Employee was grinding the edges of a 4 inch by 4 inch piece of metal using a stand grinder when the metal got lodged between the grinding stone and the safety guard pinching EE's finger between them. EE sustained a broken bone and the loss of the finger nail on right hand.

Employee was moving a heavy duty jack stand when EE lost EE's balance and fell to the concrete floor in the shop. The jack stand landed on EE's right hand causing a laceration that required 7 stitches to close.

Employee got his right middle finger pinched between screen panel and screen frame.

Replacing guard on drive shaft of loader, guard slipped pinching left ring finger between guard and truck frame.

EE burned his left hand with cutting torch.

WHILE CLEANING MATERIAL UNDER PRIMARY PLANT, EE HIT HIS HEAD ON A STEEL BEAM. HE WAS WEARING A HARD HAT AT THE TIME OF ACCIDENT.

EMPLOYEE WAS HOSING OUT A CHUTE CLOGGED WITH MUD. A LARGE CLUMP OF MUD FELL HITTING HIS WRIST PINCHING IT IN THE OPENING OF THE CHUTE. INJURIES INCLUDED A BROKEN LEFT ARM AND LACERATION REQUIR ING STITCHES.
