Struck by flying object

While preparing to perform side frame/track repairs on a 4100A shovel. When the relief valve was ejected from the jack. Employee was struck by a pressure relief valve in the lower left abdominal/waist area. The injury resulted in the death of the employee.

Employees right-hand middle finger pinched between the drill pipe & lifting strap. Xray was negative for fracture/dislocation with minor bruising to finger. Medical provided performed fingernail trepanation & issued oral oxycodone & Ibuprofen. Second opinion requested -Stated that with conservative care such as O/C ibuprofen /ice packs would achieve the same desired outcome as FAI.

Employee in truck bed, properly bent down to place case of liners in bed. EE heard/felt a pop in hamstring. Placed ice on the leg and continued to walk. On 3/12/25 EE returned to work and complained of leg pain. EE was seen at clinic and taken off work for 2 days

Caught in, under or between a moving and a stationary object

EE was attempting to put an insulator on an anode which was about 4"" off the ground. As the EE went to hammer the insulator it caused the opposite side of the steer horn to come off the strongback. This resulted in the employee's hand being pinched between the strongback hook and the steer horn. EE was seen at ER and GHR clinic and was released back to work with restrictions.

EE was helping to replace hoist ropes on Shovel. EE was trying to pry the hoist ropes to the next rung on the hoist drum. EE lost contact with the rope causing pry bar to break free, smashing EE's left pinky between the pry bar and drum. EE was seen at the ER diagnosed with a fracture to left pinky & released with restrictions. Restrictions accommodated.

Driller was attempting to remove a 4"" core barrel from the Sonic drill head when driller left the drill controls & attempted to grab it while it was rotating to finish unthreading/removing it. When driller grabbed the barrel it continued to rotate & the driller began rotating with the core barrel, causing driller to sustain a broken left tibia & fibula, & a dislocated left ankle.

Employee had just finished walking along a path spraying CSS-1h Emulsion and was noticed to be out of breath. Shortly after, employee was noticed on their hands and knees on the ground and then collapsed face down. CPR was started and continued until an ambulance arrived and employee was transported to the hospital.

EE was working at the 24S welding and then went to help co-workers when EE was done. Upon return to the Shop employee began to feel nauseous and began vomiting. EE was taken to the ER to be seen. EE was treated for heat illness. EE was given a full release at the ER.

The 417HT was being loaded by the 24S at the 5950 bench of Little Rock. While swinging the bed of the truck the dipper made contact rubbing against the bed of the haul truck. Employee was taken in for observation to the ER and was placed on 1 day of restrictions.

EE was trying to loosen a bolt with a ratchet and socket when the ratchet slipped striking EE in the mouth. EE's front #8 tooth was chipped and EE had a small cut on lower lip. EE was taken to the dentist. EE had temp cap installed, 2 stitches to lip and given a full duty release to work.

Employees were pulling cable to a floating barge using an airboat, when the corner of the airboat dipped down. This resulted in the airboat continuing to dip until it went fully down placing employees into the pond. Employees were wearing all their PEE, including life jackets. Employee was taken to the hospital for observation.

Employee was using a pry bar to move a lower sheave wheel, when the pry bar slipped, and EE fell backwards onto the drill pipe container (on the buggy rig) and sustained 2 lacerations to the right forearm, as EE fell. EE was examined at the local hospital and received 5 staples and released to full duty.

Employee was exiting the rear stairs of a bulk truck when EE's right foot contacted with the ground EE's ankle bent over causing pain. Employee was taken to the local hospital for medical treatment.

EE working on dozer left door wiper. Linkage in a bind, placed left hand on wiper motor and wiggled linkage. Linkage freed with the wiggling returning to center; employee's left finger caught in the linkage. EE was taken to the ER, received 5 sutures, restrictions until 10/11/22 sutures were scheduled to be removed. Started scheduled days off 10/5-10/9, returned 10/10 restricted.

EE reported to EE's supervisor on 9/8/22 that EE had noticed a rash on 9/5/22 on EE's wrists that had since grown over the past three days. EE was taken to clinic and diagnosed with contact dermatitis. EE was released back to work with no restrictions. EE went on scheduled days off and returned to work 9/12/22.

Employee was working on shim on dipper they placed a bar between the latch lever and H-block. Either the bar, latch lever or H-block shifted creating a pinch point. The employees right ring finger was caught in the pinch point. Employee was taken to ER, they were released with restrictions and referred to Ortho for further evaluation.

Maintenance mechanic called out to work on the east starter sheet machine, as it had jumped a tooth on the up ender outside the chain sprocket. The mechanic used a pry bar for leverage to check the chain tension. The bar slipped catching finger between the bar and the chain sprocket. EE was seen at ER. X-ray showed a fracture to middle finger tip, EE was referred to ortho doctor.

Fall from machine

Operator had finished cleaning windows on the small water truck as EE was dismounting with EE's right foot to lower step EE's left foot slipped off the top step. EE let go of the grab bar as the slip caused EE's body to twist landing on EE's left side.

Employees were installing a high voltage cabinet on the shovel carbody. A skid steer was being used to hoist the cabinet sitting on a wood pallet into position. The injured employee was on top of the carbody reaching over to align the cabinet, when the injured employee signaled to lower the cabinet it bounced pinching the fingers between the mounting ledge and the cabinet.

EE was switching the Y09-10 switch when it failed startling them. Employee dove to the ground hitting a knee on rock. Employee was seen by orthopedic doctor on 2/3/20 diagnosed with contusion given a knee brace and taken off work.

EE operating west starter machine when hanger bar came out. Pushed emergency stop before machine jammed. EE asked coworker help reverse machine. EE put right hand on sheet feed platform to watch. As machine rotation reversed sheet guide pulled EE's right middle finger amputating the tip of middle finger distal phalange from 3/4-to full fingernail up.

EE had finished eating lunch & was walking when EE got a sharp pain to left groin. EE was taken to ER via ambulance, taken off work & referred. EE was seen 9/20/19 & released with restrictions. Follow-up on 9/27/19 OCC Med provider determined there was no mechanism of injury & was released to work full duty & given a referral to PCP for personal illness.

EE was cutting weeds when EE stepped back with left leg. EE said felt knee hyperextend and felt a pop. EE was treated on site by First Responder ice applied, but did not want further treatment. On 8/15/2019 EE reported to work and said EE wanted to be taken in.

EE was getting off a 6' ladder. EE was at the second to the last rung and stepped off thinking EE was at the bottom. EE fell landing on right shoulder and hit left elbow.

EE called supervisor on 2/20/19 to report that on 2/19/19 after clocking out EE was walking to the employee parking lot and rolled right ankle. EE was seen at the ER, fractured fibula, taken off work, referred to orthopedic doctor.

Haul Truck driver was being loaded when the first dipper released into the truck a large boulder struck the bed jarring the employee. EE was taken to the ER and was given time off work.

EE reported on 8/6/18 that they had hurt their knee on or around 6/18/18 between 7:30 a.m. - 11 a.m., they stated that they felt the knee pop while in a kneeling position. On 8/7/18 EE was taken to the doctor to be seen. EE was released back to work with restrictions.

EE was installing flexible rubber hose on the discharge side of the pump. When the hose slipped and struck EE on the lower right leg; sweeping both EE's feet from under EE causing EE fall to the ground into a sitting position. EE was taken to the ER to be seen. EE was diagnosed with a fracture to EE's right ankle.

EE was trying to remove a railroad tie when left index finger was smashed. The employee was taken to clinic steri-strips and glue were applied to the left index finger and ee was released without restrictions.

Flash burns (electric)

Electrician was re-energizing a 480V breaker when an arc flash occurred. Site contacted MSHA as the extent of the injuries were unknown at the time of incident. After arriving at UNM, injuries were determined to be less severe. Employee was diagnosed with 2nd degrees on 3% of body.

EE was taking measurements when EE felt a ""pop"" in EE's left hip. EE was brought to the safety building and requested to be taken in. EE was seen at the ER and referred to Orthopedic Doc. EE was seen by Orthopedic Doc same day and placed on restrictions.

EE was walking back to office at Warehouse when they tripped and fell. EE caught themselves on hands and knees. EE was brought to safety to be checked by first responder; refused further treatment. On 12/7/2017 EE was taken to clinic for reported right ankle swelling. EE was given an ankle brace and given a full duty release back to work.

EE was loading cathodes when one got hung up. EE reached in to try and pick up the sheet when the cathode weight shifted smashing the right little finger. EE was taken to GRMC. Fracture to right little finger, stitches and a finger splint.

EE was installing some bolts on a pipe flange. As EE repositioned self, EE felt a pop in the right knee with instant pain. EE was taken to the ER. EE was taken off work and referred to orthopedic doctor.

EE was removing rocks from the tongue of the trailer to hook it up to the tow vehicle. EE picked up a rock and the tongue moved in an upward direction, smashing the employees thumb between the rock and the tongue on the trailer. EE was seen at the ER and diagnosed with a fracture to the thumb. Released with restrictions.

EE was working on cell furniture install. EE went to lift the copper ""round bar"" that was inside the jumper frame to place it on the hangers. The bar slipped out of EE's left hand pinching right ring finger between the round bar and the capblock. EE was taken to the ER x-ray positive for fracture to right ring finger. EE given restrictions and released back to work.

EE was retrieving chock from pickup bed. EE was standing on the bumper and slipped causing EE to fall to the ground. Landing on left side. EE complained of left wrist pain. EE was taken to the ER.

EE was closing gate at the scrape yard & lost footing and fell. EE fell backwards to the ground landing on the left side of body. EE was taken to the ER & was given 4 days off.

Struck by flying object

EE was assisting in removal of gantry pin from the 24 shovel. A spacer, along with a hydraulic ram was being used. As pressure was being applied with the ram the spacer came out of place striking the EE in the right shoulder. EE was taken to GRMC x-rays negative for break. Diagnosed with Contusion to right shoulder. Placed on restrictions for 3 days; released back to work.

EE was exiting forklift when rolled left ankle. EE was taken to the GRMC ER diagnosed with a sprain. Taken off work and referred to personal doc.

EE noticed door open on a PSA gunk pumper. EE hit it to close it. The cable holding the door struck the employee's right hand. EE was taken to GRMC. EE was diagnosed with a fracture to right thumb and hematoma. EE was released back to work with a full duty release.

MayDay at 0136 hours for communication pole struck by lightning resulting in fire. A blade operator noticed the fire and called the MayDay. The Fire Brigade was dispatched. The fire was extinguisher at 0238 hours and a fire watch set up. MSHA was notified by site Safety at 0138 hours.

EE was changing a lip protector on the #4 Loader. While attaching the plow the bolts spun cutting through leather glove resulting in a laceration to the employee's right forefinger. EE was taken to the ER received four stitches and released back to work full duty.

EE was attempting to tighten a bolt with a pipe wrench. EE positioned self in a low stance (kneeling). EE started to pull the wrench causing it to come loose on the fitting and swivel around smashing thumb between the wrench handle and a steel guard plate. EE was taken to ER given four stitches, diagnosed with fractured thumb and taken off work until seen by Ortho.

On 2/26/16 EE was washing the 6-blade. EE bent over to remove the steam cleaner hose from the ripper. As EE stood up EE felt a pull in the lower back. EE did not report incident until 2/28/16 EE did not want medical treatment. On 2/29/16 EE stated back was feeling sore and wanted to be seen. EE was seen at GRMC placed on restrictions. On 3/4/16 EE was taken off work by doc.

EE was torquing a nut when he caught his finger between the RAD gun arm and the swivels and the inner part of the wheel. Taken to ER to be checked. Fractured left index finger, 5 sutures and returned to work with restrictions.

EE was attempting to recover a valve and small piece of 4"" HDPE pipe from under a rock. The EE attempted to lift a large rock with his left hand and pull the pipe with his right hand. The rock slipped pinching his right middle finger. EE was taken to the ER, received 3 stitches and diagnosed with a fractured finger tip and taken off work.

On 01/5/15 an employee slipped on an HDPE liner while fueling a work truck. The employee fell and landed on his left arm. An MRI revealed that the employee tore one of the muscles in his left rotor cuff. Surgery was required on 3/10/2015 to repair the damage to his shoulder. The employee is a supervisor and was able to work at full duty prior to the time of his surgery.

EE was walking on North cell line when he felt a pain in his knee. EE requested to go in and was seen at the ER. EE was taken off work and referred to Ortho.

EE was changing trash bags in the Truck Shop; as he attempted to pull a bag from the can he could not remove it. EE stated that after trying he felt a pain in his left wrist. EE was taken in to the doctor to be checked. He was placed on restrictions and released back to work.

EE was sampling blast holes when he hunched over to take another sample he felt a pain in his lower back. EE report to supervisor and initially refused medical treatment. On Monday, September 15, 2014 EE stated that he wanted to be checked as his back still hurt. EE was taken in and given two days off work.

EE was unloading bag of mortar from behind seat of pickup when felt something weird in his stomach. EE worked rest of day. On 9/5/14 EE called into work stated he had increased pain in his abdomen during the night. EE went to ER to be check diagnosed with Umbilical Hernia. EE was installing a support brace on a set of pallet shelving when the brace slipped landing on his right small finger. EE was taken to ER to be checked he was diagnosed with a fracture to his small finger. He was given a finger splint and released back to work.

EE was unloading a practice mannequin when he felt his leg buckle under the weight of the mannequin (approx. 150 lbs.). EE stated he heard his leg pop. EE was checked by first responders and taken to the ER. EE was diagnosed with a fractured Tibia and taken off work and referred to an Orthopedic Doctor.

EE was assisting with replacing hoist motor on 24 shovel. As EE was walking to get a wrench he stepped on a rock twisting his left ankle. EE was taken to the clinic to be checked he was placed on restrictions and release. EE went on 05.13.2014 for follow up with doc and given a full release back to work.

EE was stepping off the anode rack when his left ankle rolled; causing the employee to fall. The EE was checked by first responders and then transport to the ER to be checked. The employee was taken off work for a left sprained ankle.

EE was conducting post blast safety check when the material below him gave way. This resulted in the employee being engulfed with shot muck to waist. Mayday was called he was removed and transported to ER. He was diagnosed with a sprain to his knee. On going.

Employee was attempting to close the door to the HVAC trailer when she felt pain in her left shoulder. The employee was diagnosed with a shoulder strain and placed on restricted duty.

While working side by side other employees, employee collapsed. CPR efforts were unsuccessful. ALS intercept personnel declared death at 4:05 p.m. Following investigation, determination was made that nothing work related contributed to his passing.

EE was putting cathodes in cell. When he unlocked cap blocks cathodes fell into the cell. This caused electrolyte to splash up and under his glass into his eye. EE flushed and was transported to ER for a checkup. EE was prescription medication and sent to eye doc. Eye doc released employee back to work with no restrictions.

He attempted to relieve the pressure by turning on the Ignition Key and cycling the Implement Pilot Controls. He then continued to loosen the Clamps when the D-Ring ruptured spraying Hydraulic Oil across his face. When descending the ladder he missed the bottom step and landed harshly on his Right Foot.

EE was attempting to remove the pull cord/rope used to pull spool of the Grinh Winch. When the cord/rope begin to reel in catching the employees left middle finger causing it to strike the side of the winch. EE was wearing gloves. EE was taken to the ER diagnosed with a fracture and released back to work with restrictions. EE was seen on 3/22 by ortho specialist and taken off work.

EE was attempting to manually hold a ground strap that he was trying to bore strap lug to a bigger hole size with a drill press. As the bit made contact it pulled the strap along with his hand towards the side of the rotating drill bit. Removing the thumb nail. EE was taken to the ER and diagnosed with small fracture to his left thumb. EE was released with no restrictions

EE was installing chain links when a metal fragment embedded in the EE neck. EE was taken to ER to be checked. EE released back to work with no restrictions, referred to specialist. EE was seen on 3/8/2013 by specialist and the fragment was removed thru a small incision, EE received two stitches.

New EE was out with the training group when they stopped at the 6D ready line to use the porta john. While attempting to enter EE slipped and fell injuring EE left shoulder. EE was taken to the ER and diagnosed with a dislocated left shoulder. EE shoulder was put back into place and in a sling. EE was released back to work with restrictions.

EE was mounting blade. He had each hand on the grab iron and his feet on the steps. His right hand came loose from the right grab iron and he slipped; twisting his back. Taken to the ER and was placed on restrictions.

EE was walking out of the sign in shack to the morning line up to report to work. As he walking down the stairs his left foot caught the edge of the 2nd step resulting in the EE falling. EE was taken to the ER and placed on restrictions for two days.

EE was jarred while operating haul truck. EE notified supervisor, but initially refused any medical treatment. EE worked her last shift on 4/18/2012 was off two days and came back for her first scheduled work shift on 4/21/12 and requested to be taken in to clinic. EE was placed on restrictions.

While at the Reclamation Office Trailer located in Tyrone, New Mexico Mine the employee attempted to lift a heavy 44 gallon barrel trash bag into the cargo bed (tailgate) of a f-150 pick-up truck by herself. Employee heard a pop in her right shoulder and was unable to lift her right arm. Employee sustained shoulder injury.

The wind had picked up in the afternoon of the 15th, sometime after that EE felt some discomfort in his left eye. He washed it out with water and it seemed to be okay the rest of the shift. He awoke the next morning and his eye was swollen and irritated.

EE and co-worker were picking up material left on ground from change out of a tooth on the shovel. EE and co-worker picked up the last piece of the lip protector and were putting it on the truck EE smashed his finger. EE was taken to office to be checked and taken in to the clinic. EE had a small fracture to his left middle finger. EE was released back to work without restrictions.

EE was working at the east unloading table when his respirator which was attached to his right hip; which got caught in the directional valve handle pulling him back. As the EE fell back he reached out with his left hand; resulting in his left forefinger being smashed. EE was taken to the ER and taken off work.

EE was setting out of the cable truck when he twisted his ankle on uneven ground. EE didn't go to clinic until Monday, August 22, 2011 when he was placed on restrictions and released back to work.

The employee was directed by an FMI employee to drill a manifold out. The drill given to use was used for vertical drilling, the guys were asked to drill horizontal with it. EE was holding the drilling shaft to keep it stable when his glove was caught on the shaft. The glove caught on the index finger and sprained the left wrist and broke the middle finger.

EE had just exited the boom truck bucket and was walking away when he felt a pain in his left knee. EE reported the incident on 8/10, but denied medical treatment on 8/11 EE came to work and requested medical treatment. After 30 mins his testicle descended. DR couldn't attribute to anything. Not until appt on/near 03/22/11 did the EE report a diagnosis of groin strain. Employer is still without backup & conf.

EE was carrying a Y-pipe to the storage room when the tail end hit the door jam resulting in the catalytic portion of the assembly hitting the employees left index finger cutting the finger. EE was taken to the clinic to be checked. The cut was cleaned and 3 stitches were applied. The EE was released back to work with no restrictions.

There was a fire at the Tyrone Hydromet Filter Clay Press, a mayday was called at 3:54 AM. Safety Pro notified MSHA at 4:17 AM of the fire at the Tyrone Hydomret. The fire was called out at 5:55 AM and Safety Pro called MSHA at 6:00 AM to notify MSHA that the fire had been called out.

EE was walking up stairs to enter building (it had snowed a little bit that night) and there was ice on the steps. EE slipped on the first step, resulting in him falling forward, striking his head on the ground. EE suffered a laceration to the side of his head. EE was taken to the clinic to be checked, he received 7 staples to the laceration and was released back to work.

EE was helping position 12"" DR15.5 HDPE pipe for fusion. While positioning the pipe the stored enegry created during the pulling of the pipe result in the pipe suddenly being projected over the stationary pipeline towards the EE striking the EE in the face/head. EE was transported to ER he received stitches on his hairline to forehead and had a broken nose.

EE was trying to maneuver a 55 gallon drum of used oil into place with a co-worker into the back of a pickup when EE felt a strain in his shoulder. EE was taken to the clinic and released back to work. On 2/7/2011 EE was taken off work due to restrictions that were placed by doctor.

On 10/6/10 EE notified sup and sr. sup during a meeting that he had an open Workers' Comp claim for his back. EE alleges that he injured his back on, but is unsure as to the specific date, but says that it occured on one of his dayshifts Sept 22-24, 2010. EE was also not sure as to what event caused his back pain .... This is under investigation ...

EE was building up bearing blocks for the shovel. When he finished making a pass, he lifted up his welding hood, when a piece of slag popped up from where he was welding and hit his eye. EE was taken to the front gate, eye flushed and then taken to the doctor. He was given a prescription for his eye and released back to work with no restrictions.

EE was pushing a cable under HDPE pipe. When his coworker grabbed the other side and pulled the cable, the cable rubbed against the EE's leg, ripping his pants and resulting in a small laceration to his leg. EE was taken to the ER and he received 3 stitches.

As EE was rotating the sheets on the scissors table, he reported he felt cramps in his lower abdomen that turned into sharp pains that went down around his groin area and right upper leg area. EE was attempting to remove a hose reel from its base, as he removed the last bolt, the reel spun coming in contact with the EE's left leg in the knee area. EE was taken to the ER and received 4 stitches and was released back to work.

EE was driving the TS22 Forklift to the 25 Shovel assembly site. When he turned the fan on some dust blew into his right eye. EE was taken into the clinic and referred to eye doctor. A foreign body was removed from and a presciprtion eyedrops were given. Employee was released back to work.

EE told the safety pro that the ""injury was not work related"" as he was filling out paperwork to be taken into clinic. On 5/4/10 EE alleges he hurt lower back when assigned to remove 3 & 4"" pipe from one section of the stockpile & install air vents. On 5/7/10 EE reported his lower back was sore to sup. The EE did not want medical treatment. On 5/10/10 EE spoke with his supervisor at start of shift reported his back was still sore and now wanted to be checked out. Still under investigation.

EE draining oil filter removed from equip. He set filter in container allowing fluid to fall in a secondary bucket. Draining oil was blowing on everything due to extreme winds. EE tried to prevent oil from blowning everywhere by placing drain tube from the collection drain to 2nd bucket. He cut a piece of 1"" vinyl tube with pocketknife and cut his right middle finger.

EE was going to the warehouse yard in the TS18F forklift yard to load a motor when the right rear tire blew out, resulting in a loud bang. The tire size 1200x20 had a 100 pounds of air. The EE said that he had a headache with some ear pain. EE was taken to the clinic. EE was taken off work for the inner pain.

EE was trying to loosen a bolt to remove the leads to the motor. The bolt gave resulting in the EE hand hitting the junction box. EE report incident to his supervisor. EE was taken to the ER his right thumb was splinted and he was prescribed an anti-inflammatory. He was released back to work.

EE was uncoupling a Vogelsang trailer mounted pump from the MS4 truck as the hitch cleared the ball the trailer tongue begin to move sideways away from the ball. EE realized the jack was falling and jumped backwards. As he landed he came down on his right ankle rolled. EE was taken to the Front Gate to be checked out and transport to the ER. Diagnosed with a broken right ankle.

The ee entered the manvan at shift change and proceeded to the rear seat. The ee put his seatbelt on and the man van proceeded out of the main pit. The manvan struck a hole in the road and the ee head was jarred into the roof of the manvan. EE was checked by ERRT team to have ee checked out and transported to the ER.

EE was mounting his RTD he was on his second step when he lost his grip with his left hand. Causing him to fall to the ground. EE got up and got in his RTD and noticed that it hurt to push the brake. He called his supervisor. EE was checked out by a First Responder and was then taken into the ER. Xrays were taken and confirmed a break in his leg below the knee.

Employee was returning to his vehicle when unknowingly stepped on a rock which caused his ankle to roll. In an attempt to prevent an injury to his ankle employee intentionally fell to the ground. The fall resulted in discomfort to his left side of his lower back.

Employee arrived at the shovel to start the PM. As he was walking to the shovel he slipped on some small rocks falling to the ground. Employee was taken to the safety building to be checked by a First Responder the decision was made to take the employee into the ER. Employee was taken off work.

Employee was helping to install the hoist cables on the 23 shovel. He was guiding the hoist cables up towards the sheave wheel when the second hoist cable came around striking the employee in the left knee. Employee was taken to the ER and taken off work.

EE was driving a company vehicle up an incline on the SX access road when he fell asleep at the wheel. The vehicle climbed up on the left side of the berb, traveled approx. 75 feet and came to a stop on top of the berm. EE was taken to the ER he was diagnosed with whiplash. EE was taken off of work.

Worker was attempting to lift a tire onto a trailer, when a co-worker suggested they wait for a CAT IT Loader in order to help lift the tire. The worker went to set the tire down and felt his lower back tighten. He worked for a short time afterwards and then sought medical attention.

On call safety pro received call from Tyrone Town Site Volunteer Fire Depart that a wildland fire was appoarching our property at 4:22. At 4:45 pm guard shack radioed that fire was not burning on our land. At 5:02 pm another EE called MSHA to notify them of fire and at 5:07 called the state mine inspector. The fire was called out at 5:57 pm.

It was snowing badly as the employee was stepping out of his pickup causing slippery conditions. The employee slipped and fell on his side hurting his shoulder and ribs. He got back into his truck and drove to the mine offices. He was checked by a first responder and then taken into the ER. He was diagnosed with a fractured rib. He was released to work.

EE was being loaded by the loader when a rock rolled out of the dipper into the bed of the haul truck, jarring the employee. EE alleges that when he was jarred he began to experience back pain. EE was checked by a first responder and then taken into the ER. EE was released back to work with 2 days restricted duty. EE worked his last day in the rotation on restriced duty status.

EE was unloading a drill bit that weighed approximately 130 lb. off the back of the truck onto a pallet. As he was throwing it on the pallet, his right glove got caught in the butterfly valve, taking down the employee as if fell to the pallet. EE was taken to the ER to be checked. He was released to work on a restricted duty status.

EE was greasing a loader in the shovel pit. As he was dismounting the ladder he stepped on a rock at the same time he let go of the ladder which caused him to fall and twist his right ankle. EE was taken into the ER to be checked & he was taken off work.

This is an 'Alleged Injury"", this report may be amended upon further investigation. EE bent down to lift a roll of felt with a coworker, before lifting felt a strain in his lower back, he took a break and went and ran a rock truck for a while. Around noon his back was still hurting and that's when the foreman noticed and EE asked to be taken to a doctor.

Worker reported to his foreman that while stepping down from the Cat D9R dozer in order to start a light plant, he felt pain in his lower back; the worker indicated he was fine. On October 3rd, the worker said that the pain had not gone away and requested to see a physician. The physician diagnosed a lower lumbar strain.

The unwitnessed EE stated that when he was lifting a roll of fabric, he mis-stepped and twisted his back. He was taken to the hospital, received Tx and injectable meds and scrips. He failed to show up 2 days later for follow-up visit with Dr. and we discovered that he had taken employment with another employer and denied any problem with his back.

Employee was helping mechanic replace cutting edge on 966H FEL. Mechanic was inpacting off nuts. The bolt became dislodged from holding socket and started to spin freely. Employee was trying to hold the back of the bolt in socket and a burr caught glove, ripped glove off hand. Employee received 8 stitches to left hand.

Three inch water pump had been cavitating causing the water to heat up inside the pump. When the hoses were disconnected it sprayed hot muddy water which caused first and second degree burns.

Employee was working in an arroyo to do erosion control for tailings dams using a 3/4"" drill. The drill bit grabbed in rubber, twisted his hand and broke it. The employee was taken to the doctor shortly after accident happened and placed off work.

Technician was removing edges off the bucket of a 966 H loader when she placed her hand on head side of the carriage bolt. The head of the bolt was worn and razor sharp, lacerating right middle finger. Laceration required 5 stitches at the emergency room. Full recovery is expected.

EE was placing a chock down to support a utility trailer. When the trailer came down prematurely, smashing the EE's right thumb. He was taken int the ER, his thumb was stitched up and he was placed on light duty.

A maintenance shift work electrican was replacing fuses on a 4100 P&H shovel when a 7200V transformer are flashed causing a burn to his right arm, EE was taken to the ER and from there transported to the burn center on Phoenix.

Surveyor was pulling wooden lath out of back of pick-up truck. He got a splinter from the lath in the side of his left hand below the pinky finger. Splinter became infected resulting in minor surgery to remove the splinter and medication to treat the infection. Surveyor was not wearing PPE, work gloves.

Haul truck driver was entering the shovel pit when he noticed no power on the shovel & smoke coming out of the back of house. The fire suppression system activated & a mayday was called at 11:26. The electrician killed the power-fire brigade was dispatched. The fire was out at 11:54 & a fire watch was put in place. At approx 12:10 it started to burn again & was called out at 12:26.

Wagner Equipment mechanic pinched hand while rolling a refill tank across shop floor, when wheel of tank got hung up, tech pulled on cart a couple of times and cart rolled forward and pinched hand between tank and bench vise. First aid was administered and full recovery is expected.

Tech was climbing up of roller frame to remove bolt from differential housing on D-11 Dozer. While climbing down slipped and fell hitting back of head on frame. A small laceration on back of head occurred. This was right below the strap of hard hat. No stitches were required.

Tech was climbing up back of 637 Scraper between transmission and rear fender to change a pump. Tech had grease on bottom of foot and he slipped and fell backwards. As he turned to catch himself, he fell to the ground approx 4 feet. He fractured his left wrist trying to catch himself.

Employee's scraper came to a sudden stop when the slack was released after the hook up. Afterwards, employee began to feel some discomfort in his neck and head. He was transported to the ER where he was checked out. He was taken off work. RTW 2/21/07.

Fall from machine

EE was mounting his truck when he lost his footing, causing him to fall back, twisting his knee. He was taken to the ER and given a prescription for pain and taken off work. On going.

EE was trying to open the drain in the cell. As EE was opening the drain, he slipped and fell into the trench, rolling his ankle. He was taken into hospital for futher examination. EE was take n off work and given a prescription for pain. On going.

Employee was placing pipe in fusing machine to get ready to cut. The valve that clamps the cylinder on pipe clamped prematurely causing his hand to move the other valve which pinched his right hand.

On October 3, 2006 employee reported a near miss. Employee refused medical examination at the time. On 1/8/07 employee notified company that he had seen his physician on 1/5/07 and was diagnosed with a lower abdominal hernia. He is scheduled for surgery on 1/18/2007. Ongoing.
