Lead Technician was on their second day working alongside Field Technician to complete field assembly of an HD605-8. Work is being performed inside the equipment maintenance shop. Technician finished welding the left side tire guard on. Technician was using a cloth and solvent to wipe the welded area when they noticed something had entered left eye from behind safety glasses.
Operator of an excavator climbed up onto the machine to enter the cab. While standing on the track to enter the cab the operator fell backwards to the ground. operator dislocated their left shoulder, fractured their left hip and cut their left elbow requiring stitches.
A piece of steel was cut from a chute with a torch and slid down and pinched and burned the hand of the employee performing the work.
Two EEs were using tanker bars to adjust a radiator on a pallet to prepare it for shipping. As the two were both pushing on the bars, the injured EE lost control of bar and slipped off the radiator causing EE to fall to the ground. When EE fell, right knee impacted the concrete floor of the shop. EE immediately felt a pain in lower back, side, a leg.
While operating loader employee felt pain in right knee. This injury was 1st Aid until ee had surgery on 2/23/2018.
Hand rail broke causing employee to fall 4 feet
Employee slipped and struck knee with a wrench causing a small cut. Injury became reportable on 9/14/2016 when employee went for medical treatment and was give restrictions and prescription medication for infection.
Employee tripped over wheel chock onto concrete floor.
Cut finger on sharp bolt while changing cutting edge.
Checking for oil leak in hose, when oil squirted employee moved his head striking lip on sharp object (2 Stitches).
Employee struck left middle finger between rock and angle iron while unplugging crusher.
Pinched finger moving rock.
Changing plate in chute, plate slipped struck EE on left ring finger.
EE WAS OPERATING A 773B CAT QUARRY TRUCK ONT HE QUARRY BED WHEN A SPRING BROKE INT HE SEAT HE WAS IN CAUSING HIM TO JAM HIS LOWER BACK.
EE WAS CRANKING THE HANDLE OF THE LANDING GEAR ON A TRAILER. HE LET GO OF THE HANDLE, THE HANDLE THEN REVERSED DIRECTION, SPUN AROUND AND STRUCK HIM IN THE FACE (MOUTH CHIN AREA)
