Transmission was being installed inside a shop environment. The hoist was attached to one side of transfer case housing. Top of case was pulled off by hoist and broken piece cut the palm and two fingers.

Accident type, without injuries

As the ash truck operator was beginning to dump a load of ash, the load shifted causing the truck to rock back on to the endgate in a wheelstand. The operator was trapped in the cab until a manlift truck could be taken to the location to remove them from the operator's cab. This took in excess of 30 minutes.

Accident type, without injuries

Incident occurred on a stockpile at the power plant. While pushing coal to a feeder, a D11 dozer became stuck over one of the feeders. The operator was instructed to stay in the cab until coal could be removed from around it, this took approximately an hour. MSHA jurisdiction for this event is under dispute.

Over-exertion in lifting objects

Initial injury was reported on 8-25-07, while removing an air operated grease pump from fixture. Follow up visit on the 27th resulted in restricted duty.

Employee's left index finger was caught between oxygen bottle and railing in weld shop. Employee was seen at Reliance Medical and received 8 stitches.

Employee was installing a light fixture in the scale house and the light fixture slipped cutting the right thumb.

Emp was holding a straight edge to cut a template out of cardboard, while using a utility knife to cut the template, employee's hand was placed along the edge and while cutting along the straight edge the little finger of his left hand was cut. Emp received 6 stitches to close wound.

The injured employee was working on the rear electrical junction box of a Kress Haul Truck. The right side dump door of the truck was resting on a 10 ton floor jack approximately 10-12 inches from the floor. The employee unknowingly had his foot under the edge of the door when the jack suddenly slipped from under the door allowing the door to fall on the employees foot.

Repairing haul truck. Employee opened his toolbox and a welding rod fell out. As he attempted to catch the rod, his hand contacted the lid of the box, cutting his hand.

Operator of LDW 208 loader slipped from the access ladder & fell 6 feet causing him to fracture his left leg.

Emp states that he was removing a 1/4"" plastic tie wrap from a high PSI hydraulic hose using a 3"" razor knife when he cut his left thumb. Emp received 7 stitches.

There was a lrg coal spill on belt 17 headroller. Coal was being moved from coal spill to an openhole in the top of the surge bin that had not been used for this purpose in the past. The injured emp did not know that his hole was open and the emp came around the front of 17 belt chute and stepped into the open hole.

Employee claims that while operating a coal hauler she hit a hole in the ramp which caused her to bounce in the seat. Employee sought medical treatment on 12/23/04.

Emp claims he was testing the elvator door at the Coal plant. Then he inserted a tool to clean the elev door stop his right middle finger was pinched between the elev door & the tool resulting in a laceration requiring stitches and a finger tip fracture

Employee cliams he was exiting the service truck (Ford F-450) and when he stepped out on his right foot his ankle rolled over & caused him to fall. A minor sprain was detected by the doctor & employee was given a splint & released to full duty

A small fire occured at the Pinon pit on a dragline. This eqmnt was idled approx. 1 yr ago, we had welders cutting the top off so that we could fill the house with ash, sparks ignited a sm grease pan under the gear case. Fire was extinguish in 1 hr 50 min. Small fire utilized fire extinguisher but re-ignited & called for water truck to fully extinguish.

Employee claims that he was installing transmission into frame (Kress truck) using the overhead crane. Crane hit stop before transmission was lined up so employee pushed w/his left hand to guide it into place and caused him to pinch his left pinkie finger between the transmission & frame. Employee was wearing leather gloves at time of injury.

2 TIRES THAT HAD BEEN DISPOSED OF 100 FT DOWN THE SPOIL SLOPE OF PINON PIT CAUGHT FIRE. NO ONE WAS WORKING IN PIT AT THE TIME. WATER TRUCK EXTINGUISHED TIRES WITHIN 35 MINS. MSHA NOTIFIED. PINON RAMP 3 IS A NORMAL TIRE DISPOSAL AREA (MOST TIRES AT BOTTOM OF PIT) AND AN ACTIVE ASH DISPOSAL PIT. DUMP ELEVATION TO BE COMPLETED BY END OF AUGUST, 2004.

EMPLOYEE WAS REFUELING LOADER WHEN DIESEL FUEL SPLASHED IN HIS FACE AND CHEST AREA CAUSING MINOR BURN TO HIS LEFT EYE.

EE WAS SETTING UP EXHAULT TUBE ON GOB VENT TRAILER AND WHILE HE AND CO-WORKER WERE LIFTING TUBE INTO PLACE, HE TWISTED HIS KNEE. REPORTED THE FOLLOWING MORNING THAT LEG AND KNEE WERE STIFF AND SORE.

EE WAS AIR ARCHING OFF ANGLE IRON, ANGLE IRON BREAKING FREE AND HANGING. GRABBED ANGLE IRON TO BREAK FREE, DROPPED WHILE HOLDING ON TO FEDDER PAN. RESULTED IN LACERATION REQUIRING 6 STITCHES.

HE STEPPED OFF THE BED OF THE TRUCK ONTO THE BUMPER WITH HIS LEFT FOOT AND WAS IN THE PROCESS OF STEPPING DOWN AND TURNING, WHENHIS LEFT F OOT SLIPPED AND HE FELL APPROXIMATELY TWO TO THREE FEET TO THE GROUND. ON HIS WAY DOWN, HE HIT THE TRAILER HITCH ASSEMBLY, INFLICTING THE INJURY.

WHILE BOARDING THE UNTI TO REPLACE A VALVE, EE STEPPED ONTO THE SECOND STEP, AS HE LIFTED HIMSELF, HIS FOOT SLIPPED OFF THE STEP AND BUMPTED HIS LEFTKNEE AGAINST THE STEP RESULTING IN A SMALL BRUISE. THE SFAETY CLERK WAS NOTIFIED ON 2-26-01 THAT THE EE WORKED 2 DAYS OF LIGHT DUTY.

EMPLOYEE ATTACHED A CHAIN FROM THE RAM HANDLE TO THE TRACTOR SO THAT HE COULD LIFT IT OVER THE SIDE OF THE SLED. THE BATTERING RAM WEIGHING APPROX 500-800 LBS, SLIDE SIDEWAY AND PINNED THE EMP LOYEE'S LEG TO THE TRACTOR AND RECEIVED SIGNIFICANT BRUISES TO HIS LEFT LEG JUST ABOVE THE KNEE.
