Caught in, under or between a moving and a stationary object

Employee was removing a steel door from a pump box pinning the right little finger between the door and hopper.

Employee was screeding and shoveling concrete. At the end of the day the employee allegedly felt pain in right arm. The employee was taken to the industrial clinic on 07-10-09 with a diagnoses of lateral epicondylitis placed on restricted duty and referred to a therapist.

Employee allegedly strained lower back while operating a volvo skid loader.

In the process of removing a tail pulley guard the lower portion of the guard fell and struck employee in the wrist. This became reportable when electrical stimulation therapy was administered on July 23, 2008.
