2 recordedA fatal accident occurred at the Exodus underground mine on Friday August 31st at approximately 13:30. The employee was preparing the top cut of the 4410-315 long hole stope, when he fell into the stope.

EE stepped on wire panel while surveying and cut right index finger. Laceration to tip and pad of right index finger sutured, EE released with restrictions and site is offering modified duty.
Ee was checking a piece of equipment, going to take cap off radiator. The cap popped and coolant splashed on ee's right wrist/forearm. Seen at ER, diagnosed with burns, released to full duty. On follow-up at clinic, released to restrictions and referred for therapy. Site can offer modified duty.

Ee was putting chock blocks back on the truck when one fell between the hand rails and the drum. EE went to get the chock and the hatch hooked the chock, pinching EE's right thumb between the hand rail and the chock. Seen at ER, diagnosed with traumatic amputation of the tip of the right thumb.
Driver was backed in to unload and had what is believed as a heart attack and died.

EE was unloading cement at the rear of EE's trailer, the loader operator on site was filling the pug mill screen and sent rocks off the back side where EE was standing. EE ran to avoid being struck by rock tripping over a discarded wheel chock rolling EE's foot and fracturing EE's big toe.
Ee sustained a Standard Threshold Shift in hearing. Confirmed reportable noise induced hearing loss. Confirmation received from Au.D. CPS/A, on 01/19/2023. Retraining and refitting complete.

Ee was positioning a tire when the wheel lifted up, pinching EE's right middle finger between the tire stand and the wheel. Seen at ER, x-rays negative for fracture, sutures required to close the laceration. Ee released to full duty.
Ee was grinding a weld out when the grinding wheel EE was using blew apart, sending fragments that struck EE's thigh and face. Seen at Golden Health, sutures required to close wound on thigh. Ee released to return to work full duty 04/04/22, EE's next scheduled shift. No lost time.
EE developed chronic cough. Sought treatment for cough resulting in diagnosis of pulmonary fibrosis/silicosis. Lung biopsy taken 3/2/22 confirmed. On 4/13/22 confirmed compensable and accepted by workers' compensation.

Ee was changing a tire on an UG haul truck. The section repair that was installed blew out, causing ee concussion injury to head, ears, torso. Ee was seen at ER, CT negative for acute injury, ee released to full duty. Ee complained of ongoing symptoms that were determined occupational on 10/4/21, and put off work, prompting this report.
Ee was removing a belly pan on haul truck 203. The pan fell onto the ee's right hand causing a laceration at the base of the index finger. Stitches were required to close the wound and the employee was released with restrictions the site can accommodate.
EE had left arm out of door opening between door and door frame of lube truck, holding a wooden block, attempting to use the block and the articulation of the machine to force the door back into position. The lube truck articulated unexpectedly while arm was outside the cab, pinning the arm between the door and frame. Open fracture needed surgery.

Ee was working from a scissor deck. EE was performing some housekeeping, stepped back, and stepped on a piece of chain, causing EE to twist both knees and fall on the deck. Surgery required to repair the fracture to the tibial plateau. Ee underwent surgery on 9/4/20. Lost time.

Ee was seated behind front passenger seat in mantrip. Ee was closing the compartment door to the right from a seated position, and placed right hand in the door frame near the hinges. Ee kicked the door shut, pinching right middle finger in the door. Amputation of the tip of the right middle finger.
EE was using an electric hand drill to drill a hole. The drill became stuck in the hole causing the drill to rotate in the EE hand. This caused a dislocation of EE right hand middle finger. The EE sought medical treatment (Physical Therapy) on 8/13/2019 making this a reportable injury

Employee poured wax on the floor in the admin, turned around to grab the mop and slipped in the wax falling on their right side.
Two employees were lifting an estimated 100lb mini-power zone from the back of a flatbed truck. One of the employees hand slipped from its holding position, causing the edge of the mini-power zone to strike EE's right thigh. This resulted in a laceration to the employees right thigh which required 16 sutures.
Stored Energy from the metal of an Ore pass Can. AR 450 3/8 inch thick. Repairing damaged can and replacing bad sections. When the Can was cut from top to bottom all the way through, stored energy from the damaged plate came back and struck employee in left hand. Crew also was using man basket to reach location and propped up against to stop bounce back from damage plate.

Employee was bolting in the 4340-136 when the left spring line started to ravel causing material to fall above bolt anchorage. The estimated fall was 20' wide x 12-15 deep x 10' long. There was no injury to the event.
While at work pouring concrete for shaft sink, EE was holding the end of a 6"" x 12 foot hose with both hands, as concrete came to the end of the hose, the hose jerked and lifted EE causing feet to come off the ground and when ee came down it caused EE to strike right forearm on a metal hopper edge.

An employee was working in a manbasket of a powder truck loading a development round in the 4470 North Hanging Wall. The employees right forearm was resting on the rail when material fell from the face. The material fell onto the forearm causing a laceration and later identified a slight fracture.

Employee was lowering an overhead door with his right index finger and inadvertently placed his right thumb on the door track guide. The overhead door made contact with his right thumb resulting in five stitches.
Employee was in Muck Bay 42 attempting to cut a piece of chain hanging down from hard line using bolt cutters. While attempting to cut the chain, the employee lost the grip slipping off one handle. This action caused a chipped in a bottom tooth.

Employee was lifting a grease pump off the top of an empty barrel. The pump slipped out of his hands and when he caught it, the employee felt a pop in his right elbow. The employee continued to work normal duties until six working days later and re-aggravated the injury. The employee sought medical treatment on 1/6/15 and was classified as restricted duty.

Employee was changing out a fork lift attachment and while driving out a keeper pin he hit his thumb with a rap wrench causing a cut to his left thumb.

Employee was diagnosed with a reportable hearing loss shift.
Employee was doing some maintenance work on a piece of equipment. He was in the process of loosening a nut with his ratchet/socket when the wrench slipped. He struck his right thumb on the frame work of the equipment resulting in a laceration. He was taken to medical attention and received six (6) sutures and a full duty release.
Employee was using a hand held grinder when it slipped and struck the back of his left hand just behind the thumb causing a laceration. The laceration required 5 sutures to close.

Employee was cleaning up an underground laydown when he pulled a band out from under a bundle of rock bolts. The end of the band struck his right forearm resulting in a laceration which required 7 (seven) sutures.
Employee was lifting a portable tool box when he felt a pain in his lower abdomen.
Employee was disconnecting a water hose that was hung on the rib of the drift by a cab hook. The cab hook spun in the palm of his hand creating a laceration which resulted in 9 sutures to close.

Employee was putting Core sample tube on tube rack, caught end of left ring finger between tube and tube rack.
Employee was descending down a surface loader ladder and missed the bottom step; falling to ground. The fall resulted in a minor separated shoulder which required restricted duty for three shifts and immobilization device for two days.
While walking around truck EE slipped and cut hand on rib wire. 8 sutures.
Employee was placing a 60lb Pemco electrical box into the bed of a pickup when it slipped pinching his finger between the box and the truck bed. This resulted in a fractured left middle finger and 8 sutures.
Employee was standing at the face of a drift when a rock fell from the edge of the wire about the size of a softball striking him on top of the hard hat. It resulted in some muscle/ nerve inflammation.
Employee was using a recipricating saw when the blade bound up and his left index finger was pinched between the saw and the object he was cutting. The laceration required 6 sutures.

While getting on the crew bus employee tripped and fell. His right hand contacted the bottom step of the bus. This contact resulted in a fractured metacarpal bone behind his pinky finger. He did not seek medical attention until 11/16/2010.
Employee was collecting ore control sample and stepped on a rock and lost footing and twisted his back and he suffered a lumbar strain.
Driller was adding drill rod. Top of rod contacted sheav wheel and dropped back on hand. Finger pinched against feed frame of drill.
