While entering the scale house the employee tripped stepping over the threshold falling on EE's left side.

While operating the JLG man-lift the employee used the safety gate to pull self upright. The gate swung open and hit the employee in the face causing a broken nose and laceration needing stitches.

While replacing a lift cylinder on a haul truck the associate's right middle finger was pinched against the frame causing a small fracture in the tip of the finger.

Employee was attempting to push in a skirt rubber that had popped out. EE used a pry bar to push the rubber back in place and inadvertently made contact with the moving conveyor belt causing the bar to spin and pinch their finger between the bar and the chute. A few stitches received to tip of right middle finger.

employee was attempting to repair a diesel/electric heater. while trying to remove small air line he slipped and his right hand went into the energized fan blade causing a laceration/broken bone and damage to tendon on pointer finger, and lacerations on middle and ring finger.

Employee did not report the injury at time of incident, then went for treatment on 11/20/13 and then reported it to plant manager on 11/25/13 claiming he had the injury two weeks prior. Utilizing 11/20/13 as date of injury based on first medical visit.

Fire started inside the heavy equipment shop. The fire began with the 2006 Kawasaki 115 Z-V Frontend Wheel Loader. It is believed that the cause of the fire was an electrical short on the loader. An investigation will be performed by Kawasaki Rep. Loader heavily damaged, shop minor damages.

Miner was removing a troughing idler from a conveyor and as the idler was being pulled out from the belt, the idler frame got caught on the miner's lanyard causing him to lose balance. The employee fell approximately 5 feet landing on conveyor and chute work below. 6 stitches to chin some light bruising.
While cleaning the windows on loader, the employee was outside the handrailing, attempting to step on the front fender. The employee misjudged the distance and slipped on the sloped part of the fender, falling to the ground.

Employee stepped on pillar to check belt, he slipped off pillar and fell to ground. He sustained knee and back strain injuries. Knee was not healing, referred for surgery on 5/30/06.

EE WAS WELDING ON A PIECE OF HEAVY EQUIPMENT ON A HOT, HUMID DAY. OUTSIDE TEMPERATURE EXCEEDED 95 DEG. EE HAD SCARRING ON BOTH LEGS FROM AN ACCIDENT BACK IN EARLY 1980'S. THE BURN/SCARRING BEC AME AGGRAVATED FROM THE HIGH HEAT AND HUMIDITY. EE'S LEGS BEGAN BLEEDING AND SUSTAINED BLISTERS.
EE WAS UNJAMMING A MATERIAL CHUTE. EE PULLED OUT A ROCK. WHEN THE FIRST ROCK BECAME DISLODGED, A 2ND ROCK ALSO WAS FREED UP WHICH ROLLED DOWN STRIKING EE'S INDEX FINGER ON RIGHT HAND. EE RECEI VED 10 STITCHES.

EE WAS WORKING IN FEEDER ON JAW PLANT FITTING STEEL PLATES. HE WAS USING A PRY BAR. PRY BAR SLIPPED AND PINCHED HIS LEFT HAND AGAINST WALL OF FEEDER.

EE WAS BREAKING LOOSE THE ELECTRIC BOLTS ON CRUSHER #2 WHEN THE RATCHET THAT HE WAS USING SKIPPED CAUSING EE TO SLIP FORWARD STRIKING HIS RIGHT ARM AND NOSE ON THE SOCKET SEALING RING.
EMPLOYEE WAS CHECKING BELT TENSION ON SCREEN. SCREEN WAS JUST SHUT OFF BUT WAS STILL ROLLING ON HIGH SIDE OF ECENTRIC. EMPLOYEE REACHED AROUND GUARD TO CHECK BELT AS THE ECENTRIC ROLLED CATCH ING HIS HAND BETWEEN BELTS AND SHEAVES.
