While trying to disconnect fill line on the back of the water truck employee smashed left index finger causing a cut on the top of the finger.

When exiting bathroom, associate stepped on a rock and rolled right ankle. The employee saw an ortho on 6/7/2023 and was signed out of work when an x-ray revealed a broken bone in EE's leg.

There was a fire at the scale house that was not extinguished within 30 minutes of knowledge. Ignition source currently unknown, no injuries.
While flipping a cutting edge over on the forks of a forklift the mechanic felt a pop in EE's left elbow.
Employee felt a pain in their shoulder when lifting an 18 lb. piece of steel into a dumpster. MRI revealed no tear but employee was put on light duty.
While grinding on sand screw during maintenance, employee felt a sudden pain in their back. Employee was seen by contractor physical therapist who urged them to go to the ER. The employee was diagnosed with a back strain and given a prescription painkiller and muscle relaxer.

Employee hit their head/hard hat on an I-beam in the plant when standing up. Due to the stiffness and pain not going away, EE saw a doctor who gave EE a prescription for a muscle relaxer. No light duty or other restrictions were given.

Employee was walking from the car to the time clock to start shift on Thursday morning when EE collapsed. Nearby employees heard EE fall and began CPR, called EMS, went for the nearest AED machine, and sent employees to escort EMS personnel to EE when they arrived. EE passed away the following Tuesday night (8/11).

while assembling lock post cylinder, right pinky finger became pinched between sleeve and outer ring of lock post cylinder. 8 stitches for laceration
While leaning foot on warehouse chain, associate went to lean elbow on wall, chain hooks broke, associate fell approximately 3'. Associate sustained abrasion to back of head and laceration to left elbow requiring 3 stitches.
While cutting an electrical cord/wire with a utility knife (razor end), employee slipped and made contact with left hand creating a 1"" laceration on the palm side under pinky finger.

While employee was fitting a vertical wear plate on the wall of the rear re-crush feeder box assembly, the upper corner of the wear plate became lodged on one side. As the employee tried to free the stuck plate and while doing so the plate dislodged and crushed finger.

There was no specific accident/incident. Reporting a hearing threshold change that meets MSHA's requirements to report from annual hearing test results. When his baseline testing is compared to current testing meets the reportable criteria for MSHA. Re-test completed on 9/2/14 confirm same results. Employee claims wax build-up possible infection: to follow up with ear doctor.
Employee went to reach for lunch box in back of pick up truck (driver side); when he went to lean back he lost conscious and fell backwards. 2nd employee heard fall, found employee unconscious with laceration to the back of his head due to fall. The loss of consciousness was due to personal medical reasons, staples were received to laceration from the fall

While attempting to loosen a turnbuckle on a speed reducer, the associate strained his right shoulder. Proper tool and position were identified, however, due to the heat the conditions were warm which may have played a role.
INJURED LEFT THUMB WHILE SWAPPING OUT OXYGEN TANKS ON SERVICE TRUCK. THE PROTECTIVE CAGE OVER THE BOTTLES FELL TO A CLOSED POSITION WHILE HANDLING THE OXYGEN BOTTLE. THE CAGE FELL ONTO HIS LEFT THUMB CAUSING SWELLING AND BRUISING.

miner was attempting to loosen a turnbuckle and chose to use a bar and hammer for the task, upon striking the bar, hammer recoiled, hitting miner on the upper lip area causing a laceration of approx 1cm. Miner received 5 stitches.

Employee was operating walk behind under a conveyor. While grabbing a bucket of material, the control end lifted and pinched EE's finger against return idler guard.

While operating walk behind skid-steer, the machine was backed into a support beam, causing the operator's finger to be pinched in the controls.
Employee arrived at work not feeling well. As the afternoon progressed he began to feel worse. He was driven to the emergency room at local hospital.
