EE injured right shoulder while setting up brick rig in kiln with other EE's. Possible dislocation with spontaneous relocation. EE has light duty and follow-up with Orthopidics

EE attempted to pick up a 2-1/2 gal. plastic jug of oil and pulled muscle in back. EE was given light duty (10lb restriction) for one week.

EE exiting personal vehicle and slipped in parking lot. EE thought he slipped on round object but could not find anything.

EE stated that he stepped of haul truck ladder at end of shift and aggravated a pre-existing back injury. EE stated he twisted back, possibly from a loose stone or small hole. EE did not feel any pain at that time, but woke the next day with back pain. EE's attending Dr. gave him 2 days off work, and returned to work with no restrictions after his scheduled week of vacation.

EE tripped while walking on drag chain in an attempt to mark it with spray paint for maintenance repairs. EE hit back on drag chain deck plate and suffered fracture to right rib. Prescribed pain medication. No restrictions.

EE was hauling dust from waste dust silo to the dump pile. After break at 4:00 AM, the EE was heading back to the job site from the break room and ran into Jersey Block type berm, hitting head on the right side window of the truck. EE was found at approximately 6:00 AM where he was semi-responsive.

EE was shoveling hard/frozen slurry and hit shovel hard and heard snap in hand. EE having trouble closing his hand.

EE fell while supervising coal spill clean-up at tripper belt in the material storage shed.

EE was walking from Clinker Cooler when EE stepped off 3"" platform which he did not see, landing off balance on his R foot. When EE tried to recover with L foot, EE slipped in slurry falling to his R knee. EE flet pain in R knee and inside of R groin area.

EE was trying to position a movable conveyor belt to the west in the storage hall building. Once positioned, the conveyor belt moved to the east on its own. EE put down a steel wheel brake on the conveyor rail to stop the conveyor belt. When the EE reached over an I-beam to place the steel wheel brake on the rail, he felt something pull in his back.

EE was kneeling while working on grease lines for a pump being repaired in the maintenance shop. EE stood up and turned to take first step and foot got caught under an unrelated pump shaft. EE lost footing and fell into a 55 gallon barrel. EE treated for bruised ribs on 9/15/2009 and put on light duty for 4 days.

Flash burns (welding)

EE received flash burns to his eyes while working in proximity to welding operation. EE did not notice until after getting home. EE recieved treatment on 9/11/2009. EE returned to normal duty on next scheduled shift.

Employee slipped while removing coal from a grizzly bar. Employee attempted to catch himself and hit a wall and pulled a muscle in his right arm above the elbow.

Employee was climbing off an excavator to go to lunch and stepped on a rock. EE twisted ankle.

While Jack Hammering broken bolt from the cradle frame, the hammer slipped off and pointed to the floor. As it was going down, a piece of the shaft guard of the adjacent equipment was sticking out and pierced his left ring finger, causing a laceration.

Employee exited work area at end of shift and was bumped by supervisor. Employee fell backwards and fell on right wrist.

EE pinched right hand fingers in bag loader saddle while inspecting roller bearings on bag loading conveyor.

While packing cement, a bag of cement fell off the pallet that was being stacked by the robot. Employee stopped the machine, went inside to remove the bag of cement off the roller bed. Employee felt a sharp pain in his arm while moving bag. Employee thought it would get better, but it continued to hurt.

While flipping over an IKN cooler grate, his hand became caught on the edge. The clip on the edge of the grate cut through his glove and injured the middle finger on his right hand.

EE alleging that while walking in front of doors, slipped on ice covered by fresh snow in parking lot. Fell onto his back banging left elbow and right side. *Employee expects to have surgery on left elbow waiting for results of test, working FT.*

Fall down stairs

Employee was walking down steps to get a slurry sample and tripped on pipe left on steps. Employee worked with no restrictions until needing surgery on 03/23/2009. Light duty starting 03/30/2009 for two weeks.

Allegedly, while using a metal bar to close a bottom door on a coal care, bar slipped out hitting victim over eye. Laceration was caused by bar bitting victims safety glasses pushing them against his head.

Allegedly while adjusting the forks on the fork-lift, employee's right thumb snapped back. EE was working too fast.

Allegedly on 7/11/07 the victim was climbing on the chute to A2 Hammermill, lost his balance and grabbed onto an I-beam. He now states over time he accumulated Carpal Tunnel Syndrome at his wrist and had surgery performed on 1/21/08.

Allegedly, while removing chunck from gyp feeder, employee was hit in the face when large chunk came down hitting bit on chipping gun.

Employee was loading liners on a crate when it broke causing a pinch on the left hand 3rd and 4th finger. Employee was treated for Subungual Hematoma and was dispensed medication for pain.

Allegedly injured was descending an 8'ladder. When he got down to the 3rd or 4th rung from the ground he either missed the rung or he slipped, falling backwards hitting his head on stone ground.

ALLEGEDLY, EMPLOYEES WERE PICKING UP LOWER PORTION OF THE FEED CHUTE WHEN THE FLANGE AT THE FURTHER END GOT HOOKED AT EDGE OF PLATFORM. INJURED EE WHO WAS RUNNING THE HOIST, GOT IN FRONT OF TH E CHUTE AND PUSHED DOWN. THIS CAUSED THE CHUTE TO COME FREE AND IT CAME FORWARD STRIKING THE EE IN BOTH LEGS.
