EE was dropping bolts off at headings underground. A bolt slipped out of their hands back onto the truck, and the bottom of the bolt hit them in the face, cutting their cheek. Seen at ER, wound closed with sutures, and the EE was released to full duty.
EE was connecting a nip, lost footing, twisted and dislocated their patella. Seen at the ER and released to light duty. EE referred for surgical consult and on 3/11, NGM received confirmation from the surgeon that a tear in the knee was caused by the work-related incident.

EE stripping motor leads with stripping tool. Cable jacket difficult to remove, so they took off a glove to use their thumbnail to peel jacket off. Gripped loose jacket with left hand, while holding stripping tool with right hand. Pulled jacket, when it broke free, left hand struck stripping tool, causing laceration. Seen at ER, laceration sutured, released to restricted duty.
Ee reported back pain from riding on metal buddy seat in haul truck. Seen at ER, diagnosed with non-occupational condition after MRI. Site did not report as diagnosis was non-occupational. Workers' comp denied claim, ee did not appeal, and filed claim for short-term disability.

After employee was finished mucking out a short 6 foot round in the LA 4010 230, the employee was cleaning up the ribs and the back. The Mucker operator started to place bucket on the ground when a fall of ground occurred causing the mucker to be partially covered in material.
An LHD tire came off the Forks of a Telehandler during transport and rolled down the Range Front decline ramp. The tire contacted a CUSA Minca Paus 18.1 personnel carrier near crosscut 6, with two employees in the Cabin of the Paus. The Paus passenger sustained a laceration on the top of EE's head requiring staples to close.

EE using 1.5 ton crane moving motor. The motor was on the manual crane approximately 4"" from the cat walk, and the hoist system failed, and the motor fell, striking the EE on the head/shoulder/back. Seen at ER, diagnosed with compression fracture to spine, laceration/soft tissue injury to right leg. EE put off work, non-weight bearing.
Ip was trouble shooting a leak on the drill when pulling hand out hand made contact with the burr on the pipe causing a laceration to the right hand
EE was helping utility crew hang vent bag. EE grabbed a cable to hang the vent bag, turned around, and ran into a piece of protruding wire. Seen at clinic and referred to eye doctor. Diagnosed with a laceration to the eye and released to full duty effective 6/16.
While performing an inspection of the mine, the supervisor came across a fall of ground.
While performing an inspection in an old heading, it was found that a fall of ground had occurred.
Driller was in the process of loading a 5 foot piece of 3.25 inch Matrix rod from the rod stack to the jaw of the rod handler, when right middle finger was pinched between the rod and handler jaw. Driller immediately pulled hand from between the rod and handler jaw and noted the injury to the right middle finger tip.
EE reported loader striking side of truck repeatedly on three separate loads. They reported pain which got progressively worse. Workers' compensation investigated claim and accepted as occupational on 3/23/23 making this reportable for lost time.

TRM004 was dropping a fuel pod in the 3980-fuel bay when the operator smelled smoke. The operator shut the Multimec off to investigate and noticed flame coming from the engine compartment. The operator then activated the fire suppression system and exited the cab.
Ee was working on a drill. EE was getting ready to nip in, and hit the spool button rather than the unspool button, catching EE's glove in the spool, pulling EE's hand in. Seen at the ER, diagnosed with a laceration to the left thumb. No fracture, sutures required. Ee released with restrictions site can accommodate.

Ee was scaling off a hanger plate. A loose piece of shotcrete fell and hit the ee in the left eye and on the nose. Ee was seen at the ER, diagnosed with a corneal abrasion, blunt eye trauma, and iridoplegia. Ee was referred to an eye doctor and released to restricted duty on ee's next scheduled shift.
The onboard Air Compressor failed on Blt 7, causing heavy smoke and the fire suppression to be discharged.

Fall of ground occured in the left rib of the LC3890 intersection approximately 12 ft deep and 55 feet long
Blast induced damage crawled back approx. 5 rounds causing a break in the face approx. 20 to 25 feet high.
Employee dropped a battery on the left foot while unloading material from a pickup truck. The incident was originally a first aid, but on 5/26/20, it was diagnosed as a fracture.
While lifting a monorail beam from forks of a forklift, the beam slipped and pinned the employee's left hand between the beam and the apron feeder of the forklift.
Employee was changing the grinding wheel on a grinder and when they applied pressure to turn the disk, the grinder turned on and cut the small, right finger.
Employee was positioning a beam when the left small finger got caught between the beam and a column.

Operating a haul truck, employee struck left leg against dash of unit when employee drove over a pothole. Employee was working in tight quarters. Employee reported on 12/12/2019 as for medical on 01/07/2020 as area was irritated. No restrictions.
Employee was working from a scissor lift cutting an existing 1/2"" all thread hanging from the electrical room back. The porta-bandsaw broke away from the material and fell onto the Team Members left hand cutting through the glove and injuring hand. Laceration to Left Pointer Finger
A fall of ground was discovered in the LC3830 SD334 on 7.5.19 around 01:00 hours. This most likely occurred at shift change, durning blast, on 7.4.19 at around 19:00 hours, but is unknown. The heading was previously shot at the end of night shift 7.3.19, & was mucked on day shift 7.4.19. From the time the heading was shot until the fall of ground was likely only 12 hours.
The employee was pulling on a wrench and the wrench slipped hitting the employee in the bottom lip and front bottom tooth, causing a small laceration to the lip and a small chip to the tooth.
The Operator looked out the side door of the Forklift EE was operating and as doing such, EE struck upper front tooth on the hand held bar that assists the operators while entering the cab, and broke part of the tooth off.

The employee stepped off of a concrete step and twisted their right knee.
Employee was positioning a HILTI Hammer-Drill inside the re-bar to be able to drill the next hole for re-bar when EE's left hand made contact with the trigger engaging the drill while right hand was on the bit. EE's glove became entangled in the bit causing dislocated fingers and lacerations on the right hand.
60 foot deep ore bins are being excavated from 10 foot diameter to 21 foot diameter. Use of a hydraulic winch and workdeck are being used. Night shift crew 07/27/2018 blasted a slash round from 28 to 36 feet. Workdeck was hung below the sheave to protect the sheave and wire rope. Concussion from the blast rocked the workdeck causing a kink in the wire rope.
EE was driving underground in open cab. Ventilation blew some fine dirt into an eye. Employee rinsed the eye and continued working. The next day the eye was irritated. Employee was seen at the clinic and diagnosed with a corneal abrasion. Employee was given prescription eye drops and released to full duty.

Ee was working with another ee, had swellex in vice and coworker was using wrench to spin it. Injured ee's hand was caught between the vice and the swellex. Seen at ER, diagnosed with fractured fingertip and laceration. Stitches required and ee put off work for 2 days, then restricted.
Miner applying shotcrete to newly excavated portion of the east decline tunnel for initial ground support. Shotcrete nozzle plugged causing the supply hose to jump which caused the miner to lose footing and miner fell striking the ground with the right elbow dislocating it.
Employee energized air hose, the opened end of hose whipped around and the hose fitting struck employee on the head. The hose caused a laceration to the employee's head. Employee was released to full duty.
Contract employee was taking a mop bucket out to the janitor closet outside of the dry. EE slipped backward on the wet floor and fell, and was struck in the face with the bucket. EE was transported by light vehicle to NNRH with a cut over the eye. Subsequent follow-up at University of Utah resulted in stitches and 3 days off.
Ee was rolling up a cable reel and left thumb became caught and was crushed. Seen at ER, diagnosed with open displaced fracture of distal tuft, distal phalanx of thumb. Referral to orthopedic surgeon resulted in surgery. Ee lost time.
Ee was pulling on hose and it broke free, striking ee in the mouth. Seen at the dentist, diagnosed with 3 broken/chipped teeth, one of which had to be extracted. Ee released to restricted duty for two days.
Interwest Supply employee (contractor) was working with a Barrick employee unloading 2x2 steel tubing from forklift onto steel rack hitting contractor on the forearm causing a cut. The contractor was seen at the ER and stitches were required to close the wound. EE was released to full duty effective 03/20/2017.

Operator was hauling a load with an AD30 CAT Truck at the 11 east to exit the mine. The operator looked and turned to their right shoulder and noticed flames, what appeared to the operator to be coming from the rear engine compartment. Operator notified the mine via radio, shut down the equipment and exited the mine.
Employee was operating a Normet Transmixer when the employee pulled into a cut out, looked over EE's right shoulder to observe traffic when EE noticed flames over the right side behind the cab. Employee shut off the Transmixer and exited the area while calling ""MAYDAY"" to inform the mine of the incident.

Ee was changing a fitting on an emulsion hose and his hand slipped, cutting his right pinky finger. He was seen at the clinic where sutures were required to close the cut. The ee was released to restricted duty.

Employee was working underground and was struck on the upper lip by a bolt that fell over. He was taken to the clinic and received stitches to close the wound. He was released to full duty.
Ee was removing an air drop from a line and was struck in the face by the drop. He was seen by a doctor and sutures were required for the laceration to the face. Other diagnostic testing negative, employee released to full duty.
Ee driving down wet ramp, truck slid, ee tried to slow truck by rubbing it against right rib. Truck hit bump, bounced to left rib, and ee struck left hip against cab. He was seen at the hospital, diagnosed with a contusion, and put off work pending follow-up.

Ee was using a knife to strip cable for installation when the knife slipped, cutting the left thumb. Stitches were required to close the laceration. Released to full duty.
Ee was installing a light fixture. He mis-stepped and grabbed the light fixture, cutting his right hand on the sheet metal frame. Stitches required and released to full duty.
Ee was doing a pre-op inspection of a transmixer. He pushed the top hatch to inspect the inside of the drum and the hatch fell, landing on his right hand. Seen by doctor, no fracture, but stitches required to close the wound. Ee released to full duty.
Employee was removing a latch and pinched his right ring finger when the lock bar caught the finger. He was seen at the ER and sutures were required to close the laceration to the finger. Released to restricted duty.
Blasting round was shot and upon inspection wedge failure was identified.
Crew was working on changing teflons on the upper deck, when they went to slide the upper block out the employee thought he had the plate, it slid out at a different angle than anticipated and he did not get his hand fully out of the way. His left index finger was pinched between the block and the feed frame.
After the Helper unlatched the overshot and began lowering it to employee, the overshot slipped out of the helper' s hand and landed on the employee's right foot.
There was snow on the road to disposal site. The air mover lost traction at top of a hill along the road & was unable to continue to disposal site. There was ice on the ground where the air mover was disabled. EE exited vehicle to set wheel chocks. He was wearing required footwear, but no ice cleats. He sustained a fracture to right elbow, but is released to work w/restrictions.

While driller was loosening pressure bolts on the actuator plate, the plate broke free from the bolts and struck the driller's left pinky finger causing a laceration below the fingernail.
When stepping down off steps, rug slipped and the employee hyper-extended her right knee. She was seen at the ER and referred to an orthopedist. Diagnosed with a sprain/strain and put on restricted duty. Unable to work due to medication needs.
Employee changing hose clamps, cutting hose and razor knife slipped. Employee required sutures to left thumb. Released to full duty.
Employee was wrapping up a grout line and cement entered his right eye.
Maintenance employee working in the underground surface shop, eyes were exposed to ultraviolet radiation, diagnosed with actinic conjunctivitis and prescribed medication for pain.
While climbing off of the Mine Cat explosive carrier the miners tools became entangled on the tractor causing the miner to lose his balance and step down hard on the left leg. The miner reported hearing his knee pop and felt pain.

Reported his right arm was sore. He did not recall anything happening to cause his arm to be sure. Through the night his arm sorer and was not getting better so he went to see doctor.
Helper was closing the rotation guard on the drill when his left finger tip was pinched between the frame
Employee was installing a fan extension and the guard slipped and cut the back of his left hand. Seen at the ER, stitches and put on light duty for 10 days. No light duty available, employee put off work for 10 calendar days, resulting in 5 lost work days.
Employee was checking to see if core was inside of core tube with his hands. While his hands were at the end of the core tube, the tube was being lowered at the same time and his fingers became pinched between the tube and the floor.
Employee was removing a pin on the rear axle of a piece of equipment when the pin came out and dropped on his left thumb. He was taken to the hospital where the thumb was surgically repaired and he was released to sedentary duty. He was subsequently placed off work.
While using a folding pocket knife on strap of vent back, knife slipped, cutting left index finger. Received 5 stitches and released to full duty.
Individual was found unresponsive, pulse less, and breathless in wash bay. CPR was initiated on scene. Individual was transported to Northeastern Nevada Regional Hospital via Crescent Valley Ambulance. Initial determination is that this is a health related incident.
Employee had grabbed the casing with his fingers inside of the casing. The Driller opened the chuck, releasing the sub, causing severe lacerations of the employee's hand.

While placing pipe on a scissor lift, pipe fell about 6 inches lacerating the employee's left pinky finger. Four stitches required to close wound.
Employee was removing a header and a high pressure hose hit him in the face. He received 10 stitiches to close a laceration on his right cheek. Released to full duty.
Emp climbing in and out of hopper chipping frozen mud. His right hip was hurting, but no accident. He saw doctor on 12/15 and on 12/20 investigation completed and claim accepted by workers' comp. Emp having electrical stim treatments at chiropractor, making this medical treatment.
Employee's hand slipped while putting inner tube back into the hole. This caused the employee to pinch his fingers between the rod and the latch head. The weight of the tube caused a laceration of the left ring finger and the left pinky finger of the employee.
Driver was climbing up onto work truck to close bay door, stepped off and landed the wrong way causing ankle to break.
While working on a scissor deck, employee was working in articulation and the machine articulated to the left pinching employee between cab and deck. Was taken to NNRH where it was found he had fractured ribs and a punctured lung.
Stepping off a water truck, missed step, fell to the ground. Seen at ER, diagnosed with head injury, released to full duty. Contacted safety on 9/3/10, was having pain in head and left shoulder. Seen 9/17/10,diagnosed with a shoulder strain, released to full duty and referred to physical therapy. He is not progressing. Medical reportable for treatment due to physical therapy.
Employee was installing a motor on the batch plant mixer when his left thumb got caught behind the motor. Thumb was de-gloved from knuckle to the tip. He was taken to NNRH where the tip of the thumb was reattached.
Employee felt what she thought was a rock in her boot. When she took it off at the end of the shift there was a chemical burn from shotcrete that had gotten into her boot. She was later seen at Reno ER when the burn got infected on 4/08/10. She was given a full duty release.
Helper lifting empty tube into rod string felt a ""pop"" in groin area.
Employee was driving haul truck down the west main drift when truck stopped suddenly and jarred employee's back. Was released to full duty 02/10/10 and then was given a restricted duty on 02/13/10. Follow up MRI was completed on 02/25/10 and it was found he had ruptured disc. Employee's injury will become a lost time.

On 07/10/09 emp. was rolling up cable when he felt a hard pop in his right shoulder. He was taken to the ER and was looked over and given a full duty release. He continued to be in pain so he was sent back in for a check up and was then scheduled for a MRI. On 09/09/09 it was discovered during the MRI that he had a torn rotator cuff. He became a Lost Time on this date.
While bending over to pick up a suction hose employee slipped causing him to loose his balance. Sticking out his arm to maintain balance, his hand struck the shotcrete rib.
Miner was washing out transmixer drum and as material was draining out the back it caused the water to splash up underneath the miner's safety glasses.

Driller was breaking off the sub on the caseing, there was a rod in the chuck about 11/2 ft away from the sub. Helper(injuried) went to retreive the sub, as driller released the footclam the rod came down on helpers hand. Helper was struck in the back of the left hand, causing laceration and fracture to little finger.
While spraying shotcrete, cement entered boots and burned feet.

EE was manually re-positioning the forks on a sky trak 8042 fork lift. One fork perched on a frame bracket/divider. The EE dismounted the machine to correctly position the fork, placing his right hand on the fork frame and pushed the hung fork over. The suspened fork impacted his finger on the frame when dislodged
Attempting to secure the quick couple vic clamp on a 2' pipe with both legs under him for support. Left patella displaced.
