Struck against stationary object

On 2/6/25, employee was working in the top deck of the wash screen, when employee stood up and hit head on the spray bar. Did not seek medical treatment until 2/11/25.

The employee was utilizing a torque multiplier tool to remove a tie rod form a hydraulic rock breaker and while releasing tension on the multiplier the ratchet that EE was using broke a tooth, skipped and quickly spun around and caught the employee's hand and caused a laceration.

Employee tripped and fell while walking on the cat walk.

The employee was attempting to open the ladder for the 57's catwalk when EE felt a pop and severe pain in EE's left arm.

Employee was performing maintenance on the screen deck of a wash plant. Employee slipped and fell backwards and struck against a chute work on the right side of rib area.

Struck by falling object

Ee was working on belt. the need was to remove a rock slab from the belt. ee was accompanied by another employee. in process of removing the slab, the slab slipped and landed on ee's left wrist. ee immediatley notified about the incident to plant manager. ee was taken to urgent care location where X-Ray were performed. the X-ray reports came back negative. ee on light duty

Employee parked haul truck to take a break. Employee stated slipped and fell while walking across the haul truck parking area. With all the rain we were getting the area may have been slippery.

On Tuesday 2/14 around 7:10 am EE was rolling up a 24 inch wide conveyor belt when EE slipped and fell, injuring lower back. This incident was not reported until Wednesday 2/15 by text messaging Supervisor at 2:27 am. Supervisor called and informed me about the incident.

While installing an engine on a Caterpillar 992G machine, our Field Service Technician was under the machine installing the drive shaft and hydraulic lines. While attempting to move from under the machine, our Technician hit hard hat on the step located on the side of the machine knocking off hard hat resulting in a scalp laceration requiring stitches.

Employee bending over to fix Bucket Pin on 992G Cat - his hard hat fell off and he hit his head on a steel lock plate - laceration required stitches

EE was working on the 3/4 cross belt - while prying an idler from the conveyor, EE caught his left ring finger between the conveyor and bar. Finger is fractured.

EE slipped on patch of ice causing him to lose his balance and fall backwards on his right ankle. Fibular Fracture - surgery needed.

Employee was changing screen panels on the Telsmith #2 middle deck. When he rolled over to reach for a panel he felt pain in his chest area. He was taken to the hospital and diagnosed with a pulled chest muscle.

Employee was operating the man lift in order to remove snow from the conveyor belts. As he was elevating the man lift up under a conveyor, the machine cover on the man lift came too close to an ""I-Beam"" and his hand got caught between the control panel and the underside of the I-Beam. He lacerated his hand requiring 5 stitches and has hairline fractures on 2 fingers. Right Hand.

Employee was injured when he was replacing the side guard on a loader bucket. The mechanic had one of the bolts in place and was using a 4 lb hammer to hit the side guard in order to align the other hole. The hammer struck the side guard and a steel chip flew off and struck his right inner thigh. The piece of steel went through his clothing and had to be surgically removed.

Two steel plates approx. 2'x 4' were set against each other upright near the base of the crusher, when EE was putting a clevis on one plate, they fell over his leg. EE's leg was broken from the weight of the plates.
