Employee was walking on the screen deck when EE hit EE's right arm on the come along.

Employee tripped on a stone and landed on back and got cuts to arm. Employee went to the ER and got looked at and got 10 stitches on the arm. Employee was released.

Truck was being loaded by the loader and the broom hit the truck, jarring the employee.

Employee was lifting a hydraulic cylinder with another employee and it slipped, crushing the employee's right middle finger.

Employee was using utility knife to cut fitting off of hose when cut palm of hand.

Employee was on the screen tower using a hose to clear a plug up of hard pack on conveyor when his foot slipped on wet surface causing him to lose his balance, but not fall. His left knee was twisted as a result. He was treated for a ligament injury and was given a brace, crutches and work restrictions.

A frozen chunk had plugged up a conveyor on the crusher. The employee used a pinch bar in an attempt to reposition the chunk. The hook at the end of the pinch bar slipped off of the frozen chunk which caused the straight end of the bar to hit the employee in the mouth, chipping two teeth.

Employee was dismounting his loader when he twisted his knee. He was using three points of contact however the step was slippery. The employee was evaluated, treated for a strain to his right lower leg and released to modified duty.

Employee states he was looking for a piece of steel on the steel rack. The area was covered in snow, he plowed and shoveled an area around the rack. When leaving the area, the employee states he caught his right foot on a pallet and fell on his right knee. He fell onto a piece of steel causing a laceration to the right knee.

Employee was changing screen on deck and was positioning airhose when he stepped on it turning his ankle

Employee was using a propane torch and sustained 2nd degree burn when he turned it on.

Employee states he was putting belts on a loam screen motor and felt a pull in his shoulder.

Employee states that he was shoveling and when he woke up the next morning, his lower back was sore.

Employee states that while shoveling, he felt pull in his stomach region.

EMPLOYEE WAS PRYING SCREEN WITH CROWS FOOT WHEN FOOT SLIPPED AND HIT HIM IN THE HEAD.

EMPLOYEE STATED HE DEVELOPED PAIN IN FOREARMS OVER A PERIOD OF TIME DUE TO SPECIFIC TASKS PERFORMED FOR HIS ADMINISTRATIVE FUNCTIONS.

EMPLOYEE WAS GETTING INTO ROLL AWAY BOX TO REMOVE STONE AND HIT HIS HIP AGAINST A PIECE OF METAL.

EMPLOYEE WAS LIFTING AN ELECTRICAL CONTROL CABINET TO LOAD IN PICKUP WHEN HE STRAINED HIS WRIST.

WALKING TO LOADER, HE STEPPED ON A ROCK AND TWISTED HIS RIGHT ANKLE.

WHILE EXITING CRUSHER CHUTE, HE SLID 4' DOWN CONVEYOR ONTO LEFT ANKLE.

WHILE REACHING IN DESK, A THUMB TACK STRUCK HIS FINGER CAUSING HIM TO PULL BACK, PUNCTURING HIS LEFT THUMB WITH A KNIFE THAT WAS IN HIS RIGHT HAND.

EE SLIPPED ON CATWALK WHILE PAINTING CONVEYOR GRABBED GUARD PUNCTERING HIS LEFT THUMB
