Ee reports that co-worker who was providing training grabbed ee's hard hat from behind and twisted ee's neck from left to right forcefully. Ee reports increased pain and stiffness in neck. Seen at a health clinic and released with restrictions site can accommodate.

Ee at lime silo; cleaned mirror off and then rubbed eye with hand. Noticed a burning sensation, seen at ER at NNRH and UofU. Diagnosed with conjunctivitis of left eye, released to full duty. Reportable for prescription meds for eye injury.
EE backing up haul truck, shovel bucket struck bed, stopping truck suddenly and jostling the EE in the cab. The EE was seen at health clinic, diagnosed with sprain, and released to full duty. On follow-up on 7/29/21, EE prescribed physical therapy and released with restrictions, making this reportable.
EE was using a pressure washer. Enough pressure built up in the wand, causing it to ""jump back"" and strike the EE in the face. EE was seen at the ER where a laceration near the right eye was closed with sutures. EE released to full duty.

EE was changing filters on a dozer. EE set the trans housing in a bucket to drain and the housing slipped. EE twisted and reached to the side to catch the bucket and when they stood up, they felt pain in lower back. EE sought treatment on 1/5/21 and was released with restrictions.
Ee was struck on the knee by a metal hoist pen. EE was seen at Golden Health on 12/14, diagnosed with a strain and told to follow up on 12/17 prior to EE's next scheduled shift. On follow-up EE was released to restricted duty until 12/25, and then released to full duty.
Ee was stepping off pit bus to go to the piece of equipment to start work. EE slipped on ice/mud, twisted ankle and fell, landing on foot. Seen at Golden Health and referred to orthopedic surgeon in Reno.

Ee was walking to haul truck, slipped and fell, landing on EE's knee. Seen at the ER, diagnosed with pain in knee. Seen again at the clinic and put off work pending MRI.
Ee was operating loader 50 in cut 06-6040. The floor in the area was reported to be rough, and the ee was jarred while operating the loader. Ee reported to the supervisor and was seen at the ER. Diagnosis on follow-up was grade 1 acromioclavicular joint separation. Ee was released to full duty.
Ee was descending haul truck and EE's back pack caught, causing EE to lose EE's balance, twisting to the left. EE caught self with EE's right arm. EE was seen at the ER and put off work pending an evaluation with an orhto. Seen by the ortho on 08/09/19 and released to restricted duty, MRI pending.
A maintenance technician was struck by metal shrapnel while working on a hydraulic shovel. The employee received a laceration on the lower back, requiring stitches.
A maintenance welder was lying down on side to weld a hose bracket on a 994 loader. The employee felt pain in low back when employee pulled self up after completing the job around 9:00am (March 24th). The employee reported the incident the following shift (March 25th). The employee was placed off work on March 29th after a clinic appointment.
Ee was driving the haul truck and hit a deep pothole. EE struck head on the window of the haul truck. EE was transported to the ER with neck pain. Diagnosed with a cervical strain and put on restricted duty for one day.

Employee was cutting a piece of wood with a cordless band saw. Employee reached down to adjust the piece of wood with left hand and hit the trigger of the band saw. The blade grabbed employee's glove and cut through to finger down to the bone. Employee received 8 sutures to close the wound.
Employee was jostled while seated (seatbelt on) in the cab of a haul truck while being loaded. On 3/13/2018 employee went to the Doctor where they were prescribed physical therapy making the event reportable.

Employee was using a hand held grinder when the wheel caught and failed. A small piece of the wheel went through ee's leather glove and impacted ee's finger resulting in 6 sutures.
Employee stepped to gain access to a drill ladder and felt a pop in the right knee. Employee sought medical treatment and was given restrictions.
While tailing-in a 10' PQ quill rod, the bottom of the quill rod kicked back causing the Helper to fall backwards onto a rod sloop resulting in a cut to the injured employee's right hip.
Employee stepped down after replacing a hose and twisted employee's knee (employee felt a pop). The employee sought medical treatment 02/16/2017 and was given restricted duty.

While getting out of a service truck the employee felt pain in the right knee when the right foot made contact with the (flat) ground. The employee went to the doctor and was put on restricted duty.
Employee was in the process of dismounting a light vehicle tire off of a wheel, using tire bars when employee felt a pop in right wrist. Employee went to the doctor and was placed on restricted duty.
Employee was lifting an argon bottle and felt pain in their back. EE was taken to the doctor and given a restricted duty work release.

Employee fell onto his left wrist getting out of a pickup truck. Employee did not seek medical treatment until 01/22/2016 where he was put in a wrist brace and given a full duty work release.
Employee was transferring equipment (pin flags) from the WCRM Conex container to a WCRM company vehicle. While walking on ice the employee slipped and fell backwards.
Metal sliver embedded in upper lip removed surgically requiring two sutures and a band-aid.
Employee had started a power trowel to begin finishing the concrete floor. The handle of the power trowel slipped out of his hand and spun around. As the trowel spun around, something on the machine caught his right forearm, causing a small laceration requiring 12 sutures.
Employee saw two rakes left on the ground by the construction company with their forks facing upward and considered them a safety hazard. He picked up the rakes with the rake heads up. One of the rakes was broken and the head of the rake slid down the handle onto the employee's hand. Employee's right hand was cut between the thumb and forefinger requiring 7 stitches.
EE was removing a shipping strap from the mateline of a multi section building. EE cut the strap from inside of the building using metal shears. When he cut the strap with his left hand, it fell down and hit his right hand. He had gloves on but were not fully leather or Kevlar, therefore, failed to prevent the strap from cutting his hand.
