Struck against a moving object

EE reports loading truck w/mud, making 2nd pass, hit return to dig button, lowered bucket halfway. Bucket stopped instantly, jarring them in the cab. They reported the incident 4 days later on 1/19, and sought treatment. Diagnosed with lumbar sprain, released with restrictions site cannot provide modified duty under.

Ee was staking voids. EE was walking on uneven ground and EE's foot caught between two rocks, EE lost EE's balance, fell, and felt a pop in EE's left foot. Seen at clinic, diagnosed with a fracture to left ankle, referred to orthopedics, released to restricted duty.

Struck against a moving object

EE operating haul truck, backing under shovel 192. Shovel bucket made contact with the truck bed, jostling the EE in cab of haul truck. EE dumped truck load at 6098 dump, then went to Q2 and called supervisor to report. EE seen at clinic, diagnosed with C-spine strain and thoracic sprain, and released to modified duty. Site unable to provide modified duty.

In the Arturo pit at approximately 01:30 am, shovel 191 was working on the 5200-bench trim Pattern #19, cleaning up the blasted material. The shovel bucket made contact with explosives, causing it to detonate. This caused damage to the window. All employees were safe, and no injuries were sustained.

Ee reports sitting in cab of haul truck when shovel loaded a large boulder on first and last bucket, jostling EE in the cab. EE reports feeling head and neck pain and then developing a migraine. Seen at clinic on 8/28, diagnosed with a cervical strain, released to restrictions. Site unable to offer modified duty.

EE was sitting in cab being loaded. On last pass there was a large boulder that was too big to go through the opening of the shovel so the operator ""shook it"" off, the boulder hit the overshot on the bed of the truck, shaking and bouncing the truck. EE reported, asked to be seen by doctor on 5/20. Diagnosed with cervical pain and thoracic sprain, released with restrictions.

EE reports tripping while they were going into the locker room to get equipment for the day. EE reports pain in left elbow and shoulder. Seen at clinic and referred for orthopedic consult. At orthopedic consult on 4/3/25, diagnosis of acute proximal radius fracture confirmed and employee put off work pending MRI.

EE reports that while driving haul truck, they hit a bump/pothole. EE reports feeling a pop in their neck, and then pain down left arm and into their back. Seen at clinic, diagnosed with a muscle strain, and released to full duty. On follow-up on 3/31, EE was put on restrictions site cannot offer modified duty under, making this reportable for lost time.

Ee pulled the top door mechanism of the hot change bus to close the door and pinched finger against the rod. Seen at ER, diagnosed with a fracture to the tip of the pinky finger, and released to full duty.

Over-exertion in pulling or pushing objects

EE trying to close the passenger side door of haul truck reports using ""a lot of force"" to ""slam it as hard as they could"". When they did this, they felt a sharp pain under right thumb, shooting down towards the wrist. Seen at clinic, given a brace and released to modified duty, no operating equipment or driving any light vehicle on mine site.

Ee had arrived at work and completed a tour of the pit in a light vehicle. Was then walking up 5 steps to enter a building, sat down, said wasn't feeling well. EMTs were called, during assessment, ee complained of severe chest pains and lost consciousness. CPR administered. Life saving efforts were unsuccessful.

EE was removing hose from a machine with cutting wheel. EE was wearing a face shield, gloves, and safety glasses. A piece of debris got in EE's right eye. EE was seen at health clinic, referred to eyecare clinic, diagnosed with a conjunctival abrasion, prescribed antibiotics, and released to full duty.

Ee in hot change bus that hit a pothole, jostling ee in the bus. Seen at ER, scans negative for fracture, diagnosed with head injury, released to full duty. On follow-up 10/07 reports soreness/aching in neck, referred for physical therapy, making this reportable for treatment.

Over-exertion in lifting objects

EE was switching skid steer trailers. EE reports shifting all EE's weight to EE's let leg and hearing a pop in EE's left knee. On 7/12/24 NGM received confirmation that this is a work related injury. The EE was put off work pending surgery.

EE was passenger in the shift change bus on the way to the time shack. The bus hit a pothole and it jostled the EE in the seat. Seen at clinic, diagnosed with a contusion to the left knee and on follow-up complained of wrist pain as well. Released to restricted duty.

EE reports while ""spinning off"" filters while laying on their left side and reaching with their left arm, they felt a pinch in their shoulder. They reported the next morning that they had pain. Seen at the ER and diagnosed with a rotator cuff injury. Put off work pending MRI and ortho consult.

Ee performing walk-around of haul truck, tripped on small rock, fell, landing on knee and hand. Reported and continued working. On 11/13 sought treatment, diagnosed with pain, referred for MRI. Released with restrictions site can accommodate.

Fall from machine

Ee on nights was walking across deck of track hoe. Hatch on the deck was left open from previous maintenance and was shadowed. Ee stepped through the opening, fell, injured left elbow, left leg. Leg continues to have pain in the thigh area; ee was referred to physical therapy on 08/17/18 making this reportable for treatment. Ee continues on full duty.

Over-exertion in lifting objects

2 EEs were carrying a heater, load shifted and strained 1 EEs mid-back. WorkCare called, advised self-care. Incident was reported 1 shift after occurrence. 5.8.2015 Dr released EE w/care not to aggravate back due to findings of Xray & MRI as soft tissue injury. 5.11.2015 F/U appt EE was again release care not to aggravate back. F/U on 5.18.2015 EE was given restriction. F/U 6.1.15.
