EE tripped while carrying a box of booster. Reached out with left hand/arm to break fall. Seen at ER, diagnosed with possible fracture, referred to ortho. Ortho appointment on 1/21 confirmed fracture. EE working restricted duty.

Fall onto or against objects

EE was removing wire from conveyor belt. While pulling wire with force, the wire gave way, ee fell back onto metal stairs, hitting head and neck. Seen at NNRH and then Health. Released with restrictions until 11/1/25, when EE is released to full duty.

Ee was using band saw to cut cap hooks. EE's glove got caught in the band saw and cut EE's finger. Seen at ER, laceration was closed with sutures, released with restrictions. Site offering modified duty.

Ee was removing paste pipe. While removing the chain, the pipe moved, pinching the ee's hand between the chain and the pipe. Seen at Health for a 4 inch laceration to the hand, referred to hand surgeon. Ee released with restrictions site can offer modified duty for.

Ee was replacing transmission mounts. While positioning mounts in holes, the load shifted, pinching the ee's finger. Seen at ER, diagnosed with a left finger fracture, given a splint and antibiotics, and released with restrictions.

Over-exertion in lifting objects

EE was working on a charge hose. While positioning the hose, they felt a sharp pain in left elbow that radiated up to left shoulder. Reported and continued working. Seen at clinic 2/6 and released to full duty. MRI was ordered, done on 3/6 and reviewed on 3/26. EE diagnosed with torn rotator cuff and put on restrictions. They will be having surgery.

Caught in, under or between a moving and a stationary object

EE was hand stabbing a bolt into the face to perform a resin test. Their right hand was in the line of fire inside the centralizer clamp and the clamp came down on their hand. The operator released the clamp, releasing the EE's hand from the centralizer. Seen at the ER, diagnosed with a fracture and laceration, and referred for an orthopedic consult. Released to full duty.

An employee loaded two dumpsters on the bottom deck of the service cage and the top deck with a dumpster and steel tote. The employee shut shaft door and sent the cage to surface. A minute later, personnel heard a loud noise come from the shaft. When the hoist heard bells coming from the console, EE stopped the conveyance. The Supervisor inspected the cage and noticed damage.

EE was working in their office when they felt a bite/sting on right ankle. Continued working and went home for the day. Next morning their ankle was red, swollen, and hot. Seen at the clinic, diagnosed with an insect bite with resulting cellulitis. Given an antibiotic and released to full duty.

Ee was carrying a chair into the UG batch plant office. While stepping up onto the platform, ee tripped, dislocating and fracturing, left ankle. Seen at ER, put off work pending orthopedic consultation.

Ee was in their office at the Leeville admin building. Ee squatted down to plug in the computer and Ee reports their kneecap dislocated. Ee reported dislocating same kneecap at home 2 days ago. No injury by accident, circumstances are not unique to the work environment. Ee given brace for support. Released to full duty.

Struck by falling object

Driller wanted to discuss with the blaster what kind of burn cut was drilled in the heading and what was needed to load the round. They examined the round on the face, and when they turned around to head back to the drill, material fell on them, striking them both from behind.

Driller helper was in the process of adding a quill rod back on the drill string. EE missed placing the rod in the rod roller stop causing the quill rod to fall on the right dorsal area of EE's hand striking the box end of the rods protruding out from the rotation unit.

While being loaded with muck from the 3944-119, haul truck 101 was parked in the 3950-215 cab first. As loader 94 was placing a bucket of material into the truck's bed, a fall of ground occurred 15.5x16x11 releasing material onto the bed/articulation of the haul truck. The operator could exit the cab, evaluate the condition of the back and find a safe path out of the 215 drift.

Struck by falling object

Ee was removing/demobing paste pipe. While removing the pipe, it fell, striking the ee on the right foot. Seen at ER, diagnosed with crush injury, referred for MRI. MRI and subsequent x-ray identified two small fractures, ee released to restricted duty that site can accommodated. Referred for orthopedic consult.

EE was moving air line to lock it out, and the air line came on and hit them on the knee. Reported it on date of injury, but sought treatment on 3/25. Diagnosed with abrasion/contusion/infection to right knee. Antibiotics prescribed to treat infection. Released to full duty.

Struck against stationary object

Ee was walking from the 3893 load center back to the truck. Ee stepped on a rock, it rolled, ee lost balance and caught self. This move caused ee's back to twist and ee felt immediate pain. Ee requested a consultation on 1/25, seen at Golden Health, diagnosed lumbosacral strain, released to restricted duty that site can accommodate.

Ee washing equipment with water hose equipped with ball valve. Ee finished task, closed valve, put down the hose in a manner that caused the valve to open. There was enough pressure to cause the hose to jump, striking the ee in the mouth and breaking a tooth. Seen at dentist, tooth is being extracted. Released to full duty.

EE was pulling pipe at 4050 LVL N underground. EE removed the clamp from the pipe EE was removing, and the pipe next to it fell. EE's left pinky finger caught between the handrail and the pipe. Seen at ER, diagnosed with a fracture to the left pinky, released to full duty.

A breaker in a load center failed as designed which caused some smoke to be released. There was no indication of fire at any time and Leeville and Pete Bajo mines were evacuated as a precaution.

Ee was exiting the compartment beind the cab on loader and slipped on the driveline cover. Ee felt right knee pain. Ee caught self from falling and strained left arm/shoulder. MRI done on 5/12 positive for biceps tendon rupture. Referred for surgery.

A jammer backed over some boxes and signage in a muck bay to pull out of the way a haul truck. It is believed that heat from the engine compartment ignited the boxes and or signage, resulting in a small fire.

Over-exertion in pulling or pushing objects

EE was repositioning a jack under a loader when they felt pain in right shoulder. EE reported that day and performed self-care. EE was seen at the clinic on 2/5, diagnosed with a sprain, and released to full duty. On follow-up, diagnosed with a tear in the shoulder. On 3/23/21 doctor determined tear related to the 12/25 incident. EE will require surgery.

Ee reported wrist pain from repetitive motion. Diagnosed with DeQuervain's tenosynovitis. Accepted as an occupational cumulative trauma on 2/1/21. Ee released to restricted duty.

While removing a 15ft section of old 6"" slick line in the Leeville #2 shaft, the pipe brackets below the 15ft section became loose and allowed the pipe to shift slightly from the shaft wall. The #2 shaft was shut down until the brackets and pipe could be secured back to the shaft wall

Employee was removing a differential and mounting a grease line. The line came off of the mount and fell on the employee's finger.

Ee was walking back to nipper to grab some bolts when EE rolled EE's ankle and fell. EE was seen at the ER and diagnosed with a right distal fibula fracture. Referred to an orthopedist. Released with restrictions which site could not accommodate.

During the pm/inspection of the service hoist, a crack in the yoke for the service hoist brake system was found.

Absorption of radiations, caustics, toxic and noxious substances

Ee was using a screwdriver to clear a blockage in a shotcrete accelerator hose. The blockage was scraped away, and backed up material sprayed the ee in the face. EE flushed eyes, EMTs flushed EE's eyes. EE sought treatment the following day and put off work due to corneal abrasion and med requirements.

Struck against stationary object

Ee climbing ladder on pipe handler and struck head on pipe. Seen at ER diagnosed with head injury, neck/lumbar strain. Released to full duty. On follow up on 07/15/19 ee placed on restricted duty making this reportable.

IP was in the process of closing the service cage at the 4450 station. In that process, IP's left ring finger came into contact between the handle and latch. The injury resulted in a laceration which required sutures to close. IP is currently on restrictions.

Fall onto or against objects

IP was conducting a post blast inspection in the 4200-212. IP was standing on top of the muckpile checking for misfires. As IP was walking down the muckpile, the IP fell forward resulting in laceration to the left wrist. One suture was needed to close the wound.

IP reported experiencing right wrist pain. IP sought medical treatment and was put on restricted duty, and was diagnosed with Ulnar Carpal Instability.

Struck by flying object

IP noticed air and water headers come down off the rib in a muck bay. IP decided to move the water line first by bending it back on itself and stuck the header into the ground. As the IP was in the process of retrieving the air line, the water line came dislodged and the valve turned on. The water line struck the IP's right forearm resulting in a fracture.

At 11:58 a.m., a power outage occurred in the surface and underground of the mine. When the outage occurred, three miners were on the #1 shaft conveyance performing a hoist inspection. The miners were 40 ft. from the 1350 station when the power went out.

While the driller was cycling a core tube down, standing on the helper deck a 5' rod in the rod rack started to slide to the other end of the rod rack. The driller turned around and tried to stop the sliding rod when 2 other rods slid against the first one pinning the driller hand between the rod and rod rock causing lacerations to the left fingers.

Installing Underground Bulkhead Panel. Panel slipped from the left side of the forklift hitting the ground then bouncing and striking the employee in the abdomen area.

IP was using a pressure washer in an attempt to flush plugged material out of a grout line. The pressure washer backfired during use and something pierced into the glove and hand of the IP's left index finger. IP sought treatment which required sutures to close and placed on modified duty.

Employee was sitting in the lunch room at the end of shift when ee stood up and lifted arm ee shoulder popped out.

Struck by flying object

Employee utilized a 20lb sledge hammer to assist in the removal of teeth from a loader bucket. In doing so, a piece of metal from the tooth came off, striking the employee in the left thigh. 2 sutures were required to close the wound.

Fall to the walkway or working surface

Employee was using a pallet jack when EE slipped and fell to same level. Initial chief complaint was EE's left ankle. Next day the chief complaint was the upper left shoulder. Employee self cared and never lost/missed any work days. Became reportable when EE went to the Physical Therapist on 4/9/18.

Fall from headframe, derrick or tower

While operating bolter UDR 053 at the face in the 4450-604 the operator went to egress from the bolter to change a bit. When descending the operator tripped and grabbed the handle of the door to catch the fall. The result was a dislocated right shoulder.

Employees were replacing liners in 4315 feeder #1. Employee was adjusting a top liner with an alignment bar to align bolt holes to secure with bolts. While adjusting the liner up the pry bar slipped out of the bolt hole it was in and allowed the liner to fall. The employee's finger became pinched between the two liners.

Ongoing rehab in the 4000 LV. Operator was peeling lt. rib and was making operator's last cleanup pass to bring in the bolter. Rocks began hitting operator's cab and then the back came in on operator. This area is known to be faulty ground. Operator was able to make visual contact with coworkers.

At the Leeville mine in 4000 Level North a rib that was in the process of being re-supported failed. This temporarily trapped a contract employee with Small Mine Development (SMD) in a mucker underground. The employee was successfully rescued at 6:08 am, uninjured.

Employee was stepping over an enclosed auger when ee felt discomfort in right knee. EE went to the doctor that same day and was diagnosed with a mild strain and given a full duty release. On 3/8 ee went back to the doctor, given the same diagnosis and a restricted duty release.

Injured Employee (IE) was cleaning up spillage around Rock Breaker 1 at the end of the shift. Upon closing the barrier gate to the grizzly the IE caught their right ring finger between the gate and a deflector plate. X-ray revealed a fracture to the tip of the finger and sutures required to close laceration.

A compressor located underground overheated and caught fire. Initial attempt to extinguish the fire was not successful and the mine was evacuated. There were no injuries as a result of this event. When the mine rescue team reached the compressor the fire was out.

The service cage was being brought to surface with a paste pipe rack loaded on the second deck. The rack contacted the sub-collar and damaged the divider between the first and second decks on the service cage. There was no damage to the shaft.

Employee was scaling a rock out of the back when a rock began to slide along the scaling bar. The employee attempted to deflect the rock and received a laceration to EE's left hand which required sutures to close.

Injured employee was cutting collar pipe when the knife became stuck. In the process of freeing the knife the employee cut left forearm resulting in a laceration requiring 3 sutures to close.

Employee was working a cubex drill. EE placed one hand on some hydraulic hoses then activated one of the controls. When the hoses moved from the pressure EE felt a prick to left hand in the webbing between the index finger and the thumb. The next evening the employee reported the incident and was taken to the hospital as a precaution for infection.

Employee was investigating a needed repair to a headlight on a tractor when the engine cover shifted causing a laceration to left index finger requiring sutures to close.

A shaft crew employee was attempting to clear an obstruction from the belt magnet. The belt caught the scaling bar the worker was using and pinched the employee's fingers between it and the support structure holding up the belt causing a finger laceration.

Employee was grabbing a wire off the stack that was leaned up against the rib. Employee took a step back and the whole pile fell on top of employee knocking employee down and landing on top of employee.

Mechanic was removing the right rear tire/wheel assembly from a mucker. After removing all of the lug nuts the tire slid off the spindle, tipped over and fell onto the mechanic resulting in a minimal, nondisplaced fracture to the pelvis.

Employee was removing a bucket and shovel from a tractor when left leg went out from under ee. The employee was diagnosed with internal derangement of the left knee and taken off work. There were no witnesses to the event.

While Shotcrete Sprayer was ready for tramming and boom folded back, operator forgot to bleed the system of air and water. The nozzle was pointed in ee's direction and blew accelerator, water and dirt to the side and back of the head at 90 psi.

Hoistman identified that was having issues with the service cage positioning on the hoist monitor. The empty service cage was then sent in manual to the 4450 station, a known location, to verify the accuracy of the position indicator. In the process the service cage traveled past the station and contacted the shaft bottom damaging a guide wheel and chair linkage.

Employee was replacing a hoist brake actuator when EE's left hand got caught between EE's wrench and a piece of the brake assembly. After removing EE's glove the employee noticed a laceration on the webbing between EE's thumb and index finger. The laceration required sutures to close.

Employee pinched EE's left index finger between a 2 inch airline and the rib causing a laceration to the tip of the finger requiring sutures to close.

Struck by falling object

Employee was stocking a heading with bolting supplies when EE lifted EE's hardhat above EE's head to wipe the sweat off EE's forehead. At the same time a bolt that was leaning up against the rib tipped and struck the employee on the head resulting in a laceration requiring sutures to close.

A fall of ground was discovered at the 4200-201 intersection. At the time of discovery there was no work at or beyond the affected area. No equipment was involved and there were no injuries as a result of this event.

Employee was dismounting a Caterpillar Jammer in the 4490 and while stepping down, stepped on a rock. The rock shifted as well as the employee resulting in a severe sprain to the foot and mild sprain to the ankle. No broken bones.

Employee started to step over stinger rod, slipped and caught himself before falling. This resulted in lower back pain. Employee initially received a full duty release. During a subsequent follow-up on January 8, 2016, the employee was given restricted duty release

EE was cutting a piece of ready rod (3/4"") in diameter with a grinder, upon completing the cut of the ready rod, the piece came up and struck EE on chin causing a laceration.

Employee was maintaining a sump when water came over his rubber boots. The employee's calves became irritated and when he requested that he change into dry clothes about an hour later both calves were reported as red and painful. Employee sought medical attention and was given a restricted duty release.

EE was installing a 6"" piece of steel pipe to concrete wall. Pipe was being guided into position when it slipped and pinched right index finger between pipe and wall. 15 Sutures to right index finger on the pad of finger.

Employee was diagnosed with a noise induced hearing loss in the left ear. (Per MM #2 on 11.12.2015, EE diagnosed with a standard threshold shift in both ears.)

On 09/16/2015, employee was loading a bolter carousel with 12 foot bolts when the bolt being loaded tipped backwards. At the time the employee felt pain in the left wrist. On 10/29/2015 the employee was evaluated at a medical facility and given a brace to wear while performing her regular duties.

In the 4450 the back came down at the intersection of a load center. The affected area was estimated to be 25 feet X 20 feet X 2 to 5-6 feet deep. The fall did not impede ventilation or passage in the drift. Only access to the load center.

On 08/18/2015 a sill failure in the 4260-135 occurred as a result of a fall of ground in the inactive 4200-135. A sill failure is not reportable, the ground fall did not occur in an active area and there were no injuries therefore a call was not immediately made to MSHA. On 11/16/2015 an order was issued for not reporting the event within 15 minutes.

While tripping in HRQ to suspend a cutting tool the pipe wrench slipped and the employee stumbled back rolling his ankle.

Employee was in the process of loading a face round when he inadvertently stepped on the foot pedal which initiates the prill hose. As prill exited the hose a small amount of prill went under the employee's safety glasses and struck his right eye causing a corneal abrasion.

Periods of dusty environment from dust coming down the Leeville decline lead to a mechanic getting dust in the eye making is irritated. He was wearing contacts, so dust collected under contacts.

Inhalation of radiations, caustics, toxic and noxious substances

Employee claimed he had symptoms of CO overexposure. He was taken to NNRH and was given oxygen. The investigation determined the CO levels in the area were well within MSHA guidelines. It was also determined he is a smoker and hadn't eaten well prior to shift and consumed large amounts of soda. The attending physician diagnosed him with elevated carboxyhemoglobin.

Employee was approaching another employee who was grouting a bolt and at this time the grout hose ruptured and grout sprayed him in the face and eyes. Even though he was wearing safety glasses the grout got in his eyes causing some burns to the eyes.

The employee was tightening a bolt on a drifter shoe with an impact gun. As the bolt tightened, a pin hole developed in a weak portion of the shoe as residual cleaning solvent in the bolt hole pressurized with the bolt being installed. This caused the remaining solvent in the bolt hole to inject into the employee's left middle finger.

Employee stepped off a loader and felt pain in his right knee. He was not evaluated by a doctor until 07/08/2015 when he was prescribed physical therapy for the knee.

Employee was removing a differential plug when he felt a strain on his right shoulder. Employee was evaluated by doctor on 6/9/2015. Employee was referred to physical therapy as needed. On 6/18/2015 employee attended his second PT session which resulted in a series of professional treatment.

Employee was adjusting the position of a survey tool reel on the back of a truck when he pinched his left pinky finger between the reel frame and the tool box on the truck. The resulting laceration required sutures to close.

On 04/16 employee reported that he felt pain in his right hip while stepping out of a light vehicle but did not wish to seek medical attention. On 04/23 employee reported hip was almost pain free when he reinjured at home doing yard work. On 05/01 employee sought medical attention and was prescribed physical therapy to aid in healing. Employee continues to work at full duty.

Employee was driving a haul truck when he reported driving over a rough section of road, after which the employee reported experiencing pain in his back and leg.

While the lower skip work deck with 3 personnel on the conveyance was exiting the dump pocket the dump arm on the skip became jammed. An ""all-stop"" was given, the condition evaluated and it was determined that the dump arm would need to be cut to free the skip. The task was completed in approximately 1.5 hours.

Employee was attempting to marry a fan silencer to a fan. In the process he pinched his right index finger between the silencer flange and cradle.

Employee was working as a longhole drill helper when he reported numbness in his left thumb and index finger which was radiating up to his shoulder. On 01/13/2015 the employee sought medical evaluation where he diagnosed with a neck strain and was given a restricted duty release to not wear his fall protection harness.

Accident type, without injuries

While investigating decreased air flow in the 4540, a rib failure was discovered at the top of twin ventilation raises on the 4675. The raises are located behind a permanent bulkhead which can only be accessed when a sealed inspection cover is removed. Although the condition was in an inactive area as a courtesy was reported to MSHA. As a result a 103K order was issued.

Struck by falling object

Employee was repairing a damaged metal door. When he hit the door with a double jack, foreign material from above went behind his safety glasses and into his right eye. Although the employee immediately rinsed his eye, the following day the remaining foreign material was removed by an optometrist.

In the 4510 Level North, the right rib came in. The affected area was estimated to be 17 feet x 12 feet x 5-10 feet deep. The affected area did not extend into the back nor did it impede passage or affect ventilation.

Over-exertion in pulling or pushing objects

While attempting to bend a wire mat, the wire mat sprung free, his left arm went straight suddenly, felt pop in left arm, began to feel pain in left arm.

A fall of ground occurred in the 4100 Station East drift. An 8'x8'x3' deep section of back fell into the access as contract miners were working to relieve the basket. The miners were able to safely pass by the affected area where ground support was still fully functional. The affected area did not go beyond anchorage, affect ventilation or impede passage.

Shaft plumb bob assembly being pulled up via it's winch. Cable crosby clamp caught on bottom deck, as employee went to investigate it suddenly came loose and snapped up and stuck employee on right side of face.

During the service cage tests at the beginning of shift it was noted that a recently replaced brake cylinder was not disengaging smoothly. It was decided to take the service cage out of service and conduct additional maintenance on the hoist braking system. No personnel were trapped and the Chippy remained in service throughout the repair.

Employee was moving wire in preparation to start bolting when he felt a pop in the inside of his right arm near his bicep. After being evaluated the miner was given a restricted duty release.

Employee was struck by a piece of concrete remnant released from the wall.

Employee was traveling up the decline when his batch truck hit a pothole which resulted in the employee hitting his head on the cab ceiling. Medical evaluation determined the employee experienced a cervical thoracic lumbar strain and was placed on restricted duty.

Employee was changing an air conditioner filter in the ceiling. At the end of shift the employee was waiting for the cage to arrive when something got in his eye. The employee flushed out his eye but continued to feel discomfort in the morning. When seeking medical attention nothing was found in the eye and the eye was not infected, however the employee was given a precautionary prescription to prevent infection.

Employee was welding from a prone position and on his left side when something fell into his right eye. When medical consultation was sought, treating physician gave a prescription for antibiotics as a preventative measure. There was nothing embedded in the eye nor was their any sign of infection. Employee was given a full duty release.

Employee opened a storage door on an underground crane to retrieve a strap. When he reached for the strap the door closed on his left hand. On 12/2/2013 the employee had the hand X-rayed which revealed a non-treatable hairline fracture to his left pinky finger.

On 11/01/2011, employee hit a cut bolt with a hammer causing a metal sliver to shear off. The sliver entered the employee's upper lip. Employee did not wish to have sliver removed at the time. The employee then found that an unrelated non-occupational procedure would not be possible with the sliver in place. The employee had the sliver removed on 11/09/2012.

Crew was tripping back into the drill hole with 5.5 pipe, it was the start of a new row on the pipe truck. Helper went to pull the pipe back to line it up with the tilting top-head on the T130 rig; causing the J-Hook to come out of the slot on the pipe and the pipe fell back to the bed of the pipe truck where it struck the employee on top of the left foot.

The employee was getting onto the mantrip/ Kubota tractor when she fell on the ground. The employee's boots were muddy and she lost her footing when getting onto the Kubota.

Employee was unloading a bundle of drill steel from the cage. The bundle was place next to the rib with the forklift. The bundle was not sitting square on the pallet, so the employee grabbed one end to move it closer to the rib. Attempts were made to extinguish the flames, but were unsuccessful so the mine was evacuated. All personnel were evacuated with no injuries and the brass board was cleared at 7:45 pm. Several mine rescue teams were utilized to put the fire out.
