, landing off balance on his R foot. When EE tried to recover with L foot, EE slipped in slurry falling to his R knee. EE flet pain in R knee and inside of R groin area.

EE was trying to position a movable conveyor belt to the west in the storage hall building. Once positioned, the conveyor belt moved to the east on its own. EE put down a steel wheel brake on the conveyor rail to stop the conveyor belt. When the EE reached over an I-beam to place the steel wheel brake on the rail, he felt something pull in his back.

EE was kneeling while working on grease lines for a pump being repaired in the maintenance shop. EE stood up and turned to take first step and foot got caught under an unrelated pump shaft. EE lost footing and fell into a 55 gallon barrel. EE treated for bruised ribs on 9/15/2009 and put on light duty for 4 days.

EE received flash burns to his eyes while working in proximity to welding operation. EE did not notice until after getting home. EE recieved treatment on 9/11/2009. EE returned to normal duty on next scheduled shift.

Employee slipped while removing coal from a grizzly bar. Employee attempted to catch himself and hit a wall and pulled a muscle in his right arm above the elbow.

Employee was climbing off an excavator to go to lunch and stepped on a rock. EE twisted ankle.

While Jack Hammering broken bolt from the cradle frame, the hammer slipped off and pointed to the floor. As it was going down, a piece of the shaft guard of the adjacent equipment was sticking out and pierced his left ring finger, causing a laceration.

Employee exited work area at end of shift and was bumped by supervisor. Employee fell backwards and fell on right wrist.

EE pinched right hand fingers in bag loader saddle while inspecting roller bearings on bag loading conveyor.

While packing cement, a bag of cement fell off the pallet that was being stacked by the robot. Employee stopped the machine, went inside to remove the bag of cement off the roller bed. Employee felt a sharp pain in his arm while moving bag. Employee thought it would get better, but it continued to hurt.

While flipping over an IKN cooler grate, his hand became caught on the edge. The clip on the edge of the grate cut through his glove and injured the middle finger on his right hand.

EE alleging that while walking in front of doors, slipped on ice covered by fresh snow in parking lot. Fell onto his back banging left elbow and right side. *Employee expects to have surgery on left elbow waiting for results of test, working FT.*

Allegedly after filling Env truck, employee returned to power disconnect for fuel and slipped on ice in parking lot. Fell onto left shoulder.*Employee expects to haave surgery when results of MRI received. Currently working FT.*

Employee was walking down steps to get a slurry sample and tripped on pipe left on steps. Employee worked with no restrictions until needing surgery on 03/23/2009. Light duty starting 03/30/2009 for two weeks.

Allegedly, while using a metal bar to close a bottom door on a coal care, bar slipped out hitting victim over eye. Laceration was caused by bar bitting victims safety glasses pushing them against his head.

Allegedly while adjusting the forks on the fork-lift, employee's right thumb snapped back. EE was working too fast.

EE went to lift 35pd jackhammer and felt a pull in lower abdomen.

Allegedly, while trying to get out of the 1K25 drag conveyor casing, victim slipped and fell against the flange on the cover.

Allegedly on 7/11/07 the victim was climbing on the chute to A2 Hammermill, lost his balance and grabbed onto an I-beam. He now states over time he accumulated Carpal Tunnel Syndrome at his wrist and had surgery performed on 1/21/08.

Allegedly, while removing chunck from gyp feeder, employee was hit in the face when large chunk came down hitting bit on chipping gun.

Employee was loading liners on a crate when it broke causing a pinch on the left hand 3rd and 4th finger. Employee was treated for Subungual Hematoma and was dispensed medication for pain.

Allegedly injured was descending an 8'ladder. When he got down to the 3rd or 4th rung from the ground he either missed the rung or he slipped, falling backwards hitting his head on stone ground.

EE lifted spool of wire and turned before raising up and twisted back. This was a pre-existing condition. EE was placed on restricted duty for 3 days.

ALLEGEDLY EMPLOYEE WAS TRYING TO OPEN ELEVATOR DOOR, HE FELT PAIN IN HIS LEFT CALF MUSCLE.

Allegedly, Employee was moving boxes and rubber conveyor belting around throughout the day. At approx. 1 pm employee experienced mild pain in his upper back.

ALLEGEDLY EE WAS CHECKING FLUID LEVEL IN A YALE FORKLIFT. HE FIRST CRACKED ""THE RADIATOR CAP"" THEN WAITED A FEW MINUTES AND THEN TOOK THE CAP OFF. AT THIS TIME HOT ANTI-FREEZE CAME OUT BURING HIM ON THE LEFT FOREARM

ALLEGEDLY, WHILE MOVING A KILN FAN WITH TWO OTHER WELDER/REPAIRMEN, HE FELT A MUSCLE PULL IN THE RIGHT GROIN AREA

EE was performing work on the clinker cooler grate line.

Allegedly, while walking into finish mills motor control center, employee tripped over cement build-up twisting his back.

ALLEGEDLY WHILE OPENING SLIDE GATE FOR SILO #91, EMPLOYEE WAS USING A 4' PIPE WRENCH. SHE FELT A PULL IN HER SHOULDER AND BACK AREA.

ALLEGEDLY, WHILE TRYING TO CLEAN OUT AN AIRSLIDE, PRESSURIZED AIR BLEW DIST INTO EMPLOYEE'S EYES.

ALLEGEDLY, WHILE EMPLOYEE WAS VACUUMING IN 1F15 ELEVATOR PIT, HIS HANDS SLIPPED OFF OF THE 4"" VACUUM HOSE CAUSING HIM TO FALL BACKWARDS.

ALLEGEDLY, EMPLOYEE'S FOOT GOT CAUGHT IN GRATING AT COAL UNLOADING AS HE WAS WALKING, CAUSING HIM TO FALL.

ALLEGEDLY, WHILE PULLING A TROLLEY WHEEL OVER A RAILING WITH A CHAIN FALL THE EMPLOYEE FELT A STRAIN TO HIS GROIN.

ALLEGEDLY EE WAS GETTING OUT OF A BOBCAT SKID- STEER LOADER WHEN HE STEPPED ON WET SLURRY, FALLING BACK ONTO THE EMPTY BUCKET.

ALLEGEDLY, EE CLIMBED UP ON LANDING TO CLEAN AND SLIPPED ON CLINKER, BANGED ON ANGLE IRON, CUTTING AND TWISTING LEFT KNEE.

ALLEGEDLY, EE WAS WALKING FROM ""C"" SILO LOAD CENTER TO THE LAST TRUCK WEIGH SCALE WHEN HE TRIPPED OVER A PIECE OF 2""PIPE, FALLING & STRIKING HIS KNEE ON A PIECE CONCRETE.

ALLEGEDLY, WHILE STEPPING ACROSS AIRSIDE FROM SILO #52 EE STUMBLED AND FELL LAND HIT #62 AIRSLIDE.

ALLEGEDLY, WHILE CHECKING AIRSLIDE FOR CEMENT FLOW, AFTER OPENING THE COVER, CEMENT BLEW OUT STRIKING EE IN THE FACE.

ALLEGEDLY, EMPLOYEES WERE PICKING UP LOWER PORTION OF THE FEED CHUTE WHEN THE FLANGE AT THE FURTHER END GOT HOOKED AT EDGE OF PLATFORM. INJURED EE WHO WAS RUNNING THE HOIST, GOT IN FRONT OF TH E CHUTE AND PUSHED DOWN. THIS CAUSED THE CHUTE TO COME FREE AND IT CAME FORWARD STRIKING THE EE IN BOTH LEGS.

ALLEGEDLY, EMPLOYEE WAS CLIMBING DOWN LADDER AT FEEDER BELT FOR SILO #D83 WHEN HE STEPPED ON A 8 OZ SPRAY CAN. HE THEN FELL BACKWARDS LANDING ON HIS LEFT WRIST.

ALLEGEDLY, EE IS CLAIMING BOTH OF HER WRISTS ARE HURTING FROM CARPAL TUNNEL SYNDROME. THIS CLAIM HAS BEEN ACCULMINATING FOR A PERIOD OF TIME.

ALLEGEDLY WHILE ON NORTH SIDE OF CLINKER COOLER EE HIT HIS HEAD ON DUCTWORK TO FAN.

ALLEGEDLY, WHILE CLIMBING DOWN LADDER FROM ID FAN, EE'S RIGHT FOOT SLIPPED CAUSING HIM TO TWIST HIS HIP.

ALLEGEDLY EE WAS REACHING FOR THE EMERGENCY STOP BUTTON ON THE STONE TRIPPER. HE REACHED UNDER A SET OF CROSS BRACES AND AS HE STOOD UP HE HIT HIS SHOULDER ON THE BRACES.

ALLEGEDLY WHILE TRYING TO OPEN ELEVATOR FELT PAIN AT RIGHT SIDE OF GROIN AREA.

ALLEGEDLY, EE WAS DESCENDING A 10' STEEL LADDER AND MISSED BOTTOM RUNG. HE CAUGHT HIMSELF, BUT TWISTED THE MIDDLE OF HIS BACK. EE DIDN'T LOSE ANY TIME UNTIL 11-17-01.

ALLEGEDLY, WHILE CLIMBING OUT OF #1 FINISH MILL EE STEPPED ON A PILE OF 1"" STEEL GRINDING BALLS AND SLIPPED AND FELL.

ALLEGEDLY WHILE SHOVELING CEMENT INTO A PORTABLE HOPPER EE FELT PAIN IN THE AREA OF HIS LOWER STOMACH.

ALLEGEDLY THIS INCIDENT IS FROM AN INJURY DATED BACK TO 1991. THE EE HAS BEEN OUT AT LEAST FOUR DIFFERENT TIMES BECAUSE OF PAIN FROM THE ORIGINAL INJURY. THERE WAS NO NEW INJURY AT THIS TIME B UT AGGRAVATION OF THE INJURY FROM 1991.

ALLEGEDLY EMPLOYEE WAS PULLING ON A DUST COLLECTOR BAG CAGE WHEN IT GOT CAUGHT. HE THEN JERKED THE CAGE UP AND FELT PAIN IN THE GROIN AREA.

ALLEGEDLY, WHILE CLIMBING OUT OF THE UNDERGRATE COMPARTMENT OF THE CLINKER COOLER, EMPLOYEE PULLED A MUSCLE IN LOWER BACK.

ALLEGEDLY WHILE WALKING BY F49 COMPRESSOR, EE STEPPED ON A 1"" STEEL BALL & WENT DOWN TWISTING HIS KNEE

ALLEGEDLY WHILE LIFTING BAR FOR DOOR ON #2 FINISH MILL, EE FELT A POP ON LEFT SIDE OF GROIN AREA.

ALLEGEDLY EE WAS PICKING UP HOOK FOR A COAL CAR AT THE COAL UNLOADING STATION WHEN HE FELT A PAIN IN HIS CHEST.

ALEGEDLY, WHILE WALKING DOWN STEEP SET OF STEPS FROM PACKING MACHINE, EE SLIPPED ON STEP TWISTING HIS RIGHT KNEE.

ALLEGEDLY WHILE CLEANING BETWEEN ROLLERS ON 2R8 BELT, WITH A SHORT HANDLE HOE THE HOE GOT CAUGHT BY THE BELT AND PULLED HIS HAND BETWEEN THE BELT AND A ROLLER.

VICTIM WAS SITTING IN OFFICE NEXT TO QUARRY AUTO REPAIR SHOP AND HAD A MASSIVE HEART ATTACK. WAS PRONOUCED DEATH AT COLUMBIA MEMORIAL HOSPITAL IN HUDSON. N.Y..

ALLEGEDLY, WHILE REMOVING DOOR FROM MILL, THE EMPLOYEE WAS HOLDING ON CHOKER CABLE WHEN CABLE SNAPPED, CUTTING HIS FINGER.

ALLEGEDLY, EE WAS TRYING TO SECURE A WATER HOSE TO A WATER GUN, WHEN THE LINE BLEW OFF AND EE FELL DOWN A 4' EMBANKMENT JAMMING HIS SHOULDER IN THE GROUND.

ALLEGEDLY, VICTIM WAS PUSHING A WHEELBARROW OF MATERIAL DOWN A RAMP CONSTRUCTED OF PLYWOOD WHEN FRONT OF WHEELBARROW CAUGHT IN SECOND PIECE OF PLYWOOD THAT WAS ON THE LEVEL SURFACE. HE FORWARD MOMENTUM HAD HIM CONTINUED FORWARD, FALLING OVER THE WHEELBARROW.

ALLEGEDLY EE WAS PUSHING WHEELBARROW OF MATERIAL OUT OF THE CLINKER COOLER GOING DOWN A RAMP, WHEELBARROW FLIPPED OVER, DUE TO A FLAT TIRE CAUSING EE TO FALL ON THE STEEL GRATING.

ALLEGEDLY WHILE TRYING TO LOOSEN A BOLT ON A FLANGE, EE FELT A ""POP"" AND THEN PAIN IN THE MIDDLE OF HIS BACK.

ALLEGEDLY WHILE PUSHING A WHEEBARROW DOWN A PLANK, WHEELBARROW GOT CAUGHT--STOPING SUDDENLY WITH EE FALLING OVER THELEFT SIDEOF WHEELBARROW.
