Fall onto or against objects

EE was rebuilding slurry pump at booster EE took rubber guard off of hub seal on pump and laid it on ground next to work area as EE continued to perform task EE tripped on rubber guard and fell on stump and injured lower right side of back and cracked a rib.

Struck by falling object

EE was working on the classifier and had the steel cover opened on the station. The cover fell on and lacerated his left middle finger. EE received six sutures to his middle finger.

Over-exertion in lifting objects

The office clerk injured her right wrist when she was moving a box of ticket paper into piece to feed the printer. A wrist splint was applied to support her sprained wrist.

Struck by... (Not Elsewhere Classified)

The ee was cutting a piece of rubber with his right hand. He was cutting away from his body and out his left pointer finger (was holding the rubber in place with his left hand) Resulted in taking off distal of left pointer. Work status is restricted duty, must keep clean, dry and covered. Wear finger splint. Follow up on Monday 8/2/10.

EE was walking and did not notice he was standing on ice, the ice was covered with sand. The ice broke and he went down to his shins and jarred his back.

EE stepped in grease before mounting equipment. When EE was climbing ladder to the truck, his foot slipped on step. He held on with one arm and bumped his arm into the trucks fire extingusher holder.

Struck against a moving object

A loading truck was off of the loading road and had his truck stuck in a loading area.Our loader operator was unaware of the vehicle directly behind him and as he backed was unable too see truck and made direct contact.No damage to loader still waiting on quotes for truck.The only injury was a neck strain to loader operator.

EMPLOYEE WAS INSIDE WORKBOAT PULLING ON POWER CABLE FOR DREDGE WHEN HE LOST HIS BALANCE STRIKING HIS KNEE AGAINST BOAT AND TWISTING HIS BACK.
