While performing mechanical maintenance inside of a controlled environment garage, the employee used a sledgehammer to dislodge a brace when the sledgehammer broke the weld the iron angel spun and struck the base of the employees right thumb.

Employee slipped while trying to clear a blocked chute above a conveyor. His feet became trapped between the end of the chute and the conveyor. He was taken to a local hospital for check-up and treatment and returned to work on 10/21/04. He was wearing safety shoes and hat. The remaining employees have been reminded of the procedure to follow when undertaking similar duties.

WHILE TRYING TO UNPLUG A TUNNEL SCREW AT #2 STATION, EE'S HAND WAS IN SCREW. IT TURNED BACK WARDS FROM HAVING PRESSURE ON THE SCREW, PINCHEDHIS FINGER AND TOOK THE TIP OF HIS L. MIDDLE FINGER OFF.
