Over-exertion in lifting objects

While shoveling off belts from the catwalk the employee jammed shoulder when employee contacted a belt clip. Initially the employee did not want further medical care. EE worked our 3rd party on-site injury prevention specialist for a few weeks but when it didn't get better EE was seen at an urgent care. The urgent care signed EE out of work on 1/19/2023.

Struck against stationary object

Hand slipped off chain wrench and became caught between back side of feed pump and metal box on T-4 drill.

Struck by flying object

While attempting to remove an ice blockage from a water line the blockage released and struck the associate in the face with water/ice causing a small laceration close to their eye. Out of precaution associate was brought to the ER where they received a prescription for eye drops. No loss of vision or pain. No restrictions.

while coming down maintenance truck, missed step fell and landed on side complaining of lower back pain

Third Party Blasting employee bumped into another employee from his company and fell on our property. INJURY WAS NOT REPORTED TO TILCON until today March 23rd, 2015 at 12pm. 3rd party company involved: Kemek Injuries sustained: landed on his hand to break his fall and later felt discomfort in his wrist and reported it to his supervisor. The outcome was a fracture to his wrist.

Fall from machine

While climbing down from haul truck at end of shift, employee missed initial step on the ladder falling in a standing position, upon landing fell backwards.

Employee was hitting conveyor frame with a sledge hammer to free up frozen material due to cold temperatures, hammered finger between conveyor frame and hammer. Broken tip on Index finger on right hand with stitches.

Employee was lifting wire screen cloth off of forklift on top of the screen tower. His finger became caught between the screen cloth standing upright against the handrail and the screen cloth being moved.

Installing chute liner with helper, the helper dropped the liner causing employee to strain his shoulder. Employee treated for shoulder sprain and did not miss any time from work. An MRI showed a tendon tear in shoulder and surgery was scheduled for 5/1/07.
