While performing mechanical maintenance inside of a controlled environment garage, the employee used a sledgehammer to dislodge a brace when the sledgehammer broke the weld the iron angel spun and struck the base of the employees right thumb.

While performing maintenance work in normal work conditions around the plant the employee got a foreign object in employee's eye that employee was unable to flush out requiring a medical professional to remove the object.
Working in the controlled environment of an enclosed garage when EE's hand slipped while installing a two-way radio into a company vehicle cutting hand between the thumb and pointer finger requiring stitches

Using sledge hammer on drill step, missed step with sledge hammer and hit left hand; swelled immediately.
EE was helping to lay 10"" plastic pipe across the floor of the quarry when EE slipped and tore EE's bicep muscle on EE's right arm.
Trip injury with first aid resulting in 1 week light duty
Employee slipped while trying to clear a blocked chute above a conveyor. His feet became trapped between the end of the chute and the conveyor. He was taken to a local hospital for check-up and treatment and returned to work on 10/21/04. He was wearing safety shoes and hat. The remaining employees have been reminded of the procedure to follow when undertaking similar duties.
WHILE TRYING TO UNPLUG A TUNNEL SCREW AT #2 STATION, EE'S HAND WAS IN SCREW. IT TURNED BACK WARDS FROM HAVING PRESSURE ON THE SCREW, PINCHEDHIS FINGER AND TOOK THE TIP OF HIS L. MIDDLE FINGER OFF.
EE WAS WORKING ON THE YARD RAMP IN THE GARAGE. HE WAS BURNING ON THE FRAME WORK OF THE WHEEL SECTION WHEN A HOT PIECE OF SLAG WENT INTO HIS BOOT CAUSING A BURN TO HIS ANKLE AND TOP OF THE FOOT (LEFT FOOT).
WHILE CLEANING DUST COLLECTOR BAGS, A DUST PARTICLE FLEW INTO HIS RIGHT EYE.
WHILE LOADING BAGS OF STONE ON THE BACK OF THE FLATBED TRAILER, EE FELT SOMETHING ON HIS RIGHT SIDE, NEAR THE RIB CAGE, STRAIN OR PULL.
EE WAS WORKING AT #1A STATION FILLING BAGS OF PLAY SAND. WHEN HE PLACED THE BAG ON THE JAWS, HE CAUGHT HIS MIDDLE FINGER IN THE JAW. THIS CAUSED A CUT TO LEFT MIDDLE FINGER. HE WENT TO NEWTON MEMORIAL HOSPITAL AND RECEIVED FOUR STITCHES.
EE WAS USING AN OVERHEAD HOIST TO LIFT THE REAR OF A FORKLIFT. HE WAS POSITIONING THE CHAIN AND HIS THUMB BECAME PINNED BETWEEN THE REAR OF THE FORKLIFT AND THE CHAIN WHEN THE LOAD WAS APPLIED TO THE HOIST.
WHILE WRAPPING A PALLET EZ40, THE WRAPPER NEEDEDTO BE RESET. HIS GLOVE TOUCHED THE PLASTIC SPOOL AND PULLED HIS LEFT HAND INTO THE PLASTIC TENSIONER.
EMPLOYEE WAS PULLING OUT SLIDE ON CHUTE WITH A PIPE, PIPE SLIPPED AND HE SLIPPED AND FELL AGAINST CHUTE.
EMPLOYEE WAS WORKING AT #2 STATION GETTING BAGS READY TO BAG LIME. HE WAS THROWING THE BAGS FROM THE GROUND FLOOR TO THE PLATFORM. WHILE DOING THIS, HE FELT SOMETHING IN HIS BACK STRAIN OR PUL L.
WHILE GETTING OUT OF THE YARD TRUCK OUT BY THE WAREHOUSE, EE SLIPPED AND FELL ON THE ICE AND SNOW. HE BRUISED HIS RIBS AND BACK.
EE WAS GETTING OUT OF YARD TRUCK WHEN HE FELT SOMETHING PULL IN HIS BACK.
WINDY CONDITIONS CAUSED METAL PARTICLES TO ENTERBEHIND SAFETY GLASSES.
EE WAS WEARING PANTS INSIDE BOOTS THEREFORE ALLOWING HOT METAL TO GET INSIDE BOOT.
EE WAS TRYING TO ADJUST THE AIR OPERATERED FEED GATE IN THE PELLETED TUNNEL WHEN HE HIT THE TOP OF THE VALVE CAUSING IT TO CLOSE & CATCH HIS FINGER IN IT. THIS TOOK THE TIP OF HIS FINGER OFF. DID NOT HAVE AIR TURNED OFF TO THE AIR OPERATED GATE
EMPLOYEE WAS CHECKING PELLETED BULK TANK TO SEE IF IT WAS FULL, HE SLIPPED ON A PATCH OF ICE FALLING INTO THE GUARD FOR THE CONVEYER BELT CAUSING A SMALL CONTUSION.
