Caught in, under or between a moving and a stationary object

Employee was removing a chain that slipped off the lower drive sprocket of a stacking conveyor motor. After locking out the drive motor, employee was removing chain on ""slack"" side of upper sprocket. As employee did this, the tension on the opposite side, pulled the chain and employee's hand and pinched it between the chain and the sprocket.

Fall to the walkway or working surface

Employee was changing screen cloth on the #1 screen when EE lost EE's balance and fell hurting EE's right knee and left shoulder. The employee continued working without restrictions until lay off when EE had surgery to repair EE's knee. Was originally first aid.

While unplugging a feeder in the surge tunnel a rock rolled down striking the EE in the left hand causing a laceration requiring 4 stitches.
